Ethical and Economic Policy Challenges in Healthcare

            There is much tension today about policy decisions regarding healthcare due to economic and ethical challenges.  Economically, our nation struggles due to the rising demands of healthcare.  Demands have increased due to the changes in our nation’s demographics as the baby boomers have aged and required more healthcare services (Laureate Education, 2012).  In the article “For Baby Boomers, Health Care Where and When They Want It,” Barr (2014) describes how the baby boomers have much higher expectations than generations before them.  The number of senior citizens, along with the demands of this generation, increases the cost of healthcare.  Also, our requirements as a nation have increased due to significant new technologies (Laureate Education, 2012).  These technologies are welcomed by baby boomers who demand to have vast amounts of information regarding their healthcare (Barr, 2014).

Ethically, the struggle is determining how to supply the care, including new technologies and new treatments, even though it is expensive.  Of course, everyone would agree that we want those who are sick to get the care needed to live a longer, healthier life.  However, with the cost of healthcare, is this feasible?  For example, the Washington Post discusses a new drug, Provenge that costs $93,000 per patient (Stein, 2010, November 8).  Studies show it could extend a prostate cancer patient’s life by about four months (Stein, 2010, November 8).  Some may say this is a lot of money for only four more months of life.  Others may feel a day of life cannot have a price tag.  In the Washington Post, Stein (2010, November 8) mentions that government agencies such as Medicare spending a significant amount of money on a medication that will extend life by four months may prevent our nation from affording other treatments.  In the article, “As Healthcare Costs Continue to Rise, Providers Weigh Care’s Expense and Effect,” Walker (2015) discusses how considering the cost against the benefit of technology would help reduce cost.  Does someone really need to continue getting these expensive serial tests if they remain asymptomatic?  Limiting diagnostic testing to only when it is essential leads to more debate.  The baby boomers and other Americans feel the need to have all information possible (Barr, 2014).  Policymaking to ensure all citizens receive what they believe to be essential healthcare required to live a longer and healthier life remains an ethical and economic challenge.

How do you use informatics (not technology) in your nursing practice? What ethical issues have arisen or might arise from use of informatics in professional nursing?

Professor’s post: Nursing has come a long way since the era of Florence Nightingale.  Everywhere we look we are surrounded by technology and informatics.  Every day we are learning new processes in the world of nursing informatics. Due to this we need to be prepared and ready for any changes to informatics in the future.  Massachusetts Department of Higher Education Nursing Initiative (2016) stated that the Nurse of the Future “uses informatics, and knowledge of the larger healthcare delivery system, to support and enhance patient care” (p. 27).  Along with this we need to make ourselves aware of the challenges we might face, such as ethical issues that may arise from the use of informatics.

LTD Acceptance is a private property and auto insurance carrier that specializes in sports cars and motorcycles. This organization is owned by LTD Capital, a large equity group with over 15 holdings. LTD Acceptance is the parent company’s single largest holding as it drives 70% of total revenue. Due to the inherent risk involved in that segment of the market, many of LTD Acceptance’s competitors do not offer policies for sports cars or motorcycles. This market segment is underserved which is why the organization has 20,000 active policies for a sports car or a motorcycle.

LTD is headquartered in Houston, TX. LTD does not sell insurance directly to the public. Instead, it uses third-party agents to sell its policies. LTD handles all customer service needs including claims intake, policy services, and general questions. The company operates in four states: California, Texas, Louisiana, and Florida. Currently, LTD does not have an active system in place to ensure that its agents are in fact using LTD guidelines to screen potential policyholders. However, no evidence of negligence has emerged so far as the organization has yet to have a year in which it was not profitable. LTD has also had the good fortune of not suffering losses because of natural disasters or catastrophic events.

Organizations in every sector experience breakdowns or failures that compromise the efficiency and efficacy of their operations. This concept is defined as an operational failure. Operational failures have a direct and adverse impact on profitability. As such, the shareholders want assurance that the risk management team is actively working on mitigating the occurrence of these failures. As the newly hired senior risk analyst, you are responsible for outlining a plan to do just that.
For this deliverable, provide a report to your supervisor which provides a framework for managing operational risk exposures. Your report should include the following: 3 pages

• Explain      why the operational losses cannot be broadly managed. Why do they have to      be categorized?
• For      each operational risk exposure listed below, provide at least two examples      of potential losses from the exposure and reasons why you need to manage      it. 
  1. Internal fraud
  2. External fraud
  3. Employment practices and workplace safety
  4. Clients, products, and business practices
  5. Damage to physical assets

3. Provide a scenario (real or fictitious) of incivility in a practice  setting. What was the impact on the individuals involved? How was the  work environment affected?
4. Describe three strategies for creating a healthful environment. Use scholarly nursing literature to support each strategy.
