CLINICAL CASE DECISION INSTRUCTIONS

Grading Criteria for Clinical Case Decisions:

The highest potential points for each Clinical Case Decision Question are 15-points. Students are required to upload their response as a word document attachment to the assignment drop box.

Each uploaded response is worth a possible 15 points.

1.       Your comments should be substantiated and substantive. Postings require two citations from 1 scholarly journal and/or 1 outside text book           Text books assigned to this course may be used for an additional citation only.   
2.       Each of the two references must be from a different reference source.
3.       To gain full credit for the assignment, the response must be a full page discussion, but no more than three (3) pages in length, which                   includes the reference list.
4.       Students are required to use APA format, proper citations, and references.  Students using direct quotes from referenced sources in the body       of the paper must include quotations.
5.       Students will be assigned to a question.

Clinical Cases

You have been assigned two (2) cases to provide analysis.  All cases should include the following:

a. Pathophysiology and pharmacology of the disease

b. Expected signs and symptoms of the disease.

c. Nursing Diagnosis with a plan of care.

d. All questions pertaining to the case are listed under each case. 

Clinical Decision Cases

C1.   Mr. Appel has a severe chronic obstructive pulmonary disease (COPD). He is admitted to the hospital with a respiratory infection and increased dyspnea; yellow, purulent sputum; anxiety; and diaphoresis. He states he feels weak and tired. He routinely takes a diuretic (furosemide) and his pulmonary medications. The following laboratory values are obtained:

Arterial blood gases:

pH 7.25                                         Serum sodium (Na+) 140 mEq/L

PaO2 60 mmHg                             Serum potassium (K+) 2.0 mEq/L

PaCO2 78 mmHg                           Serum chloride (Cl–) 105 mEq/L 

HCO3– 34 mEq/l

a. What type of electrolyte imbalance does Mr. Appel have?

b. Interpret his ABGs.

C2.   Mr. Jones is admitted to the hospital with an exacerbation of heart failure. He has +3 peripheral edema of the lower extremities.

a. What are the possible causes of his peripheral edema?

Write a formal paper (1,000-1,200 words) in which you distinguish the role of allied health in the delivery of patient care, define the scope of practice for allied health professionals, and discuss the importance of patient-centered care.

This assignment requires you to apply the academic search skills learned in the preceding topic.

Read the profiles of each of the allied health providers found in the media.

Select two allied health providers, from among those profiled, on which to base your assignment.

Using the GCU online library, perform an academic search in which you research the role and scope of practice for each of the two allied health providers you selected. A minimum of three scholarly peer reviewed articles are required.

In a formal paper that is formatted using the APA style guide, write an academic paper in which you address these six main concepts:

1. Describe the role of each allied health provider in the delivery of care.
2. Describe the education and skills needed to provide patient care.
3. Define the concept of Scope of Practice as it pertains to the work of allied health.
4. Define the concept of patient-centered care.
5. Discuss the importance of patient-centered care.
6. Describe behaviors/steps taken by allied health providers that result in patient-centered care.

 

C. T. a 48-year-old male patient presents for a checkup.  He admits it has been 4 years since he has been seen by a medical professional and at that time was diagnosed with an upper respiratory illness. He did have his blood pressure takin a year ago at his employer’s health fair and was told it was a “little high”.

He has had no major illness or chronic diseases. He has no known allergies and does not take any regular medication.

His family history shows that is father died of a heart attack at age 68, his mother is alive and well at age 72, he has a 50-year-old brother with depression and alcoholism, and one son who is healthy at age 24. He did have a maternal uncle with prostate cancer.

On review of systems, he has occasional headaches, experiences shortness of breath when he walks upstairs, and gets up once a night to urinate.

• What should the major objectives of this health maintenance visit be?
• What conditions, risk factors, and health related behaviors should be screened for during this visit?
• What are the common causes of morbidity and mortality for 48-year-old men in the United States?

For each question below, write a minimum of 75 words EACH. Be sure to cite sources for EACH of the following questions and separate the four paragraphs.

· A vulnerable population, as listed in your readings, is minorities. Identify the reasons why this particular group has a difficult time accessing health services in the United States. What is currently being done to address these access concerns? What else needs to be done?

· Explain why it is so important to safeguard a patient’s health information. What are some of the consequences that could happen to both the patient and the organization if health information is breached?

· Discuss the various components of long-term care services in the United States. Specifically, what are the differences between retirement centers, assisted living facilities, and skilled nursing facilities. What other options might be available to support the elder or the person needing extensive assistance in the community?

· Discuss the impact globalization has on healthcare in the United States. What measures will need to be taken to ensure the health of Americans now and in the near future?

It’s important to point out that the  nursing process is flexible and not rigid. It is a tool to use in  nursing care, but one that should allow for creativity and thinking  outside of the box.
