Community Assessment and Analysis Presentation

The RN to BSN program at Grand Canyon University meets the requirements for clinical competencies as defined by the Commission on Collegiate Nursing Education (CCNE) and the American Association of Colleges of Nursing (AACN), using nontraditional experiences for practicing nurses. These experiences come in the form of direct and indirect care experiences in which licensed nursing students engage in learning within the context of their hospital organization, specific care discipline, and local communities.

This assignment consists of both an interview and a PowerPoint (PPT) presentation.

Assessment/Interview

Select a community of interest in your region. Perform a physical assessment of the community.

1. Perform a direct assessment of a community of interest using the “Functional Health Patterns Community Assessment Guide.”
2. Interview a community health and public health provider regarding that person’s role and experiences within the community.

Interview Guidelines

Interviews can take place in-person, by phone, or by Skype.

Develop interview questions to gather information about the role of the provider in the community and the health issues faced by the chosen community.

Complete the “Provider Interview Acknowledgement Form” prior to conducting the interview. Submit this document separately in its respective drop box.

Compile key findings from the interview, including the interview questions used, and submit these with the presentation.

PowerPoint Presentation

Create a PowerPoint presentation of 15-20 slides (slide count does not include title and references slide) describing the chosen community interest.

Include the following in your presentation:

1. Description of community and community boundaries: the people and the geographic, geopolitical, financial, educational level; ethnic and phenomenological features of the community, as well as types of social interactions; common goals and interests; and barriers, and challenges, including any identified social determinates of health.
2. Summary of community assessment: (a) funding sources and (b) partnerships.
3. Summary of interview with community health/public health provider.
4. Identification of an issue that is lacking or an opportunity for health promotion.
5. A conclusion summarizing your key findings and a discussion of your impressions of the general health of the community.

While APA style is not required for the body of this assignment, solid academic writing is expected, and documentation of sources should be presented using APA format ting guidelines, which can be found in the APA Style Guide, located in the Student Success Center.

JC (Joint Commission) 

            The mission statement that I have gathered about this organization is that of continuously improving healthcare for the public (The Joint Commission (n.d.). They take into consideration of that of the stakeholders, they evaluate the healthcare organization and inspire them to provide a safe and effectively high quality of care and value to all patients. The joint commission organization can be considered as an accreditation since it is an organization that is not for profit. I would say that the information that is collected and placed on the organizations websites is placed there for the external stakeholders so they can view all the changes that might happen to the organization on a regular basis. When it came to finding the quality improvement measures for this organization they were easy to find on the website just by using the search bar. The list of five improvement measures are data knows best, proper planning prevents poor performance, communicate, invest upfront, and don’t be a dinosaur (The Joint Commission (n.d.). To describe some of the improvement tools a little better than their titles like don’t be a dinosaur means be open to changes as that’s one of the main ways to make changes, data knows best means that data helps organizations make decisions when it comes to changes. Those were just a couple ways in which the joint commission sees fit for quality improvement tools.

CMS (Centers for Medicare & Medicaid Services)

The mission statement for the CMS is to ensure that the voices and needs of the populations are present as the agency continues to develop, implement, and evaluate programs and policies (Mission-vision-our-work. (2019, June 19). The vision of the CMS organization is for all beneficiaries to receive the highest level of healthcare. The Centers for Medicare and Medicaid services organization can be considered or listed as being a governmental agency as it helps people all over the Unites States whether they have disabilities as a young person or they are getting ready to retire and start drawing social security. The information listed on the CMS website is like that of the joint commission it is on there so the external stakeholders can see all the information right when it changes. The quality measures for CMS are a little different than that of the joint commission organization. The list of quality improvements measures can be listed as clinical processes, patient safety, care coordination, health outcomes, patient engagement and clinical guidelines just to name some of them off.

Directions:

Part 1: Selection of 15 Articles ( knowledge and giving education in diabetes)

Select 15 empirical articles related to your PICOT question. Use the “Empirical Research Checklist” worksheet to ensure that each article you select meets all of the established criteria. At least one article must demonstrate a quantitative methodology.

Part 2: Research Article Chart

Using the articles acquired in Part 1, provide a summary review of each component using the “Research Article Chart” template.

Part 3: Literature Review

Prepare a Literature Review (Chapter 2) of 2,000-3,000 words for your scholarly project.

Utilizing the major concepts identified in the Topic 4 assignment, further develop each major concept and subtheme by locating 15 more empirical articles related to your project topic (30 articles total: 15 from Topic 4 assignment and 15 from Topic 6 assignment).

Use the “Research Article Chart” as a guide to analyze and synthesize (summarize) the literature into the paper you began in the Topic 4 assignment.

Based upon your review of the 15 additional research articles, expand on your summary of each major concept and your synthesis of the three identified subthemes that support each concept. At the end of each major concept, include a summary statement.

Please describe a situation as an LPN  that you have personally experienced (you should have been involved in this situation).  Brieflydescribe the situation in one paragraph, then describe how you might change the situation or outcome,when you are an RN.  You might want to bring up the critical thinking qualities or other factors that you have learned in this class.  Your scenario should be true and it can be about an interaction with another nurse, staff member, or patient.

If you are not an LPN – use an example that relates to a position in health care.
