These incidents continue to outline the importance of physical exams and health screenings for teenagers, especially those who play sports. During these health screenings, examiners check for cardiovascular alterations such as heart murmurs because they can be a sign of an underlying heart disorder. Since many heart alterations rarely have symptoms, they are easy to miss if health professionals are not specifically looking for them. Once cardiovascular alterations are identified in patients, it is important to refer them to specialists who can further investigate the cause.

Consider the following scenario:

A 16-year-old male presents for a sports participation examination. He has no significant medical history and no family history suggestive of risk for premature cardiac death. The patient is examined while sitting slightly recumbent on the exam table and the advanced practice nurse appreciates a grade II/VI systolic murmur heard loudest at the apex of the heart. Other physical findings are within normal limits, the patient denies any cardiovascular symptoms, and a neuromuscular examination is within normal limits. He is cleared with no activity restriction. Later in the season he collapses on the field and dies.

An elderly, Asian-looking man is admitted
to the emergency room with chest pain;
difficulty breathing; diaphoresis; vomiting; pale, cold, clammy skin; and apprehension.
Three people, speaking a mixture of Englis
h and a foreign language to one another,
accompany him. The nurse tries to speak E
nglish with the man, but he cannot understand
anything she says. Accompanying the elderly
man are two women (one elderly and very
upset and one younger who stands back from the other three people) and one younger
man.
The younger man states that the elderly ma
n, whose name is Li Ying Bin, is his
father; the elderly woman, his mother; and
the younger woman, his wife. The son serves
as the translator. Li Ying Bin comes from a sm
all village close to Beijing. He is 68 years
old, and he has been suffering with minor ch
est pain and has had trouble breathing for 2
days. He is placed in the cardiac room, and the assessment continues.
Mr. Li is on vacation, visiting his son and
daughter-in-law in the city. His son and
daughter-in-law have been married for only 1 ye
ar, but the son has lived in the West for 7
years. Mr. Li’s daughter-in-law looks Chin
ese but was born in the United States. She
does not speak very many words of Chinese.
Further physical assessment reveals that Mr
. Li has a history of “heart problems,”
but the son does not know much about them. Mr. Li had been to the hospital in Beijing
but did not like the care he
received there and returned home as soon as possible. He goes
to the local clinic periodically when the pa
in increases, and the h
ealth-care provider in
China used traditional Chinese medicine,
herbs, and acupuncture. In the past, those

treatments relieved his symptoms.
Medications are ordered to relieve
pain, and Mr. Li undergoes diagnostic
procedures to determine his cardiac status. The
studies reveal that he
did sustain massive
heart damage. Routine interventions ar
e ordered, including heart medications,
anticoagulants, oxygen, intravenous fluids, be
drest, and close mon
itoring. His condition
is stabilized, and he is sent to the cardiac intensive-care unit.
In the cardiac unit, the nurse finds Mrs.
Li covering up Mr. Li until he sweats, and
Mrs. Li argues with the nurse every time
her husband is supposed to dangle his legs. She
complains that he is too cold and brings in
hot herbal beverages for him to drink. She
does not follow the nurse’s and physician’s orde
rs for dietary restrictions, and she begins
to hide her treatments from the staff. Her
son and daughter-in-law tr
y to explain to her
that this is not good, but she continues the
traditional Chinese medicine treatments.
Mr. Li is a very quiet patient. He li
es in bed and never calls for help. He
frequently seems to be meditating and exercisi
ng his arms. When he does talk to his son,

