As many major medical companies and pharmacies face lawsuits and even bankruptcy in light of recent lawsuits, it is likely that nursing practice and evidence-based practices will change in an attempt to save lives and reduce opioid-related deaths. Working in a surgical center provides a unique nursing opportunity to address and change the opioid crisis, as many patients that begin abusing opioids begin their prescriptions either for chronic pain, or after injury or surgery (Demsey, Carr, Clarke, & Vipler, 2017).  Two ways that we can expect nursing practice to be impacted by the change in addressing our nation’s opioid crisis include patient teaching, and tracking medication use and pain levels (American Journal of Nursing [AJN], 2015). Patient education regarding opioid medications include educating the patient on the risks of opioid diversion, and the proper storage and disposal of opioid medication prescriptions (AJN, 2015). Studies show that approximately one-quarter of medications are diverted from legal prescriptions, while 71% of those diverted opioids were obtained either from friends or family, either for free or by theft (AJN, 2015). Educating patients about these facts as well as the importance of proper safe-keeping of medications and how or where to safely dispose of opioid medications is an important first step in correcting the opioid crisis.  Monitoring patient and tracking usage of opioids include patient teachings that focus on the correct usage, as well as appropriate indication and expectations (AJN, 2015). For example, patients after surgery should be educated that they will not be pain-free immediately following their procedure and that opioid prescription medications should not be used for any other indication other than pain, which includes induction of relaxation or sleep (AJN, 2015). It is also important to teach the patient about keeping a pain medication journal which includes the type of pain the patient is experiencing, the pain scale rating associated with the pain, as well as time and dosage of the medications taken, reviewing these journals with the patient can help the healthcare providers notice usage or patterns that could indicate a current or potential problem (AJN, 2015). Helping to identify issues before they become an actual problem are other interventions that the nurse provides to the patient that will aid in addressing and correcting the opioid crisis in the United States.  References  American Journal of Nursing. Following an industrial accident, two patients arrived at the emergency room of that hospital at the same time, presenting with very similar inhalation injuries. The hospital received a great deal of negative press due to how the patients were triaged in the ER. Your supervisor would like you to use the specifics of this case to review triage procedures and best practices at your facility. Here are the details:

  • One is a 32-year-old firefighter, Frank Jeffers, who is presenting with respiratory difficulties that he obtained while evacuating victims of an industrial accident. He is a married homeowner and father of two young boys. He has lived in the community all his life. He has full and comprehensive health insurance through his employer.
  • The other is Brent Damascus, a 58-year-old man. Brent is presenting with respiratory difficulties with the same intensity as Mr. Jeffers above. He is well known at the hospital emergency room, as he is a frequent visitor with various complaints, including asthma, headaches, and tremors. He is homeless, unemployed, and uninsured. He stays many nights at the YMCA and eats lunch at the soup kitchen. He has lived in the community for over 10 years and has been arrested several times for petty theft.

Preparation

Search the Internet for scholarly and professional peer-reviewed articles on best practices in triage nursing. You will need at least three articles to use as support for your work on this assessment.

Directions

Create a 15-minute presentation 4 pages that examines the moral and ethical issues that occurred when triaging these two patients and the best practices for managing this in the future.

Divide your draft into a number of talking points that you can summarize neatly. Keep in mind that an oral presentation requires slightly different language than an essay. The aim is to communicate your message so keep sentences simple and focus on the key points you want to deliver. Address the following in your presentation:

  • Recommend evidence-based strategies that should be applied for managing the care of uninsured and indigent population.

Patients today are much sicker than they used to be. The advanced practice nurse needs to manage all comorbidities in the plan of care. In this post, I will discuss the patient’s health care needs and a recommended treatment plan, including pharmacotherapeutic choices. Also, I will present an education strategy to assist the patient in managing their disease conditions. The patient has a history of hypertension, myocardial infarction, hyperlipidemia angina, and diabetes type two. Her needs center around the management of these comorbidities. Also, I noticed that her serum creatinine is on the high end of the range if not outside of it. This fact is not surprising considering her diabetes. The case reports that she was doing well until about a month ago. It was tempting to consider adding another medication to the regimen. However, I believe that either the metoprolol is not high enough to manage her current disease state or she has been missing doses, and I am leaning toward the latter. If withdrawn suddenly, the drug can increase the incidence and intensity of anginal attacks. Metoprolol is the first-line drug for stable angina and should be working (Rosenthal & Burchum, 2018).  

