   Scenario
   You have been hired as a consultant for a large healthcare organization. In your role, you have been tasked with examining the current organizational structure at the micro, meso, and macro levels, including the impact on healthcare delivery.

   As you develop your plans, you will reflect how these roles individually and collectively influence the ability of the organization to deliver high quality client services.

   The roles and titles of people working in the three levels of systems may vary. A brief definition of each is offered to clarify the contributions of leaders at each level. 

   • Micro system: This is the point of contact with the client or customer. In the health care setting the micro level may be a specific unit. Roles of those in the micro system include all that come in contact with the client and the manager of the unit or department. 
   • Meso system: This level reflects a collection of micro systems, such as a Surgical Services Department including multiple units. The meso level aims to group and coordinate similar micro system levels of service. 
   • Macro system: This level is a larger scale of groups and departments typically more at the administration level. 
   • Nursing Hierarchy
     
   • Macro Level: Vice President of Nursing Services
   • Potential Titles: Chief Nursing Officer, Vice President of Nursing, Director of Nursing Services.
   • Meso Level: Program Manager
   • Potential Titles: Department Head, Service Manager, Director.
   • Micro Level: Unit Manager
   • Potential Titles: Bedside Nurse, Unit Nurse, Unit Manager, Change Nurse.

   • Research a healthcare organization and include their mission statement, vision, or philosophy in your memo.
   • Describe how the organization’s mission, vision, and philosophy influence the expectations of leaders at the three system levels: micro, meso, and macro.
   • Describe how nursing leaders in each of these levels of the organization’s operations address health care delivery goals.
   • Describe nurse leaders at the micro, meso, and macro level of the organization including a discussion of how these three levels may integrate to improve outcomes.
   • Provides stated ideas with professional language and attribution for credible sources with correct APA citation, spelling, and grammar.

3. Download the EBP Change Process form (Links to an external site.) during Week 1. The use of this specific form is REQUIRED 
4. Identify a clinical topic and related nursing practice issue you think needs to be changed. 
5. Locate a systematic review on your topic from the CCN Library databases. Be sure this involves nursing actions.
6. Work through each step of the ACE Star Model as outlined on the assignment form (Star Points 1-5: Discovery, Summary, Translation, Implementation, and Evaluation). Respond to the instructions provided on the form.
7. Follow the activities and thinking of Nurse Daniel in Weeks 1-6 in the ‘Illustration’ part of each lesson. He will be working through a clinical topic and nursing practice issue to demonstrate a change (ACE Star Model and systematic review).
8. Work on a portion of the process each week, as the illustration unfolds.

In the state of New York, certain professionals are fit for the role of mandated reporter of child abuse and maltreatment. If the abuse is not reported, the professional that did not report it would be held responsible, in both a civil and criminal manner. All health care professionals are required by New York State law to report suspected child or adult abuse that they may come across in their professional setting. According to NYC.gov, if a mandated reported comes across any reasonable cause to suspect abuse or neglect, it is the mandated reporters responsibility to report the abuse or cause the abuse to be reported, through your supervisor. The reasonable cause is viewed as assessments that the health care professional performed and trust the health care professional’s judgment. Certain important information needed to make a report is the name and address of the patient and family members, the patient’s age, sex, and primary language, nature of the injuries, type of abuse/neglect, and any additional information that was helpful. If a abused patient is discovered during initial assessment, the patient’s safety is ensured first, by removing any factors that may cause the patient harm, such as abusive family members or heath care workers. Next the supervisor and social worker on the patient’s case would be notified. Thirdly, the patient is more thoroughly assessed to uncover any other signs of abuse. Finally, the patient is not allowed to leave until the police arrive and the report is filed properly.

  Review your state’s mandated reporter statute. Provide details about this in your post. If faced with a mandated reporter issue, what are the steps in reporting the issue? Create a mandated reporter scenario and post it. Respond to one of your peer’s scenarios using the guidelines for submission/reporting in your state. Be sure to include a reference to your state’s website related to mandated reporting.