This case study is a composite of actual situ
ations. Marianne, who is American, and Ken
Shimizu, who is Japanese, have worked
in Tokyo for over 30 years as Methodist
missionaries. They have annual furloughs and
occasional sabbaticals, during which they
visit relatives and sponsoring
organizations and engage in
continuing education in the
United States. They met as college students
in the United States, and their three grown
children have established their ow
n careers in the United States.
Ken’s 98-year-old mother resides with
Marianne and Ken. She is not Christian
but has always been extremely supportive of
Ken and Marianne’s work. Ken teaches at
a large Christian university, whereas Marian
ne has served in various church-related
positions over the years. As missionaries, they live in subsidized post–World War II
housing near Ken’s university. Marianne has
been a frugal housewife, preparing local
foods in the Japanese style for her family.
Ken, who is nearly 60, recently learned
that he has glaucoma. By the time it
was discovered, he had lost a signific
ant amount of peripheral vision. Although
Marianne delivered all three
children at a Christian hos
pital in Tokyo, she gets her
annual physical examination when visiting rela
tives in the United States. She has never
believed that the Japanese health system is
as proactive as that in the United States.
On her most recent visit to the United
States, Marianne learned that she has
hypertension. Her physician prescribed a medica
tion that is readily available in Japan,
but the physician was concerned about the leve
l of stress in Marianne’s life. Mother
Shimizu is quite confused and requires considerable care, but it is unthinkable for Ken,
the only child, to put his mother in a long-

HPI: E.D a 38 -year-old Caucasian female presents to the clinic with complaint of pain, itching, inflammation, and “yellow” right great toe. She noticed that the toe was moderately itching after she took a shower at the gym. She did not pay much attention. About two weeks after the itching became intense and she applied Benadryl cream with only some relief.  She continued going to the gym and noticed that the itching got worse and her toe nail started to change color. She also indicated that the toe got swollen, painful and turned completely yellow 2 weeks ago. She applied lotrimin  AF cream and it did not help relief her symptoms. She has not tried other remedies.

Denies associated symptoms of fever and chills. 

 1.According to the AAFP/CDC Guidelines, what antifungal medication(s) should this patient be prescribed, and for how long? Write her complete prescriptions using the prescription writing format in your textbook.

 Onychomycosis is a fungal infection of the nail, either fingernail or toenail. The common dermatophyte that is found in onychomycosis is tinea unguium , with candida infections also a cause . Effective treatment usually involves months of systemic antifungal medication, commonly griseofulvin, ketoconazole ,itraconazole or terbinafine. Topical treatment is usually not effective except for ciclopirox nail lacquer (Woo & Robinson, 2016).

 

Terbinafine 250mg 1 tab PO daily x 12 weeks

 2.  What labs for baseline and follow up of therapy would you order for this patient?

Liver enzymes and complete blood count should be monitor every 6 weeks if treatment last longer than 6 weeks. 

 Terbinafine is an orally and topically active allylamine fungicidal agent which is used to treat superficial fungal infections of the skin and nails.  Terbinafine has been clearly linked to rare instances of acute liver injury that can be severe and sometimes fatal.

Drug induced liver injury due to terbinafine was identified shortly after its introduction into medical use.  Oral therapy with terbinafine is associated with elevations in serum aminotransferases in less than 1% of patients and the elevations are generally asymptomatic and resolve without stopping therapy.  The estimated probability of developing elevated serum aminotransferase levels requiring stopping treatment is about 0.31% for 2 to 6 weeks’ treatment and 0.44% for treatment longer than 8 weeks (“Terbinafine”, 2019).

Follow up

According to the recommended guidelines, what are the non-pharmacological approaches to Onychomycosis? Provide patient education. Keep in mind the past medical history of this patient.

Patient should be referring to Podiatrist for follow up. Foot care is essential for the diabetic patient, and should definitely follow up with podiatrist for further treatment.  

Patient will also be referred to endocrinologist for follow up due to her history of diabetes mellitus type 2. There is no documentation that diabetes is being treated. Patient is obese which may indicate she is not following an appropriate diet.  A referral will also be given to a dietician due to obesity (BMI: 31). 

According to Woo and Robinson Vicks VapoRub is commonly used for onychomycosis. A pilot study was conducted by Derby, Rohal, Jackson, Beutler, and Olsen (2011), to test Vicks VapoRub as a non-pharmacological approach to treat onychomycosis. The study included 18 participants and 83% had a positive response to this treatment after a course of 48 weeks. This study was done with a small sample; however Vicks VapoRub still showed to be a safe and cost-effective non-pharmacological treatment of onychomycosis (Derby, Rohal, Jackson, Beutler, and Olsen, 2011). 

