Mrs. Jones is a 32 year old Caucasian female who presents for follow up on her gestational diabetes.  She is 3 months postpartum and was diagnosed with gestational diabetes.  This was her first pregnancy.  She has a strong family history of diabetes and was told to get it checked out once she stopped breastfeeding.  She has not checked her glucose since she came home from the hospital, stating she has been too busy.  

Allergies: Sulfa

Current medications: Multivitamin

PMH: Gestational Diabetes

PSH: Tonsillectomy, Wisdom teeth extraction 

Social: Denies tobacco, Alcohol, or illegal drug use.  She is married and lives with her husband and new baby.  She works as an elementary teacher.  She follows a regular diet and walks 2 days a week when she has recess duty.

As a master’s-level nurse you will be responsible for promoting equitable quality of care. In the biopsychosocial model of care, there are a number of areas where you can have an impact on improving the quality, safety, and equitablility of care. One example area where equitable quality of care is an issue is with medications, especially considering the cost of various medications to treat the same condition versus the effectiveness and potential side-effects of the medications. A further pharmacological concern for safe and quality care is medication errors. This could include considerations like a patient getting the wrong medication or a dangerous interaction due to other aspects of a patient’s health and treatment situation. An example could be vitamin or herbal supplements the patient has not disclosed as part of the medication reconciliation process.

One way to communicate the need for quality improvements, and your plans for achieving specific improvements, is through a presentation poster. This type of communication tool is used both in the work place and at professional and academic conferences.   • The second is to identify a new issue related to a biopsychosocial (BPS) clinical issue, or a clinical issue related to one of the items from the list of conditions, diseases, and disorders presented in the Resources section for the Concept Map assessment. The focus should be on a community rather than on a specific patient or health care setting.

As a master’s level nurse, your specific focus is on reviewing the implications of the data relevant to the clinical issue you are trying to address. Once you research this, review the aggregate data, and understand the BPS considerations relevant to the clinical issue, you can suggest strategies for improving the quality, equitability, and safety of care around the issue. Basically, I think there are ways in which to decrease the number of falls that my facility could implement that wouldn’t be too difficult to accomplish. One would be to assign the fall risk patients rooms closer to the nurses’ station because we have a few right now that I think are too far and on more than one occasion, I have found certain patients halfway out of the bed. I know reducing falls is a very common project, but I do think it’s vitally important to do so.

The continued adoption of electronics in the hospitals leads to increased delivery of quality care to patients and improvements in health and healthcare administration (Prezerakos, 2018). The most common electronic systems in hospitals include electronic health record (EHR) software, imaging and visualization, medical diagnosis software, appointment scheduling, medical billing, hospital management software, e-prescribing software, medical research, and medical equipment management. EHR is the most common electronic system in hospitals. It is extensively utilized to collect data about patients, including their medical charts, personal information, authorized users, and medications for use. It enables the integration of patient care across various healthcare departments.  The medical database software enables nurses, physicians, and other healthcare professionals to enter and share patients’ data on various digital platforms.
