1. Go to DSMO website at http://www.hipaa-dsmo.org (Links to an external site.)Links to an external site.. Review the changes that have been requested in transaction standards. Click on one of the active change requests. Review the details and determine which HIPAA transaction standard is affected by the request and which DSMO has expressed interest.
2. Visit the HL7 website (http://www.hl7.org) and locate information on Version 3. Write a brief report on the reference information model and the requirements for vocabularies.
3. Locate federal regulations for either the Meaningful Use standards and certification program (42 CFR 170) or for HIPAA transactions and code sets. Write a brief report on recent changes to these regulations.
4. Work with a small team; identify a task that could be done more effectively using data exchange standards. Describe how the task is performed today, and how data exchange standards today make this task more efficient. Identify the information systems that are involved. Research the existing standards that could be used to solve this problem. Discover whether anyone is currently working on this problem. Report in your findings. Extra credit: Submit a proposal to a standards or profiling body to develop the use case into a solution.

1. Search the internet and identify the vocabulary standards that ONC adopted and against which EHRs must be tested for certification to meet meaningful use criteria. Develop a table or grid comparing the vocabularies required by ONC and the terminologies required for administrative transactions under HIPAA.
2. Using PHIN VADS, locate the value sets associated with National Healthcare Safety Network Healthcare Associated Infection (HAI). Find the value set for type of adverse reactions to an immunization. Write a report identifying the contents of the value set.
3. Using an Internet search engine, access and listen to the CTS2 in action audio (https://wiki.nci.nih.gov/display/LexEVS/Common+Terminology+Services+Release+2+%28CTS2%29+In+Action (Links to an external site.)Links to an external site.). Write a report describing the different ways in which CTS2 can be used.

The CEO and the Board of Directors has tasked you with developing an intake packet for new patients that will reestablish patient trust in the facility, employees, and staff. The patient packet will address new patient concerns by including information about HIPAA, informed consent, a confidential health history report, and a living will. The new packet will also include the values of the organization and a code of ethics.

Your intake packet will start with the Hospital’s Code of Ethics that includes at least 10 ethical points. The code of ethics should include (at a minimum) include an additional 3

• Patient relations
• Physician activities
• Billing for services
• Political activities
• Conflict of interest
• Communication, including social media
• Privacy

The Code of Ethics should be submitted in bullet format in a Word document with an introduction. APA formatting for the ethical points and proper grammar is required.

Upon completion of your code of ethics, please prepare a PowerPoint presentation  in which you explain each ethical point and its relevance in the healthcare industry with examples to the CEO, so that she can answer questions from the Board.

1. The PowerPoint should have a minimum of 5 slides and a maximum of 12.
2. Describe each ethical point in the Code of Ethics.
3. Use the notes area on each slide as needed to expand on the key points.

Throughout this course you will be developing a formal, evidence-based practice proposal.

The proposal is the plan for an evidence-based practice project designed to address a problem, issue, or concern in the professional work setting. Although several types of evidence can be used to support a proposed solution, a sufficient and compelling base of support from valid research studies is required as the major component of that evidence. Proposals are submitted in a format suitable for obtaining formal approval in the work setting. Proposals will vary in length depending upon the problem or issue addressed; they can be between 3,500 and 5,000 words. The cover sheet, abstract, references page, and appendices are not included in the word limit.

Section headings and letters for each section component are required. Responses are addressed in narrative form in relation to that number. Evaluation of the proposal in all sections is based upon the extent to which the depth of content reflects graduate-level critical-thinking skills.

This project contains seven formal sections:

1. Section A: Organizational Culture and Readiness Assessment

3. Section C: Literature Support
4. Section D: Solution Description
5. Section E: Change Model
6. Section F: Implementation Plan
7. Section G: Evaluation of Process

Each section (A-G) will be submitted as separate assignments so your instructor can provide feedback (refer to applicable modules for further descriptions of each section).

The final paper will consist of the completed project (with revisions to all sections), title page, abstract, reference list, and appendices. Appendices will include a conceptual model for the project, handouts, data and evaluation collection tools, a budget, a timeline, resource lists, and approval forms.

Quality Improvement and Patient Safety Initiatives

The vice president of nursing, Dr. Sasha Mendez, has had a lot of complaints from patients and their families about the difficulties they experience navigating the system at her organization; e.g., going from the E.R. to admissions. Dr. Mendez’s nursing specialty area is psych/mental health. As such, she is very concerned about the stress this is causing the patients.

What are the macro systems that are affecting patient satisfaction at Dr. Mendez’s facility? What micro systems should she also analyze to look at how the patient moves through—from entry, to possible admission, to possible discharge? How do these systems relate? What impacts each system?

The concepts presented in Week 8 serve as a foundation for this week as you continue to look at factors within an organization’s environment that are barriers and facilitators for implementing change. In this Discussion, you examine the macro and micro systems within the organization (of your practicum setting) and identify changes that may need to take place to facilitate quality improvement and achievement of patient safety outcomes.

To prepare:

• Reflect on the issue you have been addressing      through your EBP Project.
• Identify the macro and micro systems that      influence the issue. Review the McClusky and Middleton (2010) article for      an example of this process (See attached file).
• Determine the desired outcomes of resolving the  EBP Project issue and how resolving it will improve quality and patient     safety. (Note: Next week you will evaluate these outcomes. You may      want to view the Week 10 Discussion as you prepare.)
• What procedures/processes would need to be      modified to achieve the desired outcomes?
