JC is an at-risk 86-year-old Asian male with numerous factors that impact his ability to seek adequate care. One major source of concern is his physical and financial dependence on his daughter that is unable to devote ample time and money to him. JC has numerous comorbidities that require consistent medication therapy. JC suffers from hypertension and takes lisinopril daily for management. Failure to treat the hypertension would result in him being at greater risk of suffering from a myocardial infarction or ischemic cerebrovascular accident related to atherosclerosis of various arteries due to continued hypertension. JC also appears to be at risk for developing depression. His feelings of being a burden to his daughter signal that he may already be suffering from the effects of depression related to his overall condition. It is imperative that the clinician further assesses his feelings and his fears to determine his emotional state. Demirtürk and Hacıhasanoğlu Aşılar (2018) stated that patients suffering from depression are at much greater risk for non-adherence to antihypertensive medication therapy, placing them at risk for the complications listed above.

JC also suffers from gastroesophageal reflux disease and takes prilosec daily for management. Failure to treat this condition could cause gastrointestinal ulcers and severe gastrointestinal pain. JC also suffers from vitamin B12 deficiency and gets vitamin B12 injections monthly to correct his deficiency. Failure to seek treatment monthly would result in increased fatigue and place JC at risk for muscle atrophy and activity intolerance related to a sedentary lifestyle. Finally, JC suffers from chronic prostatitis and takes cipro daily. Failure to take the prescribed medication therapy could result in prolonged suffering from the side effects of the disease, including frequency, urgency, and difficulty initiating urination. Each of these side effects places JC at greater risk for falls, as he would likely have to attempt to make it to the restroom frequently if he is able to do so. A lack of physical ability places JC at risk for not being able to routinely get his prescriptions filled, which places him at risk for the adverse complications of non-adherence to medication therapy listed above for each comorbidity. A lack of funding places JC at risk for not being able to pay for his prescriptions that yields the same risks.

JC’s statement concerning not wanting to be a burden to his daughter serves as proof that the clinician needs to further explore that situation and determine if his daughter is able to be of assistance with JC’s care. If JC is unable to provide self-care and lacks resources that allow him to seek care outside of his daughter that is unable to assist, then the clinician needs to seek resources for JC so that he does not avoid healthcare out of ignorance or embarrassment. Lee, Rhee, Kim, and Ahluwalia (2015) conducted a study of Asian American immigrants to determine their health literacy compared to participants of other descents, and determined that the Asian American participants had significantly lower health literacies than the other ethnicities in the study. The study concluded that for this reason, Asian American patients are much less likely to seek indicated healthcare for necessary reasons.

When interacting with JC, it would be important to be sensitive regarding the self-care activities that he is unable to do for himself. It is apparent the JC is embarrassed concerning his inability to care for himself, as he is stating concern about being a burden to his daughter. For this reason, it is extremely important that the clinician be sensitive regarding his inability to provide self-care so that JC does not shut down emotionally. Also, the clinician would need to be very careful not to break JC’s spirit by focusing on the unfavorable circumstances regarding his inability to provide self-care and the lack of support resources that JC possesses. JC is making an active effort to seek healthcare despite his shortcomings. It is important that the clinician avoid overwhelming JC with the facts of the situation to avoid breaking JC’s spirit and causing him to lose the aspiration to seek necessary medical care. Applauding JC’s effort and perseverance throughout the circumstances could be effective in inspiring JC to continue putting forth his best effort.

To further assess the patient’s health history, I would ask the following questions:

Is there anyone else besides your daughter that can assist you with your healthcare needs?

Can you further explain the aspects of your care that you need assistance with so that I can be in a better position to get you the care that you need?

Do you consistently take your prescribed medications? If not, what keeps you from consistently taking them?

What are your feelings regarding assisted living?

Would you be interested in speaking with a social worker to further explore your specific situation so that we can provide you with the assistance that you need?

            The above listed questions would aid the clinician in establishing a rapport with JC while also providing needed information concerning JC’s needs. Ball, Dains, Flynn, Solomon, and Stewart (2015) stated that it is important to build rapport prior to progressing the patient’s health history for better comfort and results. The questions above introduce the idea of additional support resources without coming across as aggressive or forceful. In JC’s case, it is important that the clinician present these things in a helpful manner to ensure that JC doesn’t begin to feel as if he is a burden to the clinician or any of the support resources.

Statistics show that “approximately 1 in 5 adults in the U.S., or 43.8 million people, experiences mental illness in a given year” (National, 2018a, p. 1).  The adolescent population, ages 13-18, accounts for “21.4% of mental health illness cases” (National, 2018, p. 1) which doesn’t include the “estimated 13% of children ages 8-15” (National, 2018a, p. 1) also suffering with mental illness in any given year.  In the U.S. of the “20.2 million adults living with substance abuse, 10.2 million of them have a co-occurring mental illness (National, 2018a, p. 1) and the price tag for treating mental health illnesses costs “America $193.2 billion in lost earnings per year” (National, 2018a, p. 1).  These statistics demonstrate a very big problem accompanied by very few solutions.  Advocating better care for our mental health patient population will also target the homeless and substance abuse populations as well.  Proper care can help give these people a second chance at life to live independently, secure employment, and to be productive citizens within their communities.

