Discussion: Nurses as Leaders in Health Care Reform

As health care delivery in the United States continues to evolve, either through mandates, improved technologies and training, or other drivers, nurses remain at the forefront in facilitating the success of new initiatives. In 2010, the Institute of Medicine formed a committee of experts to address the following question: “What roles can nursing assume to address the increasing demand for safe, high-quality, and effective health care services?” In the final report of the committee, The Future of Nursing: Leading Change, Advancing Health, the authors stated:

This report offers recommendations that collectively serve as a blueprint to (1) ensure that nurses can practice to the full extent of their education and training, (2) improve nursing education, (3) provide opportunities for nurses to assume leadership positions and to serve as full partners in health care redesign and improvement efforts, and (4) improve data collection for workforce planning and policy making. (p. 10)

For this Discussion, you will focus on the research recommendations presented by the committee concerning the role of nurses as leaders. The committee members believe that answers to these research questions are needed to advance the profession of nursing and to further expand their role in health care reform and improvement.

Review Chapter 7 in The Future of Nursing: Leading Change, Advancing Health report (attached bellow). 

Focus on the information in Box 7.3, “Research Priorities for Transforming Nursing Leadership.”

Select one of the research priorities listed in Box 7.3 that is of particular interest to you and applicable to your career interests. Consider the benefits and challenges of researching and addressing this priority in nursing. 

Using the Walden library, identify two to three current articles that address your selected research priority. Consider the current state of research efforts on this priority.

Reflect on how the research findings for your area of priority impact nurses as leaders in organizations and health care reform. Why is research on this priority important?

Post a description of the priority you selected and the benefits and challenges of further researching this area. 

Provide an overview of the articles you found (using appropriate APA citations) relating to this priority, and highlight any key findings.

 Explain how continued research in this area could strengthen the ability of nurses to lead in both individual organizations and as advocates of health care reform.

Planned change in nursing practice is necessary when focusing on evidence based practice.  Using a change theory improves likelihood of success. When using a change theory the effort looks purposeful, calculated and collaborated (Roussel, 2006). But even with a good implementation, theory, and idea, executing change is more challenging than perceived. For example it is alleged that two thirds of organizational change projects fail (Szabla 2007), others suggest the figure is higher.

In 1951 Kurt Lewin identified 3 stages for change: 1) unfreezing (when change is needed), 2) Moving (when change is initiated), 3) Refreezing (when equilibrium is established). This changed theory is simply written, easy to understand and easy to implement into a personal or organizational change strategy. Lewin discusses forces (his word) or barriers to change, examples (i.e, culture, money, structure, even attitudes).

Lippit’s theory uses nurses jargon, assessment, planning, implementation, and evaluation. Lippit’s theory gives more detailed instruction, allowing for detailed explanation with trying to implement change. Using the nursing jargon will help to deliver the change idea to staff nurses if the change occurs at the bed side.  Lippit’s change theory can also be used with stakeholders, with variation with words.

The idea of using a change theory to introduce needed improvements using EBP whether education, management or organizational appears more organized, strategic, and professional. 

Lippit’s theory will work best for my change project because I will be instructing nursing and secuity staff on how to recognize mental health decompensation.

C hange   theories  define a social problem and identifies a long-term outcome that would signify the problem has been fixed and build a roadmap that will guide for the realization of the long-term goal.  The two change theories I will  be  focusing on are  Rogers change theory  and  Lippitt ’s  change theory .  Rodger’s theory of change has five steps which include: awareness, interest, evaluation,  implementation, and adoption. Rodger’s theory provides for the awareness and participant interest part which will incorporate all the multidisciplinary team in a healthcare facility before the implementation of the HIT similar to Lippitt’s theory of change which comprises of four elements: assessment, planning, implementation and evaluation (Mitchell,  

1. Discuss how an uncontrolled diabetes mellitus can affect vision and lead to blindness

2. Discuss how hearing aids work to correct hearing deficiencies.

3. Discuss the auditory (eustachian) tube and its normal role in pressure equalization.

4. Discuss some of the more common conditions leading to vertigo (dizziness), such as Ménière’s syndrome, otitis media, motion sickness, and diplopia.

5. Clearly differentiate between static equilibrium and dynamic equilibrium.

6. Discuss  how the Rinne test and Weber’s test using tuning forks can be used to determine localization of hearing loss.

7. Describe a scent that triggers a specific memory, such as the smell of newly baked bread or a certain perfume. Note the connection between the olfactory senses and long-term memory.

8. Discuss where the taste buds are located on the tongue.

9. Correlate taste sensations with their chemical counterparts , i.e.  sweet is, of course, sugar detection.

If a regression analysis was to be completed on body mass index (BMI), what could be an independent variable in that analysis? Why? If we could, what other independent variables should be included in the analysis? What statistic(s) would show the value of that regression in understanding BMI?

Alternatively, find an article that uses regression analysis to study a medical concern. In that study, what was the dependent variable and what were the independent variable(s)?  Further, how would you use this study to highlight the difference between correlations and causation?
