From the chapter “The Changing United States Health Care System” in your course textbook, using either the story of Wald’s efforts to improve the health status of impoverished immigrant communities or the story of how advanced practice registered nurses have struggled with their legitimacy within the health care delivery system, compare your own experience in nursing.

• Have there been challenges you have faced? If so, what strategies did you employ to overcome these challenges?

Cultural competency means to be respectful and responsive to the health beliefs and practices—and cultural and linguistic needs—of diverse population groups. Discuss strategies to provide care to patients who are from different cultures and may have different cultural beliefs. For example, what might the APN do if the patient refuses a high-quality, evidence-based treatment meant to cure the illness because of cultural beliefs? How can the APN respect cultural beliefs while still providing quality care? In what ways can the nurse do more than simply “do no harm?”

Silence Can Also Be Deafening (Marquis & Huston, 2012)

You are a nursing student and share an apartment with a friend who dropped out of school several months ago. Your roommate works as a bartender at a local restaurant four evenings per week and often doesn’t get home until the wee hours of the morning. When she was in school, her general routine would be to come in quietly and go to bed so that she could be up early for classes the next day. Since she dropped out of school, however, she often brings home her coworkers who party all night and then sleep on the sofa or floor in your apartment. This has made it very difficult for you to get the sleep you need to be clear-headed for your early morning clinical courses, and you resent the loss of privacy in your apartment.

When the situation first began, you attempted to talk to your roommate about the problem. She became very angry and accused you of being jealous that she “gets to have fun and you don’t.” She refused to discuss the situation further. Since then, she has been cold and aloof, and the situation in the apartment has become worse, not better. When you come out at night and ask her to turn the music down, it is often turned up even louder as soon as you go back to your room. You can hear her and her friends mocking you. Today, you discovered toothpaste squirted inside your shoes and your stethoscope was hung on the toilet.

When you attempt to confront your roommate about the latest incidents, she states she does not have time to talk and that “this is your problem, not mine.” Your lease does not end for 6 months, and you do not have the financial resources to simply walk away and find a new place to live.

Comprehensive Physical Assessment

This Comprehensive Assessment activity provides the opportunity to plan and conduct a full health assessment on a patient in a single clinic visit. Students spend, on average, three hours on this activity.

Before entering the clinic room you will complete the Plan My Exam activity to organize the exam procedures you will use into a sequence that moves smoothly from head to toe by mapping them to the region of the body where you will perform them. After you have planned your exam, you will enter the clinic room and conduct a Comprehensive Assessment by interviewing and examining your Digital Standardized Patient.

These prompts help you think more deeply about your performance in the activity. Reflective writing develops your clinical reasoning skills as you grow and improve as a clinician and gives your instructor insight into your learning process. The more detail and depth you provide in your responses, the more you will benefit from this activity.

Employee Empowerment & Psychological Safety Interviews and Analysis

To prepare:

• Choose and secure the agreement of three individuals you know well (i.e., family and friends) to be interviewed on the topics of employee empowerment, engagement, and psychological safety in the workplace. One person you interview must work in a healthcare setting.
• Access and review the document Employee Interviews: Guidelines and Questions for guidance on planning, conducting, analyzing, and comparing the interview responses.

Note: You are required to submit the signature of each person you interview to confirm the individual’s agreement to participate. Fill in Page 1 of the Employee Interviews: Guidelines and Questions document, print the page, and obtain the signatures. (There is a place for all three signatures on Page 1.) Scan and submit Page 1 as a separate Word file with your completed Assessment.

• Consider definitions of “employee empowerment,” “employee engagement,” and “psychological safety” that clarify these concepts to explain to your interview subjects in enlisting their participation and guiding responses.
• Conduct a brief (20–30 minutes) phone or in-person interview with each individual. Speak with each person separately so that you have three sets of independent views to compare. Questions to ask, including a sample script for explaining questions as needed, is provided in the Guidelines document. Note the questions are repeated in a section for each interviewee. Take notes in the document, or record the interviews with each person’s permission.
• When you have completed all three interviews, review each individual’s experiences and sense of empowerment, engagement, and psychological safety at work. Compare and contrast the responses with the goal of arriving at clear conclusions on how the degree of employee empowerment, engagement, and psychological safety influences job performance, satisfaction, and morale.
• Compare the responses of the individual who works in healthcare and your own experiences in professional practice in healthcare to the responses of your interviewees working in other areas. Consider conclusions you can make about employee empowerment, engagement, and psychological safety in healthcare compared with other professions.
• Consider quality care and safety issues in healthcare and the relationship among psychological safety, employee empowerment, and engagement as positive or negative factors.
• Consider strategies nurse executives can apply to improve or enhance employee empowerment, engagement, and psychological safety within healthcare settings.

To complete this Assessment:
Based on your three interviews and drawing on current literature, and from the perspective of a nurse executive’s role in supporting employee job performance and satisfaction, write an analysis that addresses, in 4–5 pages, the following points:

• Describe the three employees you interviewed, including their industries and job titles.
• Write your working definitions of “employee empowerment” and “employee engagement” that are the basis of the interviews.
• Summarize how the interviewees characterized the culture of employee empowerment and engagement within their organizations.
• Analyze how employee empowerment and employee engagement within healthcare organizations compare with other industries. Be specific and provide examples.
• Write your working definition of “psychological safety” that is the basis of the interviews.
• Summarize how the interviewees characterized the presence, absence, and/or degree of psychological safety within their organizations.
• Analyze how psychological safety within healthcare compares with other industries. Be specific and provide examples.
• Analyze how degree of psychological safety, employee empowerment, and engagement are positive or negative factors for quality care and safety issues within healthcare.
• From the position of a nurse executive, explain at least two leadership strategies you would recommend for actions to improve or enhance psychological safety, employee empowerment, and engagement within healthcare organizations. Speak generally, or specifically, to your current healthcare organization or one where you have worked in the past.
