Critical Appraisal   

           As advanced practice nurses, it is vital to critically appraise research studies.  Barr-Walker (2017) discusses that critical appraisal is the practice of systematically analyzing research to evaluate its trustworthiness, value, and pertinence in a specific situation (p. 71).  When critically appraising quantitative studies, it is imperative to assess three factors.  

Quantitative Studies 

Validity, reliability, and applicability are three essential components of quantitative studies that must be assessed.  Melnyk and Fineout-Overholt (2015) discuss that the validity of a study addresses whether the results of the specific study were acquired from valid scientific resources (p. 93).  If confounding variables are identified in the study, one can conclude that the findings are in jeopardy.  Melnyk and Fineout-Overholt (2015) explain that quantitative studies utilize statistics to release their conclusions (p. 96).  As a provider, one must evaluate the statistics in order to gain an understanding of the reliability of the reviewed study.  Melnyk and Fineout-Overholt (2015) point out that the primary considerations are the breadth of the intervention’s effect and how accurately that effect was predicted.  Combined, these decide the reliability of the study findings (p. 97).  In order to properly assess for accountability, providers must exhaustingly evaluate if the quantitative study is directly applicable to the specific patient population.  Melynyk and Fineout-Overholt (2015) discuss that each study addresses explicit questions that, when addressed, help providers in critically appraising the study in order to determine its worth in the practice setting (p. 107).  

Although each of these three factors are vital components to assess, applicability is the most important.  If the quantitative study results cannot be directly applied to the specific patient population, the results can simply not be utilized.  Melynk and Fineout-Overholt (2015) emphasize that analysis of quantitative study results requires the provider to consider the clinical significance of the findings (p. 93).  Consequently, if the quantitative study findings are not directly applicable to one’s specific practice setting, the study results lack pertinence.  

Conclusion 

LoBiondo-Wood and Haber (2014) explain that it is essential to initially examine the level of evidence that is supported by the design and how the promising strength and quality of the findings can be utilized to improve or alter practice (p. 177).   As advanced practice nurses, it is key to consistently critically appraise studies in order to boost patient outcomes.  Furthermore, consistent appraisal of quantitative studies will assist in expanding one’s clinical knowledge base.

 In a perfect world, research studies would be flawless however, that is not typically the case.  Inherently, flaws exist related to study design, how the study is conducted, and also the manner in which research is reported.  “Given that research is not perfect, users of research must learn to carefully evaluate research reports to determine their worth to practice through critical appraisal” (Melnyk & Fineout-Overholt, 2015, p. 92-93).  Factors that need to be assessed when critically appraising quantitative studies include validity, reliability and applicability (Melnyk & Fineout-Overholt, 2015).  

       The validity of a study relates to obtaining results from utilizing sound methods which are scientific.  Bias and confounding variables can compromise validity of a study.  Bias can occur at any stage of the process but while assessing the reliability of the results one must look at if the study was systematic, grounded in theory, and criteria was followed for all of the processes.  If so, then the study, more than likely, is valid and reliable.  Clinicians need the ability to interpret the results so they can implement EBP into their practice (LoBiondo-Wood & Haber, 2014).  “Whether we are interpreting the research studies of others, or designing our own, we need a good understanding of research design and an ability to recognize weaknesses in intervention studies which may reduce the reliability of study findings” (Ebbels, 2017, p. 229).      Although each factor is important to quality research, I feel the most important factor is the applicability to practice.  Having the ability to appraise research, which is evidence-based, and implement EBP interventions with our patients is essential in promoting optimal patient outcomes.

The development of the electronic medical record (EMR) has been a pivotal advancement within nursing and to the delivery of healthcare. I have been a nurse for 19 years. While the electronic medical record was difficult to embrace in the beginning, it has proven to be a reliable and necessary tool. EMR has evolved over time impacting delivery of care and improving outcomes including time sensitive acute care patient scenarios resulting in positive outcomes (Sweeney BSN, 2017).   The availability of viewing labs and health records simultaneously allows nurses and critical care team members, i.e. pharmacist and intensivist, the ability to make safe time sensitive decisions for patients.  

