When I reviewed the assignment for this week, sheer panic took over. Then after reviewing all the tutorials on www.screencast-o-matic.com I felt a little relieved. The next tutorial reviewed from Professor Lease’s, and this one helped to pull it all together. This is one of those things that I had to get to website and navigate through a lot. I can easily record, stop, and erase after recording myself narrating. I use Microsoft online. I had a lot of trouble using PowerPoint Online, and trying to record by voice while using the slideshow view. This is impossible using the online version of PowerPoint. I had to click on “open in PowerPoint,” on the upper right hand side of my PowerPoint viewing area of the presentation. The computer asks if you want to switch apps. One must click “yes,” and it will bring you to the version used on the desktop. Before finding this out, I had a lot of trouble recording. The Screencast-O-Matic control board would completely disappear. This became very frustrating, and almost had me in tears. Finding out that PowerPoint online had to be changed to the regular PowerPoint used for the desktop was my saving grace. I was even able to record using the webcam. After saving the video, I could not find it but knew it was somewhere. On the top of the screen I clicked “for education.” This will lead to a different screen layout. There is an option for “my videos,” that is located next to your account e-mail address. After clicking on “my videos” this will bring one to all the videos saved. Downloading and installing was easy. I have a new laptop so on the upper right hand side of the screen are 3 dots. I clicked on that, then clicked on “pin this task to the toolbar.” It puts a little icon on the bottom of the computer. This icon acts as a direct link to the website. I use this for Aspen University’s Classroom, Aspen’s student portal, Microsoft online, and any other important places I need to get to quickly with a click of a button. I do have a question for you all. What does Professor Lease mean when she wrote “this can not be a voice over PPT?” My understanding is screencast is a PPT with voice recorded as a narrator, but what is voice over PPT? Does she mean we need to record our face as well?

When I found out what the discussion question was going be for this week I was shaking in my boots. I thought to myself, here we go with something else I’ve never heard of and one more thing in life I must try and figure out. But to my surprise, with a little patience and not thinking it was the end of the world I was able to look up Screencast-o-matic with very few complications. Finding the website was easy. Looking at it for the first time I thought surly its not going to be as easy as logging in, but it was. The steps to recording wasn’t as bad a thought also. I tried to do it on my own but found later that it would have been best if I took the tutorial. After watching the tutorial, I was able to record the words on my first two slides. It was nice to learn to choose the section that I wanted to be heard within the PowerPoint. I must admit, I thought I sound quite crazy. The first thing I said was “wow is this what I sound like”. Then I quickly hit stop. After regrouping, I was couldn’t figure out how to get the screencast onto the PowerPoint. After playing around with it for a while and calling for help, I was able to set things up the way it needed to be presented. I don’t think the task of getting the screencast together was hard. I just think it was one more thing new that I had to become aware of, and we all know newness can be scary. I’m still having some trouble with the upload. Sometimes it worked for me and other times it didn’t. I’m sure if I go into it again when I have more time it will be fine. In the end it turned out to be not as bad as I thought it was. I admit that I didn’t know anything about screencast. Preparing those two slides were great. It was nice to learn something different and how other things beside graphs and pictures can be added to a PowerPoint. I look forward to seeing how my final presentation will turn out.

One of the most prevalent age related changes I have noticed over my career is changes to the skin. Skin loses elasticity over time and decreased mobility can cause pressure ulcers to the patient. This is compounded as aging also reduces the ability for wounds to heal. Pressure ulcers affects the patient’s quality of life and can increase morbidity and mortality (Mervis, Phillips, 2019). It is important for elderly people to inspect their skin routinely and notice any breaks in the integrity of the skin. Teaching for these individuals would include making sure they reposition at least every hour while sitting or lying to offset pressure points. It is also important to have the elderly inspect pressure points over bony prominences. 

The second age related change that many elderly patients face is bowel function. Aging can cause decreased motility of the GI system and dietary changes affect the amount of fluids the individual have in their body. Many elderly people are very aware of the frequency and consistency of their bowel movements and become anxious when they do not have a bowel movement at least daily. It is important with these individuals to encourage a high fiber diet with adequate fluid consumption. The addition of dietary supplements such as fiber supplements or stool softeners can also help these individuals with regularity in their bowel functions.

In the Functional Consequences Theory, age-related changes are the inherent physiologic processes that increase the vulnerability of older people to the detrimental effects of risk factors. From a body–mind–spirit perspective, however, age-related changes are not limited to physiologic aspects but include potential for increased cognitive, emotional, and spiritual development. Thus, nurses holistically focus on the whole person by identifying age-related changes that can be strengthened to improve the older adult’s ability to adapt to physiologic decline. For example, nurses can work with older adults to strengthen their coping skills. In addition, nurses have many opportunities to build on the wisdom of older adults, especially their “everyday problem-solving” skills, by teaching about interventions to address risk factors” (Miller).

