Observe your environment in the health care facility where you have clinical and monitor the site for signs and symptoms of the need for change. Look at the nursing processes in place: do they work efficiently, are infection control practices being done routinely. Anything can be a catalyst for change as long as it adds something positive to the establishment. Then, diagnose the problem that indicates a need for change.

1. Pick a change theory. Lewin’s change theory is used widely in nursing. Despite this fact, there are many other change theories you can use like Spradley’s theory of change, Lippitts change theory or Roger’s theory of change. Pick the ones that best suit the change you are trying to bring about. 

 

2.Put a plan together. Using your selected change theory, develop a plan based on each step of the theory you have chosen. Your plan should address what you will do to solve the problem you have diagnosed. It should include activities that will combat the problem and bring about change, a method to measure the success of the plan such as a quiz and a way to make the change permanent. Be sure to include a timeline or timetable for your change project. It needs to have a beginning and an end.

3. Type a 4-5 page double-spaced, APA formatted professional paper that describes your project and submit to your instructor at the end of the semester. 

The paper has to include:

o An introduction: Brief description of the issue & Purpose of the paper 

o Assessment of the environment which needs to be changed: Current practice, organizational structure, deriving and restraining forces, advantages of change, cost and resources for implementing the change.

Kimberly Morris    2 posts   Re: Topic 10 DQ 2  Over the past couple decades, nursing has taken a turn becoming more knowledge driven (Stevens, 2013). We are now expected not only to just care for our patients but also educate them and increase their health outcomes by recognizing and knowing what care would be best for them. We are now supposed to make recommendations to providers where before we just made sure patients were comfortable and their needs were met. Because the role of the nurse is changing and becoming a much more active participant in patient care, it is important for the nurse to continue to seek new information and advanced practices by utilizing evidence-based practices.  One way in which I will continue to integrate evidence into my practice is by using the literature review strategy. Our facility is currently doing a literature review four step problem solving on falls. We have currently had a large number of patient falls on our floor and we are doing evidence-based research to see if we can find a practice which would work for us to help decrease the number of falls. The second way I will integrate evidence would be to continue to read and research on my own time. If there is a topic which I feel I need to be educated on, I will look at journal articles or attend conferences or trainings to help increase my knowledge of the subject. This will help me in patient care and will give me necessary information to increase the health outcomes of my patients. If I notice an area within my department in which I feel we need to be better educated or I come across information which could be of interest to my coworkers, I will share this information with them in our weekly huddle or our monthly staff meetings. If I start doing this, maybe it would encourage others to do the same, thus encouraging increased use of evidence-based practice.  Obstacles which would challenge this is lack of interest from my coworkers. If they do not buy in to the idea of sharing knowledge, then they will not listen or learn. But I feel we have a team which is always looking at how to grow and how to become better. The manager of our floor is a driving force behind this. She is continually wanting us to move forward in our careers and encouraging us to learn and become better nurses. This assignment requires you to interview one person and requires an analysis of your interview experience. 

Part I: Interview

Select a patient, a family member, or a friend to interview. Be sure to focus on the interviewee’s experience as a patient, regardless of whom you choose to interview.

Review The Joint Commission resource which provides some guidelines for creating spiritual assessment tools for evaluating the spiritual needs of patients. Using this resource and any other guidelines/examples that you can find, create your own tool for assessing the spiritual needs of patients.

Create a survey to assess the subject’s spiritual need during the interview. The spiritual needs assessment survey needs a minimum of five questions that can be answered during the interview.  During the interview, document the interviewee’s responses.

Submit the transcript of the interview. The transcript should include the questions asked and the answers provided.  Be sure record the responses during the interview by taking detailed notes. Omit specific names and other personal information from the interview. 

Part II: Analysis

Write a 250-500 word analysis of your interview experience. Be sure to exclude specific names and other personal information from the interview. Instead provide demographics such as sex, age, ethnicity, and religion. Include the following in your response:

1. What went well?
2. What would you do differently in the future?
3. Were there any barriers or challenges that inhibited your ability to complete the assessment tool? How would you address these in the future or change your assessment to better address these challenges?
4. Describe the spiritual experience you had with your patient, family member, or friend using this tool. How does this tool allow you to better meet the needs of your patient?
5. Did you discover that illness and stress amplified the spiritual concern and needs of your interviewee?  Explain your answer with examples.