     As a critical care nurse, things can go bad quickly at the bedside. For example, a patient arrives to the ED in an acute CHF exacerbation.  Lab work, radiologic studies and a baseline of vital signs are crucial to fast and effective care. If this patient is established in the system, baseline lab values, previous radiologic test for comparison and possibly even notes from a specialist i.e. cardiology/nephrology are available while waiting on new test to arrive and stabilizing the patient.  This information is available to the entire team instantaneously through his/her EMR. EMR has allowed physicians, nurses and other medical team members to make safe and effective decisions producing favorable and effective outcomes. For example, knowledge of the patient’s renal function or availability to notes from his/her renal physician will direct the ED physician to the appropriate treatment. This could potentially avoid possible long-term effects to this patients’ kidneys and provides insight to treatment options that have worked well for this patient in the past.  The Agency for Healthcare Research and Quality (AHRQ) supports this with study results noting fewer medication errors, improved quality of care with and cost savings as a result of fewer medical errors from utilizing the EMR (“Electronic Health Reporter,” 2018).

    Nurses have contributed to the evolution of the EMR and applications centered toward favorable outcomes. The role of the informatic nurse has changed with the evolution of healthcare redefining roles and forms, it is not consistent and will change as healthcare evolves (Nagle, Sermeus, Junger, & Bloomberg, 2017). Nurses will continue to play a profound role in the development of EMR and other tools available to provide safe and effective patient care.

Nurses rely on gathered information, statistics, and trends to help them in their day to day operations. Nurse managers rely on what is trending when it comes to Performance Improvement. The trending information helps the nurse manager pinpoint where the trouble lies and able to go to the source of the problem and educate the staff and fix the problem. For example, during chart audits, it has come to the attention of the auditor that the smoking cessation education check box has been missed in about 50% of the charts that were checked. This data that was collected was entered into a computer program to break down and pinpoint what area of the hospital it was coming from or from whom. This information was given to the nurse manager to share with the staff from which this trend had occurred. Education was able to be given to correct this problem. Information and information processing are central to the healthcare worker. A healthcare professional is also known as a knowledge worker because he or she deals with and processes information on a daily basis to make it meaningful and inform his or her practice (McGonigle & Mastrian, 2018, p. 21). Nurses have to have a knowledge base in order to do the basics of nursing. Most of the knowledge that a nurse will further acquire is on the job. Such as the scenario given above, nurses have to be trained in the policy and procedures of their facility. Gathering information to be able to educate nurses on what is missing in there charting or other paperwork that is required of them is helpful to create proficient nurses.

Healthcare informatics is defined as “the integration of healthcare sciences, computer sciences, information science, and cognitive science to assist management of healthcare information (Sweeney, 2017). By gathering the data, we were able to break it down and create a bar graph of each unit and how many unchecked boxes for the smoking education that did not get done correctly. We could see if any one nurse was trending and gave direction on how to proceed with the educating of the staff. This information came from our PI department to the nurse managers then down to the staff for correcting. According to N. L. Nagle et al., knowledge innovation and generation provides guidance and support to others (nurses, patients) in the application and use of emerging knowledge… (Nagle, Junger, & Bloomberg, 2017, p. 219).

                                                          INITIAL POST

            Evidence-based practice is the standard that guides clinical practices within the nursing profession. Adams (2010) asserts that evidence-based practice “is defined as the integration of best research evidence with clinical expertise and patient values to facilitate clinical decision making” (Adams, 2010, p. 274). Polit and Beck (2017) maintain that there is no consensus about what does or does not constitute evidence. There are, however, agreed upon sources of evidence which exist within a hierarchy. Systematic reviews are at the pinnacle because information is derived from multiple sources.  Randomized controlled trials are next, followed by cohort studies, single case-control studies, cross-sectional studies, qualitative studies, and finally expert opinion reports. Knowledge translation is the process of using evidence to evoke systemic change within the clinical practice (Polit & Beck, 2017).