            There are several ways to help advocate better care for those suffering with mental illness including, “joining or starting a local support chapter, becoming a local advocate, volunteer, or becoming an ambassador” (Depression, 2018, p. 1).  Being involved can “make a real difference, not only in your own life, but in the lives of others” (Depression, 2018, p. 1).  A lot of patients that suffer with mental illness aren’t being heard or acknowledged for the treatment they desperately need, so it’s up to the general population to join and support this type of patient care and to show that mental health care is just as important as physical health care. 

            Advocating for those suffering with mental illness “improves the individuals’ understanding of their situation, enables their views to be heard, ensures that they have the opportunity to be partners in their care and increases their autonomy” (Varghese, 2015, p. 1).  Advocating also “promotes the rights of those who suffer discrimination because of their age, disability, sexuality, gender or culture, and it can ensure the quality of the care system” (Varghese, 2015, p. 1).

            The only way that change can occur is to make the issue aware, give it a face for people to see.  The stigma on mental illness needs to come to an end.  When a patient breaks their arm, they can go the hospital without judgement and get proper medical treatment and within a few weeks their arm will heal and be useful again.  However, when a patient suffers from a mental or psychotic break, they go to hospital and are judged from the beginning and then drugged until they act in a way that society will accept them.  A large population of mental health patients are diagnosed with illnesses they were born with or due to trauma they inherited them throughout life.  Instead of offering proper treatment and acceptance, healthcare providers are easily frustrated and judgmental because they either don’t care or don’t understand.

            Ways to help promote change and encourage education is by asking your local governor to make a proclamation, take action on advocacy issues, ask businesses and landmarks to light up green to raise awareness, address culture barriers, speak with teens, hand out ribbons and education, host an event, volunteer, implement a free community outreach program, become a program trainer, or help run a support group (National, 2018b, p. 1).  The end goal needs to be better and reliable care for our mental health patient population and to educate enough to end the stigma, so those suffering will no longer feel the need to hide and be scared of what others will think.

There are many views on mental health. Some cultures view mental health issues as a sign of familial conflict. Or as a sign to receive divine intervention or forgiveness. While other cultures have a more negative view such as demonic possession or a supernatural phenomenon (Choudhry et al., 2016). In America about one in four adults are diagnosed with some type mental health disorder (CDC, 2014). This can be from substance abuse, anxiety disorders or mood disorders. The commonality of mental health is an issue that will be faced in practice as well as the negative or unknown view of the public on mental health. This is were becoming an advocate and educator are important to make change for patients dealing with mental health issues.

Education and Mental Health

One of the foundational duties of being a nurse practitioner is to educate. This includes not only the patient, but the public. By educating the public on the facts of mental health is a start. Another beneficial strategy is educating the public on the signs and symptoms of mental health. For example, a mother may notice her once vibrant and social teenager is now distant and tearful. The mother may believe her child is “going through a phase”, when the teen is suffering from depression with suicidal ideation. Or when an individual has a stressful job and uses alcohol to help him “relax”.

Becoming a Social Change Agent for Mental Health

Education of the public can be done in many ways. Handing out pamphlets is a helpful approach, but this approach is limited. Venturing to different places, such as schools, offices and churches and speaking with individuals about mental health are ways to reach a wide variety of people. By having local mental health awareness sessions where people from all different walks of life come to learn about the facts of mental health is an initiative that allows people to ask question about mental health issues. This also allows for discriminatory or perceptions of mental health disorders to be addressed. Through education, the stigma associated with mental health can be changed.

Overview: 

For your next major assignment, you will write the first draft of your researched argument. You can choose your own topic; the only requirement is that it your topic somehow involves Memphis. 

As you choose your topic, make sure you choose something you are genuinely interested in, as you will be working with this topic in different forms (draft; new media; final draft) for the remainder of the semester.  While there are obvious choices (The Blues, Beale St., Elvis, BBQ, etc) you can localize just about any topic you care about. For example, if you are interested in healthy eating, then you could do research on food deserts or school lunch programs in Memphis. If you are interested in business, then you might do research on FedEx or Autozone and use that research to argue that Memphis is a great place to start a business. 

Purpose: 

Successful writing doesn’t happen overnight, especially when research is required. Writing is a slow process, and this assignment is the first of three that will help you develop an argument, improve your writing process, learn to revise, and think about how arguments change when the genre or audience changes. While this is the first draft of a much longer project, you should turn in something you are happy with and think is complete. Then we will move on to the New Media writing project before returning to revise this draft. The goal of this process is to help you understand two things: 

  1. In order to produce your best writing, you need to revise, and revision requires space from your first draft. 
  2. First drafts may seem complete when you just finish writing, but generally first drafts can be improved with changes in organization, additional sources, and even changes in your rhetorical approach. That doesn’t mean you should turn in something that you haven’t work hard on. Even though you will revise, this draft does receive a grade. 

The Process of Developing your Argument: 

Researchers don’t begin with a thesis; they begin with a question, a hypothesis, or even just a hunch. They then do research to either answer their question or prove their hypothesis. You should do the same. You don’t need to know what you want to argue yet:

  1. You need to choose a general topic. 
  2. You need to do research and read about your topic. 
  3. After you have done research, you can begin to develop your argument and figure out your thesis. 
  4. Next you should remember what you learned about developing arguments in the rhetorical analysis assignment. What is the Rhetorical Situation? Who is your audience? How can they be persuaded? What kind of supporting evidence do you need to in order to convince them? Are there places you can go or people you can interview to support your argument? Since you are writing about a topic related to Memphis, you can conduct interviews and do other kinds of primary research to support your argument. 
  5. Draft.
