When was the last time you read Meditation XVII of John Donne’s Devotions Upon Emergent Occasions?

Unless you are a student of seventeeth-century poetry, you may not be all that familiar with this piece. However, you may be much more familiar with one of its well-known phrases: “No man is an island…”.

As you begin your journey toward achieving your academic and professional goals, you have a great opportunity to network with academics and professionals who can help ensure you do not travel alone. This network can help to clarify your own vision for success and can help guide you now and in the future. To paraphrase Donne, no one is an island.

Begin creating an academic and professional network by identifying which academic and professional connections and resources with which you need to collaborate to succeed in your MSN program and as a practicing nurse.

To Prepare:

· Consider individuals, departments, teams, and/or resources within Walden University and within your profession that you believe can support your academic and professional success.

· Identify at least two academic and at least two professional individuals, colleagues, or teams that might help you succeed in your MSN program and as a practicing nurse.

Academic and Professional Network

Complete Part 1 of your Academic Success and Professional Development Plan Template. Be sure to address the following:

· Identify at least two academic and at least two professional individuals or teams to collaborate with to be successful in your MSN program and as a practicing nurse.

· Explain why you selected these individuals and/or teams and how they will support your success in the MSN program and as a practicing nurse.

To prepare

• Review the strategies presented in the “Theoretical Foundation for Research” document provided in the Week 1 Learning Resources. Examine the literature review information presented in this week’s Optional Resources, and evaluate other strategies or tools you have used in previous coursework. Identify strategies/tools that you will use in this course to manage information from the literature and record your own thought processes while investigating a phenomenon of interest and developing a theoretical foundation. Begin to use these strategies/tools as you prepare for this Discussion.
• Select a phenomenon of interest to you. Ideally, the phenomenon you choose will relate to the dissertation research you plan to conduct later in this PhD program or other research related to your focus area. (You will submit your phenomenon for Instructor approval this week.)
• Conduct a search of the literature from nursing and other disciplines to see what has been written about your phenomenon.
• Based on your review of the literature and your own interests as a scholar-practitioner, identify elements that you may use to develop your theoretical foundation for a future program of research. This includes concepts, conceptual relationships, frameworks/theories, and graphic models related to your phenomenon of interest. (Bear in mind that these will likely evolve as you proceed through the course.)
• Consider how developing elements of a theoretical foundation may be advantageous as you strive to make a positive contribution in research, practice, education, and/or leadership.

By Day 3 

Post your phenomenon of interest. Describe concepts, conceptual relationships, frameworks/theories, and/or graphic models that may be appropriate for addressing your phenomenon. Explain how developing these theoretical elements may be beneficial as you to strive to make a positive contribution in practice, research, education, and/or leadership.

Regardless of political affiliation, individuals often grow concerned when considering perceived competing interests of government and their impact on topics of interest to them. The realm of healthcare is no different. Some people feel that local, state, and federal policies and legislation can be either helped or hindered by interests other than the benefit to society.

Consider for example that the number one job of a legislator is to be reelected. The cost can be measured in votes as well as dollars. Thus, it is important to consider the legislator’s perspective on either promoting or not promoting a certain initiative in the political landscape.

To Prepare:

• Review the Resources and reflect on efforts to repeal/replace the Affordable Care Act (ACA).
• Consider who benefits the most when the policy is developed and in the context of policy implementation.

Write a 2- to 3-page paper that addresses the following:

• Explain the ethical and legal implications of the scenario you selected on all stakeholders involved, such as the prescriber, pharmacist, patient, and patient’s family.
• Describe strategies to address disclosure and nondisclosure as identified in the scenario you selected. Be sure to reference laws specific to your state.
• Explain two strategies that you, as an advanced practice nurse, would use to guide your decision making in this scenario, including whether you would disclose your error. Be sure to justify your explanation. 
• Explain the process of writing prescriptions, including strategies to minimize medication errors.

• Review the Patient Portal Snapshot Scenario. Select one patient for your assessment. Prepare an individualized patient-focused handout for that patient that incorporates care coordination. For the video component of the assessment, you can use the Brightspace video tool or submit a .mp4. Please note the Brightspace video tool is limited to 3 minutes but the video can be submitted in 2 parts.

As a nurse at Memorial Hospital–a large city hospital, you provide care to diverse patients. As you work the nursing units at the hospital, you engage with the electronic health records of hundreds of patients each day as a way of accessing, recording, and developing guidelines for safe and quality patient care. Today, you have accessed a patient portal to help you develop and design educational materials for your patient. As you review the pertinent information in the patient portal, you begin considering how you will communicate these educational materials to your patient. After gathering your thoughts and obtaining the necessary history and medical needs of your patient, you are ready to create and deliver this educational plan to your patient.

This assessment has two-parts.  Click each of the items below to complete this assessment.

 

Part I: Educational Plan

Design and create an educational plan (brochure or pamphlet) that addresses the following:

1. Include relevant health information necessary to educate your patient on their healthcare needs.
   1. Describe and define the health diagnoses, treatments, and at least one medication listed in the patient portal
   2. Explain relevant statistics and individual health vital signs related to the patient’s medical history
   3. Explain additional sources of support for patients with similar health needs/diagnoses
      1. Consider the context of your individual patient, and be sure to include any recommendations on how to access sources of support based on racial/ethnic background or culture, socioeconomic status, educational background, health literacy and linguistic proficiency
2. Be sure to adhere to health literacy guidelines for your individual patient.
3. Make sure to address each of the six areas from the “Checklist for Easy-to Understand Print Materials”
4. Your educational plan should be visually appealing and relevant to your individual patient.
5. Be sure to support the development of your educational plan by consulting and referencing 3- 5 scholarly references.
6. Length of the educational plan: 1- to 2-pages; include a separate page for you reference list.

 

Part II: Delivering Your Educational Plan

When delivering your educational plan to your patient, include the following:

1. Record a 3- to 5-minute video presentation detailing how you would communicate this educational plan to your patient.
2. Be sure to incorporate patient-centered care strategies in the design and delivery of your educational plan.
   1. Be sure to not just read the educational plan you created, but rather, consider how you would engage with your patient based on health literacy, culture, and background when educating your patient based on their health needs
3. Include a plan for care coordination.
   1. Explain your plan for care coordination based on the educational plan you developed.
4. Be sure to speak clearly and confidently when presenting your educational plan to your patient.