Managing Delirium

While working on a busy surgical floor, I was caring for a confused, combative, elderly patient with a urinary tract infection who had fallen and broken her hip. During report, the night shift nurse told me that she gave the patient multiple doses of haloperidol and lorazepam which were ineffective. The patient continued to be confused and agitated, and because she was a danger to herself by continually trying to get out of bed, the night shift nurse obtained an order for soft restraints. Springer (2015) contends that the nurse should determine if the utilization of restraints is appropriate based on the patient’s current behavior, and should only be used when all other options such as distraction and de-escalation are exhausted (Springer, 2015). Because I was not there, I must assume that the nurse used evidence-based practice to decide that the restraints were necessary.

When I went in to assess the patient, she was sleeping; and in my professional opinion, the restraints were no longer appropriate. I removed the soft restraints and put the patient on one to one observation with a nursing assistant. Not long into the shift, the light for that room came on, and I heard staff in the patient’s room yelling. I walked in to find the patient screaming and striking the nursing assistant as he was attempting to change the patient. It was clear that the patient was still experiencing acute delirium. However, the television was on, the blinds were open, and every light in the room was on. Instead of using a chemical or physical restraint, I turned off the television, lights, and closed the blinds. I sat down beside the patient, spoke softly and attempted to reorient her. Although she was still confused, she was calm.

Bull (2015) asserts that nursing interventions to manage delirium include providing a therapeutic environment, frequent re-orientation, anticipating the patient’s needs, ensuring sensory assistance devices such as glasses or hearing aids are in use, observing the patient’s response, and proceeding accordingly. Non-invasive interventions should be exhausted prior to restraining a patient chemically or physically (Bull, 2015). In this case, the patient responded to non-invasive interventions. I continued to use the one to one observation to ensure safety throughout the shift but did not need to escalate to using chemical or physical restraints. By implementing evidence-based practice, I kept the patient safe without using restraints.

Background and PICOT Question

    Background questions are broad, generalized questions that focus on a clinical issue (Polit & Beck, 2017). In this case, my background questions would be: what is delirium? And, what causes delirium? The acronym PICOT (population, intervention, comparison, outcome, and time) is a format used to create a research question with the subsequent goal of finding evidence-based solutions to implement into clinical practice (Polit & Beck, 2017). My PICOT question is: in delirious patients (population), what are the effects of non-invasive management techniques (intervention), compared to restraints (comparative intervention), on patient experience (outcome) and does either intervention increase or decrease the recovery period (time)?

Organizational Critique

I work as a float nurse in my organization, with previous experience in critical care. As a float nurse, I have a unique perspective on organizational culture because I work in multiple units. Overall, my organization does facilitate a culture of safety that promotes an environment where nurses learn from mistakes and do not place blame on one another. Written policies and procedures are easily accessible on the intranet. Moreover, my organization utilizes nursing shared governance which has a special committee devoted to practices and standards. Nurses are encouraged to bring practice issues to members of shared governance, and clinical practices are continually being updated and reviewed. If a nurse has an immediate question about a clinical practice situation, Clinical Nurse Specialists are available as a resource in addition to written policies and procedures.

Organizational Barriers

            Majid et al. (2011) report that most nurses have positive attitudes about evidence-based practice. However, some barriers which reduce the utilization of evidence-based practice include inadequate time to learn and implement evidence-based practice; nurses lack understanding of statistical terminology and research jargon, and technological deficiencies which inhibit informational searches (Majid et al., 2017). I believe that inadequate time is the primary barrier to evidence-based practice implementation within my organization. Time is finite, and working 12-hour shifts means nurses do not want to stay in late or come in early for any type of training. I propose that team nursing would provide individual nurses with the opportunity to attend training during regular working hours. Dickerson and Latina (2017) maintain that team nursing is the practice of nurses working in pairs to deliver patient care. A pair of nurses make up a team; both nurses get report on all patients shared by the team, Then, when one nurse needs to step away for a break, or in this case for training, their partner is already ready to take care of their patients.