A review of her current drug therapy appears appropriate. Aspirin, simvastatin, and metformin are competent choices, and the doses are adequate. My plan would include an assessment of how she takes medication at home and manages them before making any changes. If this were in order, I would increase the metoprolol to 75 mg twice a day and set a follow-up appointment in two weeks. I prefer to increase the dose of metoprolol first before adding any new drug because of polypharmacy considerations, and the drug does not require renal adjustment (Epocrates, 2019). This fact is beneficial to consider because she has diabetes. Assuming that the patient has missed doses over the last month, the priority is to determine why. Costa et al. (2015) recommend explaining how to take medication, discussing reluctance to take drugs, and a conversation about the patient’s beliefs and knowledge about their health and treatment. I believe that this is a great strategy to use with the patient. Medication adherence is troubling and particularly so with the elderly who have multiple medications and conditions. I think it is essential for the patient and provider to be partners in the plan of care. The patient must agree to it and fully understand their conditions and the purpose of each medication.

    Toxicity is determined by the following:

Toxicologists often describe toxins as either hazardous or poisonous. Hazardous substances are substances having the capacity to do harm. Poisonous substances are able to kill, injure, or impair living things with a small dose.
      Assessing Risk
      After determining that a substance is toxic, toxicologists and other scientists create risk assessment models. Risk assessment involves considering four steps:
    • Identification of the hazard and its potential health effects.
    • Dose-response (amount of pollutant to which a person is exposed).
    • Mode of exposure (inhalation, ingestion, absorption, and injection).
    • Determination of overall risk based on dose response and exposure.
    • Cost-benefit analysis can be used to determine if a risk should be taken, and what strategies can be used to regulate and control the risk. In some cases, after a risk assessment, a risk can be determined as unavoidable.
      The Consumer Product Safety Commission (CPSC) is a government agency created in 1972 to address some products that have presented an unreasonable risk of injury. The CPSC requires safety labels, recalls hazardous products, and enforces bans upon them.
      Accidents
      According to the U.S. Department of Health and Human Services (2014), more than 400 Americans die each day due to injuries caused by accidents. In fact, most people experience a significant injury at least once in their lifetime (Hilgenkamp, 2006). Although, not all accidents can be avoided; understanding where the risks are and taking steps to avoid them is important to protecting our health.
      Motor vehicle accidents are the leading cause of death in individuals between 1 and 44 years of age (CDC, 2006). Seat belts, safety seats for children, airbags, and anti-lock brakes are just some of the devices that can make traveling in a motor vehicle safer. We can reduce our risk of a motor vehicle accident by following traffic laws, and always considering what is prudent based on the driving conditions.
      Accidents can also occur when we are in our homes. Common occurrences involve falls, poisonings, accidental shootings, fires, and power equipment. Young children are particularly at risk, which is why child-proofing a home is very important. Cabinets with toxic materials, medicines, and guns should be locked. Matches and lighters should be kept in a safe place.
      OSHA
      Concerns of hazards in the workplace led to the development of the Occupational Safety and Health Administration (OSHA). OSHA’s mission is to prevent injuries and protect the health of United States workers by ensuring safe and healthful places to work (United States Department of Labor, n.d.). The major areas of concern in the workplace are air contaminants (dust, fibers, gases, and vapors), and physical (temperature, noise, and radiation), biological (pathogens), and chemical (inhaled, absorbed, ingested, or injected) issues.

      The purpose of this assessment is for you to learn how to summarize and critically evaluate a scientific paper on environmental toxins.
      Select at least two peer-reviewed articles about your chosen toxin to read carefully. Craft a 3–4-page analysis of the toxin’s impact on human health based on what you have learned. Address the following in your analysis:
    • Highlight the main points presented in the articles you read. What message are the authors trying to convey about that toxin?
    • Describe the possible sources of human exposure to this toxin and analyze their risk.
    • Analyze what happens when this toxin enters the environment.
    • Analyze the persistency of this toxin.
    • Analyze the federal government’s recommendations for protecting human health from this toxin.
    • Describe what other sources of information say about these toxins.
      • Identify any discrepancies you found in your research.
      • If the authors have different perspectives, how might these differing views affect environmental health?
    • Your analysis should follow a logical structure and be evidence based. Use the MEAL Plan to help guide the organization of your analysis:
    • Main Idea: Present the main point or idea that you are making about the environmental toxin you studied.
    • Evidence: What does the research say? Support your statements with evidence from your research and personal experience.

    • Written Communication: Written communication should be free of errors that detract from the overall message.
    • Length: This analysis should be 3–4 pages in content length. Include a separate title page and a separate references page.
    • Font and Font Size: Times New Roman, 12-point, double-spaced. Use Microsoft Word.
    • APA Formatting: Resources and in-text citations should be formatted according to the current APA style and formatting.
    • Number of Resources: You are required to cite a minimum of 2 scholarly resources. You may conduct independent research for resources and references to support your analysis.
