Telehealth, therefore, aims at bridging this gap and promote equity even in the health world. This way, an expert can offer his/her services through telehealth to the affected and under-privileged populations. 

My telehealth effort will entail the use of websites and social media sites to provide people with health advice and services. The creation of a professional website will be essential to achieve functional telehealth. The website will contain information about my practice, the areas I specialize in, and the services I offer. It will also provide a message, call, and email icons, which will allow individuals to contact me for medical services directly. Since many people are connected to the internet, they can utilize the internet to access the website and, thus, acquire health services comfortably without having to travel to distant places while seeking medication. Social media sites and SMS functions will also be utilized in my telehealth efforts. They will be done to cater for the rural and poor populations that cannot access the internet and the website. Through providing a toll-free SMS number, patients will be able to send messages about their concerns, and this will allow me to provide sound advice to them. This way, my telehealth services will be provided to a larger population (including the rich and the sick individuals). They will also allow me to offer quality telehealth services. 

To determine if the information presented was understood, the telehealth information will provide in straightforward terms that can be easily understood by ordinary individuals. They will include breaking down medical conditions into simple language that can be understood by non-medical individuals. The information will also provide to a client, and he/she will be requested to state whether he/she understood the whole information well. They will also be asked to state their concerns (either through the mail, messages, or phone calls) and questions, which will then addressed accordingly. Where possible (for instance, on the website), graphic images will be used in passing out information. They will enhance understanding because visual signs can be interpreted well alongside wordings and explanations. In offering telehealth services, the practitioner will continue to work closely with the clients throughout the medical plan. They will enhance a better understanding of information and, where clarity is required, the practitioner will be able to provide sound advice. Feedback from the clients about the services offered will also note. They will allow telehealth service providers to gauge whether the information provided was or was not understood. From this feedback, improvements will be made 

 Various resources will be needed while offering telehealth services. First, a working office will be required. Although providing services through telecommunication facilities, a practitioner will require a physical office to operate well and have all his/her equipment stored safely (away from dust, damage, and theft scenarios). Technological resources required include phones, computers, carts, power backups, and internet services, and software (the University of Arkansas for Medical Sciences, 2019). They will allow a practitioner to set up a functional telehealth facility and also offer services even during power shortages (as there will be power backups such as generators). Functioning internet service is necessary to promote service provisioning in an online platform. Telehealth services will be accessed by various individuals (people of a different race, ethnicity, and countries), interpreters will be required to ensure that the telehealth service doesn’t cause further discrimination due to communication barriers. Where possible, translators will also be employed, and this will ease the provision of services. 

Since I will not be face to face with the clients, I will make up by first providing them with quality services, which will fill the gap of not meeting face to face. The provision of quality care will ensure that clients gain excellent services for their time spent in the telehealth platforms. I will also set up Skype and video call accounts that will at least provide clients with a face to face feeling during consultations (Goodridge&Marciniuk, 2016). This service, however, will be provided for urgent cases that can’t be solved by explanations through mails and messages. For instance, when a client fails to understand what is required of them, video calls and Skype will allow for demonstrations, which will then enhance understanding. 

In conclusion, telehealth is an essential concept in the field of medicine, and it allows practitioners to provide quality medical services to their clients. Since telemedicine is done on an online platform, some of the resources required include phones, computers, internet services, power backups, and a functional working office to handle all these equipment. It is important to provide simple and understandable information to clients to ensure that they follow instructions and administer medication as required. Without clarity, telehealth wouldn’t be of much help. Feedback from clients should be taken seriously as this will allow practitioners to gauge whether the services being offered are quality, and the clients also understand the information given. To fill the void of not having face to face interactions with clients, practitioners can, once in a while, hold video calls with clients, especially during pressing cases. 

You will do a presentation to the class on your telehealth effort. This presentation will be at least 10 minutes and not more than 15 minutes. You should provide the information from part 2 along with a short demonstration of your telehealth effort.

I totally agree with Sarah Palin’s sentiments on the Democrats plan for healthcare. I think what the Democrats are pushing for claiming to reduce healthcare is downright discriminatory and it will do more harm than good in the long run. The truth of the matter is, some people need more care than others. For a fact, such a system will not favor vulnerable populations like those with chronic illness, the elderly and the disabled. There are currently more than half a million people in America living with various forms of disabilities. Such people continue to remain in waiting lists to receive services due to the fact that Obamacare urges states to expand Medicaid coverage to able-bodied adults instead of prioritizing the services to those who need them the most (Hilberg, 2015).

My judgment is informed by a commitment to advance care of planning. For instance, recent estimates from the Urban Institute suggest that a big percentage of the individuals eligible to Obamacare Medicaid expansion are adults without children in their prime working years. This means that Obamacare does not provide medical services to the most vulnerable. Another report published by the Foundation for Government Accountability showcases how Medicaid rolls have erupted under Obamacare and also how individuals with disabilities have suffered owing to this.

The Role of Nurses in this Plight

It is important to note that nurses are always in the frontline in boardrooms and in leading healthcare organizations. Given their high level of education and their experience, they have the intelligence to observe, validate and the network to transmit messages that advocate for healthcare reform values. Taking this into consideration, it is not right for them to endorse a legislation even when it is not perfect. If they leverage their numbers and use their influence efficiently, they will be able to support only the legislative changes that strengthen the system instead of those that weaken it (Béland, Rocco & Waddan, 2016).