2.) Address the risk factors in relation to prevention and treatment of their medical conditions. 

“A major focus of wellness-focused nursing is to identify risk factors that can be addressed through health promotion interventions. For example, from a health promotion perspective, nurses routinely assess for risks associated with stress, smoking, obesity, poor nutrition, and inadequate physical activity. An aspect that is unique to caring for older adults is the need to identify myths or ageist attitudes that can affect health care. For example, if urinary incontinence is mistakenly attributed to “normal” aging, then the older adult will not receive appropriate evaluation and interventions. Environmental risks are also particularly pertinent to older adults because additional risk factors, such as sensory, mobility, or cognitive impairments, can compromise their safety and functioning. Identification of risk factors is an integral aspect of the Functional Consequences Theory because nurses have numerous opportunities for promoting wellness by identifying and addressing the many modifiable factors that affect functioning and quality of life for older adults” (Miller).

An example of a grounded theory analysis is” Beck’s (2002) model of   mothering twins” as cited in Polit & Beck (2017).

 

Focus Group Analysis 

Analyzes group data in relation to a specific topic. Group interviews, recordings, and field   notes .are instruments for conducting this type of research.

May be used for evaluation of a potential survey tool, consensus on a   new product. Researchers seek to   extrapolate recurring themes.

An example of a focus group analysis might be to evaluate perceptions   of a new product being marketed to test for general consensus of its   desirability. 

Quasi-statistics: a tabulation of the frequency with which certain themes or insights are supported by the data

Qualitative content analysis: analysis of the content of narrative data to identify prominent themes and patterns among the themes

Domain analysis: 1st of 4 levels of data analysis, domains are units of cultural knowledge, are broad categories that encompass smaller ones. Ethnographers identify rational patterns among terms in the domains are used by members of the culture. Ethnographer focuses on the cultural meaning of terms and symbols used in a culture

Taxonomic analysis: second level of data analysis, ethnographers decides how many domains the analysis will encompass. Taxonomy is then developed to illustrate the internal organization of a domain and the relationship among the subcategories of the domain

Taxonomy: a system of classifying and organizing terms

Componential analysis: relationships among terms in the domains are examined; ethnographer analyzes data for similarities and differences among cultural terms in a domain.

Theme analysis: cultural themes are uncovered; domains are connected in cultural themes, which help to provide a holistic view of the culture being studied. The discovery of cultural meaning is the outcome.

Focused coding: the analysis is directed toward using the most significant codes from the initial coding

Congruent methodological approach: analyzes interaction data in the same manner as a group or individual data

Managing Qualitative Data

Computer-assisted qualitative data analysis software (CAQDAS): a program that can take uploaded data files, code the narratives, retrieve information, and display text for analysis

Text Box: • Text retrievers-locate text and terms in a database. • Code-and-retrieve packages allow researchers to code text. • Theory building software functions to examine relationships between concepts, develop hierarchies of codes, diagram, and create hyperlinks to create nonhierarchical networks. • Concept mapping constructs sophisticated diagrams. • Data conversion/collection software converts audio into text.

Within a qualitative data analysis there is not statistical tests, because qualitative research is based on thoughts, open ended questions, interpretations and interviews not numerical values. Data within qualitative research is understood and analyzed during the entirety of the process. “Researchers interpret the data as they read and reread them, categorize and code them, inductively develop a thematic analysis, and integrate the themes into a unified whole,” (Polit & Beck, 2017, p.549). There is not a step by step understanding of how the process occurs of interpreting the data, researchers “live” within the data by understanding the meanings, looking for patterns, draw valid, discerning conclusions. An additional importance of understanding of the facts is having the inventiveness to find the “aha” meaning of the information and discovery of the meanings of the facts gained (Polit & Beck, 2017). 

The importance of the interpretation is just as important as the validity of the data. Thorough and sensible researchers have a high standard of their data interpretation by dissecting themselves, peers and outside reviewers. It is vital that the qualitative researchers consider possible different explanations or meanings other than their own (Polit & Beck, 2017).

It is important nurses to understand statistical data because this is a large part of the work nurses base the practice on is evidence based, which means understanding the research behind the reason of the practice is important to understand. According to Hayat, it is important to understand the difference between statistical significance and clinical importance, researchers tend to use statistics to claim proof and scientific breakthrough. Significance testing can be used to decide which data may be considered evidence to support a practice change (2010). “Judgment and subjectivity are necessary and part of the decision-making process. Statistical significance is not a measure of importance; it is a subjective and qualitative construct. Researchers conducting quantitative analyses should quantify the magnitude of an effect. The value of the data collected should be assessed by examining study design, bias, and confounding variables, as well as meaningfulness of the results to the topic under study,” (Hayat, 2010, p.222). Nurses must consider this and have an understanding when utilizing statistical methods to base their practice changes.
