Position Paper Written Assignment A position paper is a document you could present to a legislator to seek support for an issue you endorse. Present your position on a current health-care issue in a one-page paper, following the assignment guidelines below. You can select your issue topic from newspapers, national news magazine articles, professional journals, or professional association literature; and this can be the topic you choose for your ethical issues debate. Your position paper should: Be quickly and easily understood. Be succinct and clear. Appear very professional with the legislator’s name and title on top and your name and your credentials at the bottom. Condense essential information in one, single-spaced page, excluding the title and reference list pages. Be written using correct grammar, spelling, punctuation, syntax, and APA format. Clearly describe the issue that you are addressing in the opening paragraph. Include 3–4 bullet points regarding why you are seeking the legislator’s vote, support, or opposition. Bullet points should be clear and concise but not repetitive and should reflect current literature that substantiates your position. Summarize the implications for the nursing profession and/or patients. Conclude with two recommendations that you wish to see happen related to your issue, such as a vote for or against, a change in policy, or the introduction of new legislation. Use APA format (6th ed.), correct grammar, and references as appropriate.

Your instructor will send you the first draft submitted by one of your peers through the classroom email system.

Copyedit the other student’s paper using copyediting marks or the Track Changes editing function in Microsoft Word.

Provide feedback related to the key problems in scientific writing and relevance as described in Lecture 4.

This assignment uses a grading rubric. Instructors will be using the rubric to grade the assignment; therefore, students should review the rubric prior to beginning the assignment to become familiar with the assignment criteria and expectations for successful completion of the assignment.

 What are some factors that make some people healthy and others ill? Healthy People 2020 identifies five determinants of health that influence the health of individuals and populations. Healthy People 2020 describes them as “a range of personal, social, economic, and environmental factors that influence health status” (U.S. Department of Health and Human Services, 2018, About Determinants of Health, para. 1). Determinants fall into five categories, including (a) policy making, (b) social factors, (c) health services, (d) individual behavior, and (e) biology and genetics.

The fourth intervention hit a nerve because its unfortunate to know that polypharmacy is prevalent in the elderly population. As a former CNA/Home Health Aide, I would take care of patients that were taking multi-medication. Some were medication that are prescribed for existing health related issues some where not. They would have pain medication from past outpatient surgeries and still take them years later. Some medications were even expired and they still choose to take them. In my research I discovered that elderly patients  tend to have more complicated chronic conditions and respond differently to medication therapy or experience more severe adverse reactions due to differences in pharmacokinetic and pharmacodynamic characteristics, compared with younger patients(Golchin et al.,2015). They may also visit multiple prescriber’s and use multiple pharmacies that will lead to increased risk of medication-related problems through poorly coordinated or duplicated care ( Golchin et al.,2015).

Preventing polypharmacy from occurring in the first place is another area that nurses can help older adults with. They can do this by educating patients and their families about the importance of writing down all medications including OTC, prescriptions, vitamins, and even herbals and bringing this with them to every doctor’s appointment or hospital visit. By doing this the patients entire care team will be better able to prevent prescribing medications that could have a negative impact on the patient. In addition to all of these interventions the nurse must teach the patient to always tell their care team about new symptoms that they feel after taking new medications (Woodruff, 2010). This will prevent interactions from going unnoticed and or blamed on other disease processes which could then lead to more medications being given. It is important to instruct all patients to speak up for themselves and how they are feeling. A journal is another good step the older patient can take to ensure proper documentation of when and what is happening with their medications (Woodruff, 2010).

This week you participate in the second of three clinical discussions called grand rounds. In one of these 3 weeks, you will be a presenter as well as help facilitate the online discussion; in the others you will be an active discussion participant. When it is your week to present, you will create a focused SOAP note and a short didactic (teaching) video presenting a real (but de-identified) complex patient case from your practicum experience.

You should have received an assignment from your Instructor letting you know which week of the course you are assigned to present. 

To prepare: 

• Review this week’s Learning Resources and consider the insights they provide. Also review the Kaltura Media Uploader resource in the left-hand navigation of the classroom for help creating your self-recorded Kaltura video.
• Select a child/adolescent patient from your clinical experience that presents with a significant concern. Create a focused SOAP note for this patient using the template in the Resources. All SOAP notes must be signed, and each page must be initialed by your Preceptor. When you submit your SOAP note, you should include the complete SOAP note as a Word document and PDF/images of each page that is initialed and signed by your Preceptor. You must submit your SOAP note using SafeAssign.

• Then, based on your SOAP note of this patient, develop a video case study presentation. Set aside time to practice what you will say beforehand and ensure that you have the appropriate lighting and equipment to record the presentation.
• Your presentation should include objectives for your audience, at least three possible discussion questions/prompts for your classmates to respond to, and at least five scholarly resources to support your diagnostic reasoning and treatment plan.
