A professional presentation style requires the confidence and ability to convey the right image that you want your audience to see. Unfortunately, it’s easy to miss some of the opportunities available to help you present yourself in the best possible light and, as a result, enjoy the success you deserve. Remember to be forthright about what happened during the project and about how data were collected and analyzed, and present the findings so stakeholders and external parties clearly understand the steps in the process. By completing this final activity, you can help others recreate this type of project and enhance outcomes in other patient care settings.

This assessment requires you to prepare a 5–8 minute video discussion script of your care coordination intervention. You have completed the intervention and achieved enhanced outcomes. Now you need to disseminate your findings and provide recommendations to sustain the results of your intervention. You have been asked by nursing leadership to produce a scholarly video for a peer-reviewed media platform. The video will include 5–8 minutes of scholarly discussion covering the topics listed below.
Grading Criteria
The numbered assignment instructions outlined below correspond to the grading criteria in the Disseminating the Evidence Scholarly Video Media Submission Scoring Guide, so be sure to address each point. You may also want to review the performance level descriptions for each criterion to see how your work will be assessed.

1. Analyze care coordination efforts related to clinical priorities based on a PICOT question. 
   • Provide the PICOT question and a brief introduction of the issues.
2. Describe the change in practice related to services and resources that will be available for the interprofessional care coordination team. 
   • Describe the key care coordination efforts.
3. Explain efforts to build stakeholder engagement within the interprofessional team.

1. Initial posting – Health history and physical assessment varies per system and per age of the patient. There are three scenarios listed below. Choose ONE of the scenarios and discuss all of the pertinent history you would ask as a  nurse taking care of this patient. Then discussion physical exam and assessment techniques you would complete for the patient. Finally, what are some nursing interventions and educational topics you would want to address with the family.

a)  A 3 mos old infant presenting to the hospital with mom saying “He just isn’t acting like himself after he fell off the bed during his nap.” The skeletal survey revealed  multiple rib fractures in various stages of healing  and Head CT is positive for a contusion.

b) A 12 year old child presenting to the ED with his third visit for asthma exacerbation in the last three months.

c) An 85 year old gentleman with severe confusion, disorientation, and loss of memory is admitted with chest pain.

  • Write a quality improvement proposal, 5–7 pages in length, that provides your recommendations for expanding a hospital’s HIT to include quality metrics that will help the organization qualify as an accountable care organization.
    Health care has undergone a transformation since the release of the Institute of Medicine’s 2000 report To Err Is Human: Building a Safer Health System. The report highlighted medical errors as a contributing factor leading to poor patient outcomes. The Institute of Medicine challenged organizations to implement evidence-based performance improvement strategies in order to improve patient quality and safety. Multiple governmental and regulatory agencies, such as the Centers for Medicare and Medicaid Services (CMS) and the Agency for Healthcare Quality and Research (AHRQ), vowed to strengthen and improve incentives for participation, safety, quality, and efficiency in accountable care organizations (ACOs).
    Health  information technology (HIT) performs an essential role in improving health outcomes of individuals, the community, and populations. Health organizations, consumer advocacy groups, and regulatory committees have made a commitment to explore current and future opportunities that HIT offers to continue momentum to meet the Institute of Medicine’s goal of improving safety and quality.
    Understanding HIT is important to improving individual, community, and population access to health care and health information. HIT enables quick and easy access to information for both patients and providers. Accessible information has been shown to improve the patient care experience and reduce redundancies, thereby reducing health care costs.
    This assessment provides an opportunity for you to make recommendations for expanding a hospital’s HIT in ways that will help the hospital qualify as an ACO.

As faculty members, you have been asked to design a 4-hour Leadership Strategies course that would accommodate the learning styles for a selected 10 students. The students have all completed a learning style inventory in which three of the students are determined visual learners, four are kinesthetic learners, and the rest are auditory learners.
