The mission of the schools in the local area system that was investigated is to promote the health and wellness for the students and their families, their staff, and the community.  The county schools believe that the nurses should be committed to providing comprehensive, quality health care to students using a systematic approach to identify and meet the dynamic health needs of all students including chronic health conditions, disabilities, and those with acute and emergency care situation (Maryland Department of Health, 2019).  The professional school nurse is an important member of the multidisciplinary school team and collaborates with other disciplines to enhance the academic success of students.  The school nurse promotes improvement of children’s health, removes barriers to learning, and improves overall potential for academic success.  Parents are encouraged to contact their child’s school nurse to discuss any health related needs.  Some of the schools in the county have Wellness Centers where the center provides care that minimizes the physical and emotional conditions that become barriers to optimal school performance.  The goal is to make students available for learning by promoting health, preventing disease, and reducing behavioral risks.

One existing condition that the schools deal with is sickle cell disease (SCD).  Children with SCD are more susceptible to infection.  Infection is the most common cause of death in children with SCD (Maryland Department of Health, 2019).  Most children with SCD are vaccinated against infectious organisms and generally use prophylactic measures to decrease risk of infections.  Fevers should be taken seriously for a child with SCD.  Other health complications include chronic pain, stroke, vision problems, progressive organ damage, acute chest syndrome, priapism, pulmonary hypertension, and anemia.  Although people of all races and ethnicity can have SCD, in the United States SCD is most common among persons of African descent.  The condition is also common among persons of Hispanic, Mediterranean, Caribbean, and Asian descent.  There are various types of SCD ranging from mild to severe.  Each individual with SCD may have a different clinical presentation; therefore students may have individualize care plans.  The school nurse is always the leader of the school health services team.  Some students with SCD may have a designated school case manager to coordinate his or her Individualized Educational Plan (IEP).  The school nurse also serves as the liaison and advocate for the student.  The school nurse may also refer the student and or family for counseling, support groups, and medical care.  Moreover, the county has school-wide awareness and education regarding SCD management for school staff.  Awareness and education may include, but is not limited to: definition and types of SCD; effective SCD management principles; symptoms of a SCD crises to report to the school nurse; student’s emergency care plan/protocol; student’s emergency plans and protocols for substitutes including teacher, school health staff, transportation, coaches, and food services; necessary and approved accommodations during school or school-sponsored activities; confidentiality protections; disability awareness needed in classroom; student’s IEP; and education for school visitors or volunteers with student contact, as necessary per local policy (Maryland Department of Health, 2019).  It is recommended that school staff awareness and education is completed annually and whenever a student’s condition and care changes.

I looked into a major school district in my area and found that the schools in my area have a nurse assigned to each school. The school nurse helps students with chronic illnesses like type one diabetes manage their illness. The school nurse also watches out for children with communicable diseases and decides to send them home if they have diarrhea, vomiting, fever, or a deep cough. Parents are also instructed to not send their kids to school if they have a communicable disease or lice. The school district policy is that children must be 24 hours being symptom-free before they can return to school. The school nurse cannot give medications that are prescribed three times a day at school unless the doctor says that medication during school hours. The school nurse will not give narcotics or any form of cannabis even with a doctor’s order. The school nurse will also provide hearing screenings in preschool, kindergarten, grades 1, 2, 6, and 9. The school nurse also will screen vision screenings during the same year as the hearing test. Another screening that the school nurse does is scream for scoliosis when kids are in sixth and seventh grades. The school nurse also makes sure that students are up to date on their vaccines in order to be entered into the school.

The school nurse helps promote Health by helping students who have chronic diseases manage their health. The school nurse also promotes health by sending kids home that show signs of a communicable disease and prevents communicable disease spread by making sure students are up-to-date on their vaccinations. Doing a quick screening for hearing, vision, and scoliosis will also ensure that all parents are aware if their child needs to see a provider for these medical conditions.

The Center for Disease Control [CDC] (2019) explains that school health services can promote health for students by providing acute and emergency care, care coordination, chronic disease management, and family engagement. The school nurse can help with acute or emergency care if a student is in a medical crisis like a seizure. The nurse can also respond if a violent crime were to happen on campus. The school nurse will coordinate health care with families, health care providers, administration, and teachers (CDC, 2019). The school nurse will work with students with chronic conditions by helping to manage their condition for example like helping children with glucose checks and providing insulin. The school nurse can also help engage families of students by giving parents health status updates and giving students extra dietary and physical considerations (CDC, 2019).

To aid in quiz development, nurse educators can use models such as The 3Cs (illustrated below) to focus more on the content of their questions and less on the actual construction. Nurse educators can also use prompts and templates provided at quiz development websites to assist quiz design. These websites can also help instructors easily administer quizzes in the online environment.

