When you decide to purchase a new car, you first decide what is important to you. If mileage and dependability are the important factors, you will search for data focused more on these factors and less on color options and sound systems.

The same holds true when searching for research evidence to guide your clinical inquiry and professional decisions. Developing a formula for an answerable, researchable question that addresses your need will make the search process much more effective. One such formula is the PICO(T) format.

In this Discussion, you will transform a clinical inquiry into a searchable question in PICO(T) format, so you can search the electronic databases more effectively and efficiently. You will share this PICO(T) question and examine strategies you might use to increase the rigor and effectiveness of a database search on your PICO(T) question.

To Prepare:

• Review the Resources and identify a clinical issue of interest that can form the basis of a clinical inquiry.
• Review the materials offering guidance on using databases, performing keyword searches, and developing PICO(T) questions provided in the Resources.
• Based on the clinical issue of interest and using keywords related to the clinical issue of interest, search at least two different databases in the Walden Library to identify at least four relevant peer-reviewed articles related to your clinical issue of interest. You should not be using systematic reviews for this assignment, select original research articles.
• Review the Resources for guidance and develop a PICO(T) question of interest to you for further study. It is suggested that an Intervention-type PICOT question be developed as these seem to work best for this course. 

By Day 3 of Week 4

Post a brief description of your clinical issue of interest. This clinical issue will remain the same for the entire course and will be the basis for the development of your PICOT question. Describe your search results in terms of the number of articles returned on original research and how this changed as you added search terms using your Boolean operators. Finally, explain strategies you might make to increase the rigor and effectiveness of a database search on your PICO(T) question. Be specific and provide examples.

By Day 6 of Week 4

Respond to at least two of your colleagues on two different days and provide further suggestions on how their database search might be improved.

Select an issue for Community Empowerment that you have observed in your clinical setting and that you have discussed with your preceptor.  This might be a chronic health condition, a recent crisis or disaster in the community, a health-related legislative concern or something else!

• Describe the issue comprehensively.  Is the issue simply and easily explained by community members?
• Will the issue unite community members and involve them in the problem resolution or divide them?
• Is the issue consistent with the long-range goals of the community or clinical setting?
• How does the issue affect community or organizational resources (financial, personnel, supplies, etc)?
• Does the issue have the potential for creating new community partnerships?  Or new adversaries?  Why?
• What is your prediction for the final outcome of the issue?

Develop a 4 to 5 page paper, written to your organization’s leadership team, addressing your selected national healthcare issue/stressor and how it is impacting your work setting. Be sure to address the following:

· Identify and review two scholarly resources that focus on change strategies implemented by healthcare organizations to address the national issue of Full Practice Authority Restrictions for Nurse Practitioners

· Describe this national healthcare issue and its impact on your organization. Use organizational data to quantify the impact

· Provide a brief summary of the two articles you reviewed from outside resources on the national healthcare issue. Explain how the healthcare issue is being addressed in other organizations. 

· Summarize the strategies used to address the organizational impact of national healthcare issues presented in the scholarly resources you selected. Explain how they may impact your organization both positively and negatively. Be specific and provide examples.

 

Capstone Project

Evaluation of Plan Effectiveness Criteria

You spent the last two weeks implementing a portion of the comprehensive care plan you developed for the aggregate. An evaluation of plan effectiveness is now due.

In the evaluation component of the Capstone project, you are expected to evaluate the effectiveness of the intervention.

Some questions you should seek answers to are:

1. What barriers might you anticipate when implementing the intervention as planned?
2. What visible signs of success would you look for (for example, reduced health issues)? Describe them.

The evaluation component of the final paper should describe your evaluation of the implementation. Include responses to points 1and 2 above. The evaluation component of the final Microsoft Word submission document should be a minimum of 2 pages.

The complete summary document should use the Mobilize, Assess, Plan, Implement, Track (MAP-IT) steps used in pulling together your project.

Over the past nine weeks, you selected an aggregate and conducted a risk assessment of its health, developed a care plan to address those health risks, planned an implement one intervention in a small group from the aggregate, and considered the effectiveness of the intervention on the health of the small group. It’s time now for you to present your final submission of this Capstone project.

Your final submission comprises the documentation of the work accomplished through your Capstone project, a Microsoft Word Document that contains the evaluation and summary, a Microsoft PowerPoint presentation highlighting the main aspects of the project including the key things learned during the 10 weeks of working with this project.

The summary document should use the Mobilize, Assess, Plan, Implement, Track (MAP-IT) steps used in pulling together your project. The complete documentation of the work accomplished over the course of the project should contain a minimum of 6–7 pages in a Microsoft Word document and should include the following information:

• A detailed description of the aggregate
• A description of the aggregate’s strengths and weaknesses
• A risk assessment of the aggregate
• Diagnoses based on the risk assessment
• A detailed care plan for the aggregate
• A description of how at least one intervention was implemented in the aggregate to address an identified issue
• An evaluation of how you would identify the effectiveness of the intervention

The Power Point presentation should be concise and should include the highlights of the Capstone project and the key things learned over the course of this project, from developing, implementing, and evaluating the care plan. Your presentation should not exceed 15 slides.