Patient was educated on importance of an adequate diet that includes appropriate daily servings of fruits and vegetables, food low in saturated fat and low cholesterol. Patient was encouraged to continue being active, getting at least 30 minutes of regular physical activity.  Also encouraged to start taking Omega 3 fatty acids to help increase her HDL which is low (38mg/dl) and this is considered a risk factor for heart disease.

Work Environment Assessment- this is my discussion

Using the Clark Healthy Workplace Inventory (Clark, 2015) my workplace ranks 90, or very healthy.  Why it ranks as healthy or civil is that between units of the hospital there is great communication between units, to the extent that is a need on one unit as far as staffing, another unit will offer to help, this is the norm, and not the exception for our hospital. We also feel supported by management during times of crisis, I work in two distinct areas where things can escalate very quickly, both in inpatient psych, and Children pavilion for adolescent psych patients. We know we will always get backup from other building staff and security if things get escalated where there is danger to patients or staff. 

            Success in an organization comes from hardworking people who have a strong mission and vision with great strategies in place. This depends greatly on the leadership, management, and the team. To put things in perspective, the introduction of the workplace health status would give a broader picture. To be discussed below is a brief description of the results of the Work Environment Assessment at Chicago Lakeshore Hospital and based on the results, the explanation of how civil is this place with the reasons behind the claim. Also, this discussion includes a description of a situation where incivility in the workplace has happened and how was it addressed.

Workplace health is the status of the workplace concerning the workers, how civil is the place, and the overall environment safety for the employees. Lack of civility at the workplace contributes to poor employees’ job satisfaction, lack of courtesy and respect among peers, and it leads to poor patient outcomes (Tips to Improve Civility in the Workplace, 2017). Incivility at workplaces has the potential to emotional and physical distress to the workers and that can cause poor quality care and patient outcomes (Armstrong, 2017). To assess the civility of the workplaces, researchers have discovered tools to help assess the workplaces and the Work Environment Assessment is one of the tools. 

Using the Work Environment Assessment tool to assess Chicago Lakeshore Hospital revealed 90 which is a moderately healthy place. This means, in this organization, people live by the shared mission and vision, there is trust and respect among the workers, and communication at all levels is clear (Clark Healthy Workplace Inventory – americannursetoday.com. [n.d.]). The result assures the presence of teamwork and high levels of employees’ satisfaction. The workload is distributed equal and the organization offers competitive salaries. Based on my hospital workplace assessment results, the majority of the employees would recommend the place as a good place to work and overall there are high levels of employee satisfaction, therefore better outcomes (Clark Healthy Workplace Inventory – americannursetoday.com. [n.d.]).

Incivility incident at the workplace

          Courtesy and respect are not only meant for people of a certain rank within the organizations. These basic values and others are for everyone. Clark (2017), emphasizes all nurses have the obligation to foster healthy workplaces with an atmosphere of dignity, professionalism, and respect. Unfortunately, studies reveal incivility at workplaces, bullying, and lack of mutual respect (Armstrong, 2017). Incivility at the workplace has been linked to poor performance which impacts patient safety and the quality of patient outcomes (Armstrong, 2017). Marshall and Broome (2017) state that “workplace violence, incivility, and bullying are barriers to developing safe environments for providers and can occur in any setting” (p.261). The American Nurses Association (ANA; n.d.) defines incivility as “one or more rude, discourteous, or disrespectful actions that may or may not have a negative intent behind them” (para.2).  This brings back the memory of my former workplace. Doctors were very much valued and respected, but the nurses did not receive the same respect from the doctors. At times, nurses were hesitant to call the doctors for notification of patient condition change simply because of the fear of the expected response. There was no mutual respect. I am not sure how did this problem resolve after leaving the place but a few years down, the hospital was sold out to another company.

Conclusion

Civility at the workplace is not only important for the workers’ feelings it also influences overall performance, productivity, and patient outcomes. The consequences of incivility at workplaces “can result in life-threatening mistakes, preventable complications, and harm or even death of a patient” (Clark, 2017). It is with that intensity that the workplace health assessment should be one of our priorities. Despite the individual differences, we must find a way to improve civility at our workplaces. The first step towards improvement is to assess the current status and the Work Environment Assessment is a great tool.
