CHAPTER 42

a) When a person is suffering from either Uremia or Azotemia, there is a kidney malfunction. The kidney can develop problems for various reasons such as lack of exercise and taking unhealthy foods. When levels of Urea and Creatinine rise in the blood, it signifies kidney problems, Uremia and Azotemia result from this (Grossman & Porth, 2014). The two diseases, however, differ in a number of ways.

Uremia is characterized by the presence of Urine in the blood. It is a serious condition as it comes with acute kidney failure. During this stage, the rate of glomerular filtration is very low, which results in an increased urea concentration in the plasma. Uremia is chronic, and mostly the damage it presents to the kidneys is irreversible.

Azotemia, on the other hand, is characterized by nitrogen in the blood. In Azotemia, key symptoms of kidney failure are yet to manifest. However, creatinine and urea nitrogen are on the rise. If unchecked, Azotemia can result to Uremia, and it is, therefore, a stage towards kidney failure (Grossman & Porth, 2014). Unlike Uremia, Azotemia can be corrected with early identification with subsequent medical intervention.

b) Uremia’s presence in a person means that the individual’s kidneys are severely damaged. Symptoms of kidney failure are noticeable during this time. The following are the symptoms that are associated with it. Minimal tolerance to exercise, loss of appetite coupled with weight loss, fluid retention, which causes an individual’s body to swell, nausea and vomiting, cramps in the legs, and headache (Grossman & Porth, 2014). Uremia patients also have trouble concentrating and lose interest in undertaking daily activities.

The patient experiences extreme tiredness and can yield uremic frost on the scalp if dialysis is not done metabolic acidosis and pericarditis results, which are characterized by lethargy, coma and finally death.

Creating a Plan of Care

Utilizing the information you have gathered over the weeks regarding the specific illness group you identified, this week, you will create a plan of care for your chronic illness group.

Create the plan in a 4- to 6-page Microsoft Word document (the 4–6 pages include the holistic care plan). Include the following in your plan:

• Start the paper with a brief introduction describing the chronically ill group you selected and provide rationale for selecting this illness and the participants. Clearly identify the Healthy People 2020 topic chosen and why this topic was chosen.
• You will want to compile the information gathered from Weeks 1–4 over 2 to 3 pages   (((((SEE ATTACHED DOCUMENTS)))). . This should be in APA format and paragraph form. This is not to be copied and pasted from previous assignments. It is to be a summary of each week. 
• The paper should include the care plan for your chronic illness group organized under the following headings:
  • Nursing Diagnoses
  • Assessment Data (objective and subjective)
  • Interview Results
  • Desired Outcomes
  • Evaluation Criteria
  • Actions and Interventions
  • Evaluation of Patient Outcomes

A vulnerable population is a group of people who need special considerations in regard to their health and overall welfare (Falkner, 2018).Vulnerable populations are individuals who are not able to advocate for themselves due to physical, cognitive or mental disability, emotionally unstable people, children and incarcerated individuals.These populations usually have several social determinants of health(SDOH) that contribute to their vulnerability.These SDOH can include poverty, socioeconomic status, race and ethnicity, sexual orientation and no insurance or the underinsured.

 In the United States there are many vulnerable populations.One group that gets a significant amount of media coverage is the lesbian/gay/bisexual/transgender/questioning (LGBTQ) population.The inequality and hardship this group often encounters within the medical community and the higher instance of medical conditions such as Hepatitis B, HIV and AIDS is what makes this population considered vulnerable (Falkner, 2018).In the US, it is estimated that 10 million adults identify as LGBTQ (American Nurses Association [ANA], 2018).Many individuals within the population have reported prejudice from medical providers, leading to reluctance to seek care or delay in treatment due to the bias experienced.The stigma in society, discrimination and failure to recognize the unique civil and human rights deters individual self-determination and access to care which causes negative health outcomes and overall increase in morbidity and mortality (ANA, 2018).All nurses, not just CHN need to understand their own preconceptions and bias when caring for this vulnerable population.Ethically, nurses have a responsibility to provide safe, competent care to all individuals regardless of age, sex, sexual preference, race, ethnicity or socioeconomic status.