The above also applies to the recently failed American Care Act. They must rigorously oppose legislation that could undermine or compromise the goals of the healthcare system. Nurses must, therefore, make an extra effort to communicate, listen and collaborate with the new congressional leaders. I believe that by putting their voices together, they can become part of the solution when it comes to directing policy through political goodwill (Béland, Rocco & Waddan, 2016).

           This is a timely topic as this year the CDC pointed out that the risk of black* women dying in pregnancy is three to four times that of white women (CDC, 2019). ACOG (2015) discusses the consistent and prevalent disparities in obstetrics and gynecology. Healthcare access is one of the documented issues. ACOG reminds us that the United States is the only country that has a market driven health care system rather than the view that healthcare is a right that every citizen should have (ACOG, 2015). In 2013, 59% of black women lived in areas where Medicaid was not going to be expanded (ACOG, 2015). Also documented is the biases and stereotyping from a provider point of view (ACOG, 2015). Demographic and social biases have been shown to influence clinican’s decisions regarding contraception and pre-natal care (ACOG, 2015). Could the mistrust of many black women with the healthcare system have anything to do with forced sterilization in the past? (ACOG, 2015).

These disparities are nothing new. In 2011, Cox, Zhang, Zotti, and Graham discussed racial disparities and unfavorable birth outcomes. The study referenced that fact that black women consistency received less than adequate pre-natal care from providers. In addition black women had a greater chance of premature babies, babies with low birth weight, and babies who die in childbirth (Cox, Zhang, Zotti, & Graham, 2011). We have the power, as Nurse Practitioners to encourage and provider early and consistent pre-natal care free of bias and judgment. With consistent care, we can assist with nutritional and psychosocial counseling as well as assisting with modifiable risk factors such as alcohol, drug use, or cigarette smoking in pregnancy (Cox, Zhang, Zotti, & Graham, 2011).

Given the United States’ climate of racial inequality and health disparities, our patient, which is an African American woman is more likely to be exposed to stress and complications such as preterm labor, preeclampsia, depression, fetal demise or fetal growth restriction during the pregnancy. A healthy environment, financial stability, healthcare, education, and social community context are essential during pregnancy (Mohamed et al., 2014). In this case study, the patient is a 32-years-old, African American single mother, has three children from previous relationships, is financially unstable, overweight, with preexisting conditions such as hypertension and at risk for gestational diabetes. She has two jobs that probably does not offer benefits or insurance coverage. The patient is more likely to experience hypertensive disorders of pregnancy that may be attributable to pre-pregnancy hypertension. Her BMI is already elevated, which may lead to complications, including preterm birth, fetal death, macrosomia, gestational diabetes, and cesarean delivery.

According to Mohamed et al. (2014), women of color are less likely to have access to vital reproductive health services including screening for sexually transmitted infections and cervical cancer, family planning; and abortion, when compared with non-Hispanic white women. Although socioeconomic status is considered the main leading factor in health disparities, factors at the patient, practitioner, and health care system levels contribute to existing and evolving disparities in women’s health outcomes (Mohamed et al., 2014). In this case, I would inquire on factors that contributed to her delay in OB care and lack of follow-up visits. What determined the patient to have all this gap in care? Was it the lack of financial resources or other factors such as domestic violence? Based on the screening results, the patient should be screened for domestic violence and guided in the process of care based on her needs and beliefs.

 Unfortunately, African American women receive lower-quality health care related to inequities in income, housing, education and job opportunities, which results in higher risk for mortality across the life span for this population (Bryant, 2010). This contributes to racial disparities in pregnancy-related risk factors such as hypertension, anemia, gestational diabetes, and obesity and other conditions such as heart disease, HIV, AIDS, and cancer (Bryant, 2010).

Stress has been linked to one of the most common and consequential pregnancy complications, preterm birth. Studies showed that infants of African American mothers are more likely to be born preterm than infants of white mothers (Mohamed et al., 2014). But, why is preterm birth rate higher in African-Americans?

Women of color are 49 percent more likely than whites to deliver prematurely, and black infants are twice as likely as white babies to die before their first birthday (Mohamed et al., 2014). In this case, the stress, financial instability, lack of nutritional food, and comorbidities may put in jeopardy the patient and infant’s life. Although the Affordable Care Act (ACA) created historical advances in health insurance coverage, millions still go without health insurance each year, many of the people of color (Mohamed et al., 2014).

Low-income, lack of financial resources, maternal pre-pregnancy weight, exposure to stress, and maternal health status prior to pregnancy may lead to fetal growth restriction. Research showed that African American women are more likely to experience fetal growth restriction (FGR), a significant contributor to neonatal morbidity and mortality, than are women of other races and ethnicities (Bryant, 2010). The patient has to be enrolled in public programs such as the Special Supplemental Food Program for Women, Infants, and Children, to avoid food insecurity during pregnancy that may have a beneficial effect on FGR risk among women (Bryant, 2010).

According to Bryant (2010), there are multiple disparities in obstetrical outcomes between women of different race or ethnicities. The author suggests that stress induced by racial and gender discrimination plays a significant role in maternal and infant mortality. According to Kliff (2018), infants in the United States have a 76 percent higher risk of death compared with infants in other wealthy nations and African American women experience the most elevated rates of maternal and infant death. This inequity in health status can be reduced by properly addressing the social determinants of health and advocating for a system of more culturally and linguistically appropriate care for all. (Kliff, 2018).

In this case, it will be our responsibility as health care providers to encourage that all care is patient-centered, culturally appropriate, and listens to women’s needs. This new visit at the office represents a good opportunity for screening and education of the patient in a culturally sensitive manner about steps she can take to prevent disease conditions and any negative birth outcomes.
