I work on a Labor and Delivery/Postpartum unit. I work the night shift for the past 19 years and I have seen the high rate of nurse turnover and struggle with nurse retention. Of the varied reasons for discord is the perception and role that the new nurses are expected to jump into after a harried orientation. I also see the inability to conform or endure the role of a night nurse on a very busy unit. 

Expectations seem to be unrealistic and not well defined when progressing through the orientation process that begins on the “day” shift when a orientee is hired for a night shift position.  According to Mazhindu, D. M., Griffiths, L., Pook, C., Erskine, A., Ellis, R., & Smith, F. (2016), the resilience and devotion to performing the job hired for is not clear or misinterpreted. Also the financial aspect of training or orienting nurses is always increasing. According to Colosi (2018), the 2018 National Health Care Retention & RN Staffing Report shows that the average cost of RN turnover ranges from $38,000 to $61,000 which translates into the hospital cost for turnover  ranging from $4.4M – $7.0M.

I currently work in a sub-acute unit of a long term care center. The center has two units, the senior nursing facility (SNF) and sub-acute unit. My unit is very different from the SNF side because they all have tracheotomies, require total care, most of them have PEG tubes, and their acuity is higher. It is a 27-bed unit and each nurse is usually responsible for 6-8 patients over an 8 hour shift. The unit is divided into four different carts and some carts are more difficult than others. We are not only responsible for medication pass, but we also need to assist our CNAs with ADLs. A major problem the organization is facing is the high turn-over rate. I have only worked at the facility for a year and it took me about 4 months to finally get used to the unit. It is a very difficult unit to work on and many nurses have quit because they are not adequately trained. The length of training is only 1-2 weeks and at most 8 shifts total. Most of the time it is usually less until the nurse is put on the floor by themselves. Luckily, most of the nurses are very nice and helpful, but everyone is busy so it can be hard to stop what you’re doing to help the new nurse. The major problem is the inadequate training that new nurses receive.

Two implications for nursing involve adequately training nurses to improve retention. Providing adequate training to new nurses is vital because not only increases their chances of staying with the organization, but improves their competency in giving their patients the most appropriate care. Another implication is the cost for turn-over and according to Maryniak, Markantes, & Murphy (2017), the cost of turn-over for a clinical RN ranges from $37,700 to $58,400. Training new nurses is not only very costly, but also time consuming as well. Fixing the problem of inadequate training of nurses is vital because if a nurse feels overwhelmed, they are more likely to leave and find a place that prepares them better.

I think that the main issue that my organization is addressing is providing patient safety through quality care. There have been many in-services dedicated to making sure that we keep our patients safe by making sure we follow correct protocols in regards to moving and transferring. The also made sure that the residents that are a fall risk have an armband. Some of them even have mats placed by the bedside as well. According to van Gaal, Schoonhoven, Vloet, Minties, Borm, Koopman, & Achterberg (2010), educating staff and about proper interventions increases patient safety. They also mentioned that passive education (written material and large-scale educational meetings) is less effective than education that is personal and interactive. A great way for an organization to address and resolve patient safety and quality care issues, would be to require the employees to stay-up-to date on evidence-based practices. This can be accomplished by providing the information and requiring them to read the material, answer some questions based on the material, and go through a skills check off list and demonstrate how tasks should be completed. This way, the employee will read the information, be able to write or say the answers based on questions, and implement them in a clinical setting.

2-there are so many changes happening in healthcare today, I agree nurse leaders are important. In the recent years we have seen the insurance companies and the healthcare providers unite, to provide a more efficient system with the focus on cost-effectiveness. These mergers dissolve the traditional building blocks of the healthcare system (Kacik, 2016). The changes will improve patient care and decrease healthcare spending. This relationship will provide evidenced based care and create a more integrated full-service provider. These deals are happening because the consumer is the top priority in healthcare. (Kacik, 2016).