The 3Cs Model of Test Item Writing:

  • Content, concepts, and criterion. Use the content of the lesson as well as overarching concepts and nursing criterion (like NCLEX and other formal assessments) as a blueprint for your questions. In addition, consider the ideas and teachable moments that have occurred in the classroom or during clinical rotations.
  • Clarity and complexity. Be sure that your question stems are clear, straightforward, and use simple language. In addition, write your questions at the application or analysis level of Bloom’s Taxonomy. This will prompt students to move beyond basic comprehension and memorization.
  • Critique. Always go back and examine the questions that you have created. This process should be completed both before and after examinations. As with all teaching strategies, evaluation and revision is imperative to ensure the effectiveness of instruction.

  • Review this week’s Learning Resources on developing multiple-choice quiz questions. How might you use the 3Cs and other best practices to craft your own quiz questions?
  • Reflect on a Lesson Plan that you have created in this course. Which lesson might lend itself towards a pre- or post-quiz?

            The growth in online education has resulted in an increasing need for online quizzes.  However, creating and posting tests remains the responsibility of the instructor.  The purpose of this discussion is to describe how I created quiz questions, how I used a technology platform to post the quiz, and whether I would use this technology in the future. 

Challenge to Creating Quiz Questions

            I have instructed and performed the Rapid Strep Test for many years.  However, this is the first time I have been tasked with creating a lesson plan with follow-up quiz questions.  The Rapid Strep Test is a fixed skill with the process established by the manufacturer.  Additionally, the Rapid Strep Test requires memory and manual dexterity rather than an in-depth understanding of the scientific principles supporting the testing process.  Therefore, I felt I should design questions that focused on the how-to rather than the why.  I avoided negative options as those require reverse thinking rather than knowledge (DiSantis, Ayoob, & Williams, 2015).  I also made certain that each question had a single answer that was clearly correct so as to avoid ambiguity in the selection (Focuszone Media, Inc, 2019).  Finally, I made certain to keep all of the multiple-choice answers similar lengths to avoid distinguishing one from another (Focuszone Media, Inc, 2019). 

Challenge to Using the Technology Platform

            I chose SurveyMonkey as the technology platform for my quiz.  SurveyMonkey is an online platform that allows organizations to obtain feedback from employees, consumers, or students (SurveyMonkey, 2015).  I had never used a technology platform before, so the process of creating an online quiz was a new experience.  While at first, the process seemed daunting, SurveyMonkey has done an excellent job of streamlining their process.  The ease of using this application makes it likely that I will use SurveyMonkey in the future, whether through school or work.

Summary

            The increase in online education has made online quizzing a necessity. The method of creating quiz questions has changed little with the advent. However, the process of handing out paper quizzes and #2 pencils has forever been transformed into an online environment.

Parkinson’s disease (PD) is the second most common neurodegenerative disorder after Alzheimer’s disease (AD) (Moore, Shpiner, & Luca, 2019). PD has been traditionally considered as a pure movement disorder secondary to focal degeneration of dopaminergic neurons in the substantia nigra, but, in recent years, the clinical phenotype has been better illuminated, showing that PD is a multisystem neurodegenerative disorder with motor and non-motor features. Among motor symptoms and signs, the cardinal ones (bradykinesia, rest tremor, and rigidity) are mainly the loss of dopaminergic neurons, but those involving posture, balance, and gait are largely secondary to degeneration of nondopaminergic pathways and significantly contribute to impairment and disability in advanced PD patients (Moore, Shpiner, & Luca, 2019). Nonmotor features result from multiple neurotransmitter deficiencies in the central and peripheral nervous system and include psychiatric such as: depression, apathy, hallucinations, and delusions. Autonomic includes constipation, orthostatic hypotension, and urinary and genital disturbances. Cognitive impairment such as: involvement of executive functions, memory, sleep disorders, olfactory dysfunction, and pain that together contribute to worsening the quality of life and patient’s disability (Moore, Shpiner, & Luca, 2019).

  Cardinal motor features of Parkinson’s disease (PD) include bradykinesia, rest tremor, and rigidity, which appear in the early stages of the disease and largely depend on dopaminergic nigrostriatal denervation (Moore, Shpiner, & Luca, 2019). Intermediate and advanced PD stages are characterized by motor fluctuations and dyskinesia, which depend on complex mechanisms secondary to severe nigrostriatal loss and to the problems related to oral levodopa absorption, and motor and nonmotor symptoms and signs that are secondary to marked dopaminergic loss and multisystem neurodegeneration with damage to nondopaminergic pathways (Moore, Shpiner, & Luca, 2019). Nondopaminergic dysfunction results in motor problems, including posture, balance and gait disturbances, and fatigue, and nonmotor problems, encompassing depression, apathy, cognitive impairment, sleep disturbances, pain, and autonomic dysfunction. There are a number of symptomatic drugs for PD motor signs, but the pharmacological resources for nonmotor signs and symptoms are limited, and rehabilitation may contribute to their treatment (Moore, Shpiner, & Luca, 2019). 

