Case Management Implementation Plan 

Create a plan to implement case management at your workplace.  What is your recommended plan for the use of case managers in your organization for patients with your chosen chronic illness?  Identify people within your organization who are stakeholders or would support your plan.  Whose support do you need to get your plan implemented?  Identify your goals – what do you hope to accomplish with your case management plan?

Often, starting with our goals helps – it’s a backwards design.  As we think about a case management plan we are proposing for our workplace, what do we want to accomplish?  Remember – goals should be measurable.  For example, if the chosen chronic disease is diabetes, one goal may be:

• Clients will have decreased incidences of hyperglycemia requiring hospitalization

Once we determine what we want to accomplish, we can begin to construct our plan to achieve the goal.  To achieve this goal, our plan may include providing education in a manner the client can understand (being sensitive to cultural needs), access to phone support, or inputting glucose readings into their electronic health record for documentation the case manager can evaluate and use to reach out to the client. 

To put this plan in action, whose support would we need?  Primary Care Physicians, Utilization Review personnel, and the Chief Financial Officer would all have an interest in optimizing patient health and reducing costs.

                               Scholarly Writing and Plagiarism

                Learning is a journey and not a destination. A part of this journey was to use Grammarly and SafeAssign. I will discuss my experience with those two platforms as well as my perception of paraphrasing and plagiarism. The internet is full of resources, so I will be sharing an online resource that supports scholarly writing. Finally, I will share two American Psychological Association (APA) formatting rules that I believe are useful for academic writers.

                I have never used Grammarly before, but I like that I can check grammar in real time. I did not know such a tool was in existence and I will be using Grammarly for all my assignments going forward before sending. However, SafeAssign is not a new tool for me. When I went to Western Governors University, I used TurnItIn, which is similar to SafeAssign. It checks how much of your paper matches other known scholarly works. I wrote a sample paragraph with SafeAssign, and it was a 0% match. I like to see my works matching at 5% or less as a matter of philosophy. It is easy to avoid plagiarism if two things are remembered. Use thoughts that originate from your cranium and cite works according to APA rules and Walden University requirements. During this process, my insight was learning how to paraphrase better. Walden University (2012) states that paraphrasing is beyond simply rearranging a sentence and choosing synonyms. I realized paraphrasing is about turning the whole idea into your own words and then citing the author.

                Scholarly writing is a term that can be confusing for many students. I had no idea what it meant until I researched the topic. According to Jackson State University (2015), scholarly writing is structured and seeks feedback from the audience. It would seem that this is the purpose of using discussion boards in an online learning environment. I believe that this online resource is useful, and I plan on adding it as a bookmark as a reminder of the essentials of scholarly writing.

                As previously mentioned, proper APA style and formatting is essential in scholarly writing. A rule that I was not versed in was related to in-text citations with more than five authors. The APA Manual states, “when a work has six or more authors, cite only the surname of the first author followed by et al” (American Psychological Association, 2010, p. 175). Also, I did not know that the following rule existed. According to the American Psychological Association (2010, p. 170), presenting your existing work as new when it is not is the definition of self-plagiarism. I found this to be an interesting rule that I had not considered before and will make sure I never do.

Explore the functionality of the health information exchange (HIE).  Both regional and local exchanges can provide benefits to both patients and healthcare providers.  Evaluate the benefits and level of adoption of these information exchanges.  Relate these benefits to population health management.  Evaluate the value and usefulness of information exchanges.  Analyze and suggest how patient continuity of care can be improved by access to an HIE.

Week 3: Scientific and Mathematical/Analytical Perspectives of Inquiry Paper

Compose a focused paper that explains and describes your healthcare issue/topic from the scientific and mathematical/analytical perspectives of inquiry. (You will cover two perspectives in one paper.)

Address your general topic by forming and answering two levels of research questions for each inquiry.

• Choose a “Level 1 Research Question/Writing Prompt” from both of the lists below to answer in the paper.
• Compose a “Level 2 Research Question/Writing Prompt” for each kind of inquiry that provides detail, specificity, and focus to your inquiry, research, and writing.
• State your research questions in the introduction of your paper.
• Answer each research question and support your assertions with evidence (research) to form the body of your paper.
• In the conclusion of the paper, briefly review the issues, research questions, answers, and insights.

In the subcategory of practice, the focal point to consider is number 3. As a nurse, one is required to note that the reality of a person’s death is something that is difficult for both the patients and their families. As a nurse, one is needed to be in a position to offer guidance to these people. In this case, he or she should be comfortable discussing about death and offer appropriate comfort to the families. In addition, the nurse should collaborate with other professionals to ensure that the most accurate information is provided. In the subcategory of education, the focal point to consider is number 1. Nurses are required to offer emotional, physical, spiritual, and practical care for patients in each phase of life (Jors et al., 2016). They spend more time with patients nearing end of life in comparison to other health professionals. For this reason, they are required to receive the training and practical experience needed to conduct the duties in the best way possible. In the research subcategory, the focal point to consider is number 1. In this case, there has been a change in medicine towards evidence based medicine. The field of palliative care covers a group of various ailments. Hence, the future of evidence based medicine needs to be diverse to make sure that all patients are able to benefit as much as possible. Lastly, in the administration subcategory, the focal point to be considered is number 1. In most cases, palliative care takes place up to the time when the patient dies. Afterwards, the process comes to an abrupt end. However, it is important to consider that the family members still require help in coping with the demise. For this reason, there should be strategies of ensuring that care is extended to family members after death.