As a case manager in the emergency department, I find people do not know how to navigate the healthcare system appropriately. The emergency department is often used for minor issues, that are not emergencies. The rise of urgent care centers and Walgreens clinics has not deterred people from using the ED for minor, non-urgent care. I recently saw a person come with an irritated cuticle on their finger. I saw a mother bring a toddler with a small rash on his leg. Only a few examples that shock me. This is a huge cost for the health care system that we are currently, tactfully trying to address. There are so many more cost-efficient alternatives in health care. We cannot deny care to anyone, yet education regarding improper use of the emergency department is becoming a top priority.

3-I agree, nurse leaders have taken on the role of negotiator and conflict manager. We have seen many changes recently in healthcare and I believe it is going to continue. I think nurses are resilient, we have faced many changes and managed to carry on. I also feel the delivery of the information is the most important aspect. The staff needs to be gently guided into the changing status. We usually hear about the upcoming changes before they actually occur, which gives us time to process everything. When the changes happen, there are multiple personalities and each individual has their own way of dealing with change. The role of the nurse leader has been an important addition in the hospitals. There is a person to talk to and offer support, answer questions and clarify the changes. Often mangers and administration are not approachable, having a nurse leader is an important asset in our field.

(Huber 292) CQI is something that is relevant to all nurses as we all need to be responsible for continual improvement at work. As professionals we should always have this as a top priority. There are many challenges in the workplace, and by looking for ways to improve, we are constantly learning and growing as a profession, this is a large piece of evidence-based practice, which is something we all hold as a standard in healthcare today.

An example of how I would apply CQI in my current position working as a nurse circulator in the operating room, is to ensure that the time out procedure is followed every single case I circulate. This is important as we often get complacent in rules and regulations, as do surgeons that we are trying to keep happy as they are customers just as much as our patients are.

I had circulated a case with another nurse during orientation in which not all implants for a total knee replacement were in the room. I was not notified of this until after the case started, the patient was anesthetized, and time out had been completed though this requirement was not stated/asked. Later the rep for the implants then stated the implants were in route from another location. This is unacceptable, and I am glad that I was still in orientation at the time this occurred as it was a great learning experience for me. I learned how to write an incident report that day, and why the time out procedure is so important.

5-Health care delivery requires structure (staff, education, equipment, prospective data collection), and process (policies, procedure, protocols), which when integrated provide a system (programs, organizations, cultures) leading to outcomes (patient safety, quality, satisfaction). An effective health care system has all of these elements – structure, process, system, and patient outcomes in a framework of continuous quality improvement, or CQI (Kronich et al., 2015).

The purpose of QCI is to improve health care by identifying problems, implementing and monitoring corrective action and evaluating its effectiveness. Hospitals use a specific process to find areas in the health care delivery system that need improvement. When an area has been found, staff develop and implement strategies for improvement. General areas that are being studied include access to care, continuity of care, the intake process upon admission, emergency care, and adverse patient events, including all deaths (National Commission on Correctional Care, 2018).

In my previous position, working as a NICU RN, we initially did not use two RN’s to verify the content in the TPN-bags for each patient. Shortly after I started working at this facility, this change was implemented. The purpose was to catch medical errors from pharmacy that sadly was occurring more frequently. This new process of reading labels with another RN while verifying the content with the document from pharmacy, and then hanging the TPN required more time in the nurses’ day. In addition, all this was done toward the end of day-shift around 1730. This process resulted in stress among the RN’s and often overtime, which was not appreciated by management. Thankfully, a solution was found by changing the time of day TPN was delivered to the unit. By working together and brainstorming solutions, everyone was satisfied in the end; quality of care was improved, and the nurses were able to finish their duties in time.

6-I agree with your response about having a rapid response team because this helps identify patients at risk for cdoing before the event when you can give care to help prevent a code. I think rapid response teams are a must in facilities. according to patient safety primer .gov 

Rapid response teams represent an intuitively simple concept: When a patient demonstrates signs of imminent clinical deterioration, a team of providers is summoned to the bedside to immediately assess and treat the patient with the goal of preventing intensive care unit transfer, cardiac arrest, or death (psnet.ahrq.gov). I also agree with your staement about the lean technique that is in our text as a useful apporach to imrove efficincy in renedering care. When we are effective communicators and work as a team in our settings, we will definietly be more productive and work loads can be reduced, less stress in the workplace, etc. Team work is amazing when it is used the way it was intended to.