  Pharmacological therapy is based on levodopa and dopamine agonists and is very successful in the early stages of the disease, when dopaminergic symptoms and signs are predominant and long term motor complications still have not developed, but other treatment strategies are necessary as time passes (Hajj, 2018). Long term levodopa-induced motor complications include motor fluctuations and dyskinesia and affect almost all PD patients at some point during the disease course, with relevant implications in global health status (Hajj, 2018). Despite various pharmacological approaches, as well as more invasive strategies including devices and functional neurosurgery, being available to manage such complications, many patients remain significantly disabled, and a fully satisfying management of motor complications is still an unmet need of PD therapy. Nonmotor symptoms and signs are integral to PD at onset and throughout the disease course, but to date their treatment is largely unsatisfactory (Hajj, 2018).

Heart Failure

Pathophysiology is a vital discipline in medicine and is generally useful for diagnosis and patient care. The disease-related to pathophysiology is heart failure. Wright and Thomas (2015) explain that pathophysiology in the management of heart failure ensures that patients are optimized on a maximum tolerated dose of medicine to treat heart failure with reduced ejection fraction. Heart failure is a complex clinical syndrome whereby the heart is not able to attain the metabolic demands of the body. The reasons underlying the mismatch can be multifactorial. However, the various pathological conditions make the treatments and management of the condition to be complex. Therefore, pathophysiology is essential in minimizing the burden on the condition. Heart failure is on the increase for the past decade leading to the heightening of the adult population to around 1% to 2%. The male population is the greatest affected gender, with an estimated 33% of men above 55 years old having the condition as compared to the 28% in women (Farmakis et al., 2015). Pathophysiology seeks to redress the balance through initiating contemporary measures. However, the measures have become damaging to the diseases and can lead to worsening cardiac output that can result in additional stress to a failing heart.

Servant leadership is a type of leadership in which the main goal of the leader is to serve and place the needs and interests of their followers and organization over their own self-interest and needs (Greenleaf Center for Servant Leadership, 2016). “Servant leadership is a philosophy and set of practices that enriches the lives of individuals, builds better organizations, and ultimately creates a more just and caring world,” states the Greenleaf Institute for Servant Leadership. The phrase “servant leadership” was coined by Robert K. Greenleaf in The Servant as Leader and in his thesis he makes several strong statements including the philosophy that, “caring for persons, the more able and the less able serving each other, is the rock upon which a good society is built” (Greenleaf Center for Servant Leadership, 2016). This type of leadership style is set up to serve each member in the group. In a diverse workplace, this type of leadership would be beneficial in that it allows for personalized management and can help to establish cohesiveness in a team environment (Root, 2007). In other forms of management styles, the needs and visions of the company are often put ahead of the needs of the staff. In the servant leadership setting, all input and opinions are taken into consideration which helps to build loyalty from staff, can improve feelings of teamwork, and increase staff morale (Root, 2007). When employees are satisfied with their jobs and their company, workplace productivity tend to rise.

Transformational leaders work with their employees to implement change. This type of leadership involves creating a vision for followers and guiding changes through inspiration and motivation. This theory was developed by James Burins in the 1970s with the basic assumptions that, “Both leaders and followers have the ability to “raise each other to the highest levels of motivation and morality” (Marquis & Huston, 2017, p. 50).” (Grand Canyon University, 2018). “In Burns’ view, transformational leadership has the potential to motivate followers to satisfy higher-level needs, such as self-esteem and self-actualization. Those influenced by transformational leaders find meaning and value in their work, make significant contributions to the success of their employing organization, and become leaders’ themselves” (Rose O. Sherman, 2019). Transformational leaders use the following four elements when leading others; idealized influence, inspirational motivation, intellectual stimulation, and individual consideration. “Transformational leadership focuses on empowerment, viewing errors as learning opportunities, and valuing innovation which means staff members have a means of continually providing input about how to improve care. As a result, transformational leadership can revitalize healthcare from the point of patient care and more” (Rose O. Sherman, 2019). This can be a very powerful approach, but the visionary, big-picture aspect can distract the leader from the day-to-day tasks. It works best when the leader has a detail-oriented subordinate to keep him things grounded.
