She has associated her symptoms with nausea, upset stomach with all foods. Denies associated symptoms of hematochezia, melena, hemoptysis, abdominal pain, fever, chills, pain or any other symptoms. 

PMH: 

H. Pylori infection gastritis

Diabetes Mellitus, type 2

Surgeries: None

Allergies: NKDA

Vaccination History: Up-to-date

Social history:

High school graduate, married and no children. He frequently eats out in restaurants. He drinks one 4-ounce glass of red wine daily. He is a former smoker that stopped 3 years ago.

Family history:

Both parents are alive. Father has history of DM type 2, Tinea Pedis. Mother alive and has history of atopic dermatitis, tinea corporis and tinea pedis. 

ROS:

Constitutional: Negative for fever. Negative for chills. 

Respiratory: No Shortness of breath. No Orthopnea. 

Cardiovascular: No edema. No palpitations. 

Gastrointestinal: No vomiting. +Dyspepsia. + Nausea. No constipation. No melena. No abdominal pain.

Physical examination:

Vital Signs

Height: 5 feet 5 inches Weight: 140 pounds BMI: 31 obesity, BP 110/70 T 98.0 po P 80 R 22, non-labored

ABDOMEN: No abdominal distention. Nontender. Bowel sounds + x 4 quadrants. No organomegaly. Normal contour; No palpable masses.

Labs day of visit:: Hgb 15.2, Hct 40%, K+ 4.0, Na+137, Serum Creatinine normal 1.0, AST/ALT normal. TSH 3.7 normal, glucose 98 normal

Assessment:

Primary Diagnosis: Recurrent H. Pylori infection gastritis

Secondary Diagnoses: Dyspepsia

Differential Diagnosis: Peptic Ulcer Disease

Previous medication plan: two months ago and failed.

  1. Clarithromycin      500 mg po BID for 2 weeks
  2. Omeprazole      40 mg po BID for 2 weeks and then po daily.
  3. Cipro      500 mg po BID for 2 weeks

Plan: Tests

Pt had EGD done 2 weeks ago that showed H. Pylori positive gastritis in biopsy results. 

Urea breath test 8 weeks after treatment with H. Pylori medications. Pt needs to stop PPI’s 2 weeks prior to Urea Breath test. 

Labs: No new labs are needed.  

Referrals: may refer based on effect of medication therapy given for 2 weeks.

Follow up: return to office in 8 weeks to reevaluate symptoms.

As a future nurse practitioner, it is important that you determine the medications used for recurrent H. Pylori infection. 

Please discuss new therapy guidelines for H. Pylori treatment, and provide patient education.

During a job interview, it is highly suggested to always make eye contact with your potential employer or anyone in the room for that manner. Always be sure to greet with a smile and although you may be nervous, you want to maintain confidence. The best way to maintain confidence is to study your resume so that you are prepared to answer any potential questions in regard to skills and prior experience. Another form of adding to your confidence during an interview would be to research the company prior to your interview date. This will not only give you some insight on the company, but it will allow you to ask questions during your interview and many companies and potential employers love this quality.

What is your biggest Weakness?

I would have to say my biggest weakness might be that at times I may attempt to perform too many tasks at once, which I may be overwhelmed and it stress out myself. However at that time I use my biggest strength that consist in take a moment for deep breath, mentally organize my task list and determinate those with priority.

Describe how you resolved conflict with a co-worker or patient?

Working in a BH unit as a psych nurse I can said that in my job environment the most difficult situation involving a patient is management of an aggressive situation. A patient had become highly aggressive both verbally and physically. At this time, my team and I performed the techniques of de-escalation as per protocol. When this was not successful, I, as the lead nurse had to make the difficult decision to call the psychiatrist and receive an order for physical and chemical restraint for the safety of all, the patient, milieu and staff. The psychiatrist led the order and the patient was placed on a four-point restraint in addition to receiving medication to assist in calming the patient. The goal should be always, controlling the situation with the minimal physical and psychological damage for patients and staff members. Always keeping a professional, calmed, team working and positive attitude.

Tell me about yourself?

I am naturally from Cuba. I am privileged enough to be a mother whilst attaining a professional education and career. In 1997, I had the honor of graduating from General Medicine in Cuba, which would be the equivalence of Family Medicine in the United States. I then came to the U.S. and obtained my Registered Nurse certification by 2007. At this time, I began working in a home health setting. This basically involved, but was not limited to: wound care, diabetic management, colostomy management and performing intravenous antibiotics. In 2011, I then began a career at an inpatient psychiatric hospital setting. In 2017 I began my FNP course which I about to finish in this university. Working in the BH environment is what has motivated me to attain a future Post Master in Psychiatry.

A nurse chairing a political position will have adequate knowledge on how policies will work in the healthcare sector and how LGBTQ community could be protected. The political position will facilitate the enhancement of the health care services for all the patients regardless of the category (Groenwald andEldridge, 2019). The political action will facilitate the improvement of the LGBTQ as awareness to the public reducing the stigma associated with them due to non-acceptance in the society.  

The nurses can also obtain training and be involved in city councils and committees where they can raise issues of concern within the industry. Participating in these trainings on political issues will facilitate the enhancement of the needed knowledge to the community on the LGBTQ. The participation in the committees will facilitate the enlighteningof the society on the healthcare delivery for all the patients (Kung and Rudner Lugo, 2015). The involvement will generate an opportunity for nurses to protect the LGBTQ from being denied some services and priorities.

Volunteer to engage in conferences or activities related to strategy. Prepare a fact sheet and help prepare a document to educate policymakers. Inform stakeholders regarding events that provide incentives for policymakers to tackle.

Conclusively, there is a need for changes to be effected on policies relating to LGBT rights to ensure they feel safe in society. As such, nurses should politically intervene with members of LGBT. They have the potential to have a profound global impact on politics. Nevertheless, it is nurses ‘ ethical and professional duty to intervene in policies they deem to be affecting their patients, LGBT notwithstanding.
