Collaboration In Patient Education And Compliance With Older Adults 2025

Topic 4 DQ 1 Today, individuals are allowed access to their health information, skills, and descriptions of the many tools that can lead to imporved health and well-being. Family members are always included in teaching when present and can enhance ones’ decision making. Within changemaker health, individuals and families have the skills to practice self-awareness, mindfulness, and empathy so that they can effectively help others and are equipped to make decisions for themselves based on their own health objectives rather than on social or environmental pressures (Ashoka.) There are now accomodations made the the patients feelings and can allow them to have several options before choosing what option they would like to endure and have proceeded with. There are several risk factors and benefits that should be described to the patient before allowing them to make their decision. Empowering them and allowing them to make decisions has enhanced the outcomes for many. When obtaining surgical consents, the patient has several questions asked like whether they wound consent to blood if deemed necessary, they should always be involved in their care and allowed for questions and concerns to be voiced. Collaboration In Patient Education And Compliance With Older Adults 2025

Topic 4 DQ 1 Patients’ participation in decision making in health care and treatment has increased over the years. Active patients’ participation can control their healthcare changes and outcomes. Patient participation in decision making has been highly promoted. Emphasizing the importance of participation in the decision making process motivates the service provider and the health care team to promote participation of patients in treatment decision making. These efforts include enhancement of patient access to multifaceted information providing systems and tools that help patients in decision making. Today we involve both the patient and family member in decision making. Family involvement in decision making helps to build confidence and psychological support, which increases compliance to the treatment. With enhanced patient participation, and considering patients as equal partners in healthcare decision making, patients are encouraged to actively participate in their own treatment process and follow their treatment plan and thus a better health maintenance service would be provided (Vahdat, 2014). The change in practices in healthcare strengthen patients’ position in health care and the change from the traditional supply-induced care into a more demand-induced care. The growing availability of medical information sources has enabled patients to get more information about medical care. Patients could be encouraged to take on the task of understanding the relevant information and to share their values and views with healthcare providers (Van Den Brink‐Muinen et al., 2006). Using 200-300 words APA format with at least two references. Sources must be published within the last 5 years. There should be a mix between research and your reflections. Discussion Reply 2025

What is a workaround? A workaround is known as a way invented by users to bypass the system to accomplish a task, usually indicating a poor fit of the system or technology to the workflow or user (Seaman & Erlen, 2015). The book mentions an incident where they remove the armband from the patient and attach it to the bed (McGonigle & Mastrian, 2018). I witnessed an incident where the barcode was faded and they got a new one but didn’t put it on the patient and left it on their table. The risk of this is not being able to identify the patient because they do not have their armband on. If they go into a different patient room then the people that find them have no way of confirming who they are. The only benefit I see from this incident is being able to identify the armband and scan it in a shorter amount of time. I currently work just like any hospital have the MAR, charting system that automatically locks if there is no activity within a certain amount of time. Bed alarms and call lights are the most important when it comes to patient safety because I work at a rehab center and there are many patients that try to get out of bed on their own when they are not able to stand on their own. This is where the bed alarm comes in handy because we are able to attend to the patient before they attempt to get out of bed. In most cases, they get scared of the alarm and they tend to just freeze until somebody comes in the room and turns it off. There are a couple of things that can be changed to improve patient safety. The facility I work attends to have old beds and computers. They tend to have more issues where the call light doesn’t work because it’s attached to the bed, the computer is lagging, shutting off, or restarting on its own. It can affect patient charting if the information is not saved. After a certain number of years, supplies should be changed to prevent patient injury.

DUE 11/18/2021 BY 5PM….PLEASE FOLLOW ALL INSTRUCTIONS LISTED AND ATTACHMENT.,,,THIS IS A DISCUSSION QUESTION 2025

THIS IS A DISCUSSION QUESTION QUESTION. PLEASE ADD REFERENCES. CASE STUDY Mr. Smith, who has diabetes and hypertension is a patient on a medical surgical unit status post hip surgery. The computer systems in the hospital are down. Nurse Jones, who was covering for a colleague, took a verbal phone order to administer Humalog Insulin to Mr. Smith. Because of a poor telephone connection, Nurse Jones heard the order from the prescribing physician as 20 units of Humalog Insulin instead of 10 units of Humalog Insulin. The bar coding system was also not working so Nurse Jones could not check the order in the system. Nurse Jones administered the Humalog Insulin, and Mr. Smith’s blood sugar dropped precipitously and he passed out. The Nurse Manager disciplined Nurse Jones and told her she might be fired. Using a systems theory, examine the case and propose solutions that would prevent a similar error from happening in the future. Remember that a systems framework moves from a punitive model where individuals are viewed as at fault, to a process model where faults in the system are viewed as resulting in errors.