                                         Introduction/PICOT Question

Pain is the most common complaint of people who present to the emergency department (ED).  It is estimated that 75% of all patients use the ED for pain related complaints (Bergman, 2012).  Patient satisfaction survey results emphasize the importance of adequate pain management (Bergman, 2012). With this in mind, nurses have a responsibility to evaluate, assess, and be advocates for a patient’s pain control.  Studies have shown when an ED department has pain management protocols and delegates these to the nurses there is a reduction in patient’s pain (Hadorn, Comte, Foucault, Morin, & Hugli, 2016).  Discovering patient expectations for pain management allows healthcare workers and patients to understand each other.  Once understanding has taken place, we can evaluate essential questions related to the patient’s pain management.  As healthcare workers are we expected to base pain management protocols solely on self-reported pain by the patient?  If so then an important question to answer is: When patients present with pain in the ED, how does a nurses’ perception of the pain influence the level of pain management the patient receives? 

Evidence-Based Practice

              Currently, in the ED I work in pain guidelines and protocols are in place for the nurse to initiate as needed.  Hadorn et al. (2016) state, “guidelines or protocols are generally used to facilitate the transfer of compelling evidence into clinical practice” (p. 81).  This evidence-based pain management protocol is available for the nurse to initiate based on the patient’s self-reported pain, nursing assessment, and observational data.  At times, nurses evaluate pain based on vital signs, facial expressions such as grimacing, restlessness, self-report, and chief complaint (Hazelett, Powell, & Androulakakis, 2002).  In our organization, the policies and procedures for pain management support the adoption of evidence-based practices.  By using the set protocol for pain, nurses are better able to treat pain promptly.  Nurses have a crucial role in delivering evidence-based healthcare in order to have the best outcomes and treatments for their patients (Majid et al., 2011).

Barriers

       Studies noted a barrier to effective pain management is the nurses’ perception of the individual.  Some patients frequent the ED over exaggerating or making up complaints to seek pain medications (Hazelett et al., 2002).  This repetitive drug seeking over time creates a barrier to compassionate care.  The ED is a vulnerable place for drug abuse, especially by patients attempting to get narcotics or other pain medications (Bergman, 2012). 

Emergency department nurses and staff have a culture within their units that influence everyday practices.  There is often a vast age range in the nursing staff which highlights differences in work ethic, attitudes, and professionalism.  The culture of our ED hinders the initiation of evidence-based practices due to stressed out, overworked, and underappreciated nurses.  With this in mind, it is vital that as an organization we provide opportunities for nurses to have adequate rest and find ways to show appreciation for all their hard work.

             Proper access and initiation of the pain management protocol increases the likelihood of adequate pain control for patients.  By discovering the expectations of patients and barriers to providing appropriate care we can eliminate miscommunication regarding pain control.

Communicating with elected officials is an important part of public health advocacy.  Written communication to the different branches of government is often used by health and advocacy groups to ensure their perspective is considered in the debate on an issue. 

Advocacy groups often engage their member advocates in campaigns to directly communicate to their representatives to assure the community voice is included in the discussions and to influence legislators and regulators.  Constituents’ (you, the residents and voters) input on issues is a valued part of the democratic process and should not be underestimated.  Elected officials often make decisions about voting, and even introducing bills in response to public communication. You are writing a draft letter for your primary post.

For this assignment, your appeal should include a policy option. Advocacy for a general solution about the problem includes suggesting a policy option or principle you think will address the problem.

Professional nursing organizations are designed for the patient’s safety and quality care as well as ensuring nurses are competent and safe to perform quality and safe patient care (Helbig, 2018).  For example, (ANA) American Nurse Association is a professional organization with goals of improving health care and setting standards for nursing practice improving the nursing  profession and supporting nurses (ANA, n.d.c).  Another is the (NCSBN) National Council of State Boards of Nursing which was created to protect the public from incompetent nurses and require knowledge, skill, and independent decision making in order to be licensed as an RN.  There are also others like the (IOM) Institute of Medicine which protects and informs the public and focuses on suggestions to improve health care (Helbig, 2018), and the (TJC) The Joint Commission which is an organization that identifies issues of patient safety within health care organizations and then designs a plan to correct the issues (The Joint Commission [TJC], 2017).  All of these organizations advocate for patients and their safety while creating standards of practice for the nursing profession and push for change within the health care system while giving suggestions of how to give safe quality care to patients.

 I am thankful for these organizations and what they are doing for us as nurses and for the patients we care for.  This allows best safe practice and gives guidelines to follow to ensure competent nurses give safe quality care.
