The case study I chose this week was the young patient, Katie, who presents with symptoms of attention deficit hyperactivity disorder (ADHD), predominantly inattentive presentation. Her teacher describes her symptoms as inattentive, easily distracted, forgets things she already learned, and is poor in spelling, reading, and arithmetic. ADHD is the most common neurodevelopmental disorder diagnosed in childhood and is characterized by chronic symptoms of inattention, impulsivity, and/or hyperactivity leading to functional impairment in multiple settings (Danielson, Bitsko, Ghandour, Holbrook, Kogan & Blumberg, 2018).

The first decision I made in Katie’s plan of care was prescribing Ritalin chewable tablets 10mg orally in the morning. Katie and her parents returned to the clinic four weeks later stating that the medication did improve her symptoms in the morning and overall academic performance, but by the afternoon Katie became easily distracted again. She also was tachycardic from the medication. My next decision was to discontinue the Ritalin and prescribe Adderall XR 15mg orally daily. The extended release helped maintain her attention throughout the day, but Katie still was tachycardic. At this point, my next decision was simply to reduce the dose to 10mg. Unfortunately, tachycardia is a main side effect from Adderall, but Katie’s could have been worse because she was started at a higher dose. It is important to initially start at 10mg and increase to 5-10mg once the patient is tolerating the medication well. 

The administration of the associated pharmacotherapeutics I recommended may impact the patient’s pathophysiology in different ways. For example, the drugs Ritalin and Adderall are both stimulants causing a possible increased heart rate, which is the main side effect this patient presented with. This is why it is important to start the patient on a low dose and gradually increase the dose as needed. In addition, these drugs may also impact the patient’s sleep and cause eating problems, which has commonly been reported but can be managed by making adjustments to the medication regimen or daily routines (Hennissen, Bakker, Banaschewski, Carucci, Coghill, Danckaerts, Buitelaar, 2017). However, these types of drugs are highly effective in the treatment of ADHD by improving attention and focus causing the impulsiveness and hyperactivity to decline because the child is now able to concentrate on the specific task (Rosenthal & Burchum, 2021).

1. Summarize the concepts of floating that were addressed in the attached articles.

  • This week’s provided articles discussed the entirety of nurses floating and the concerns, benefits, cost, stress, emotions regarding floating. To summarize, many nurses may feel overwhelmed or frustrated when being told to float to a unit other than their home unit. Most importantly, the plan of action regarding a floating nurse must be safe for all parties involved: the patient, all staff, the floating nurse. One article mentioned a nurse’s current procedure where floating nurses are only assigned “stable, easy patients” with full access to the charge nurse if needed (Magrath, 2016). This is a great tool that most units should follow to promote overall safety and ease of transitioning for all staff and the floating nurse.

2. What steps would you take in this scenario to ensure a safe transition into the unfamiliar unit?

  • Safe transitions for floating nurses involve open communication between charge nurse of the unit and the floating nurse as well as involvement of other nurses working alongside the floating nurse. What must occur is appropriate planning from the charge nurse and the floating nurse by discussing the types of patients the nurse is certified and qualified to take care of. In this example, if the nurse is not certified to administer chemotherapy drugs or a patient undergoing radiating, then this nurse cannot care for a patient who is undergoing chemotherapy or radiation treatment.
  • Other staff members will need to share the weight of the workload by caring for patients who the floater nurse is unable to care for. Should these discussions and planning take place and there ends up being an incident where the floating nurse has orders for a task that is inappropriate, the floating nurse must NOT complete the order and instead notify the charge nurse for reassignment.

3. Why are these steps important?

  • These steps are important for the safety and protection of both the nurse’s license and of the patient.
  • Secondly, the concept of planning is really important for clinical managers, charge nurses and staff nurses to master. This week’s discussion mentioned different types of planning involved in healthcare.

4. What are some of the legal ramifications that can occur if you accept an assigned that is out of your scope of practice?

  • Medical malpractice can result from a nurse operating outside his or her scope of practice (Legal Rights and Responsibilities: NCLEX-RN2020). Patients can be injured, disabled, or die from improper care from an uncertified nurse if they operate outside of their scope. Those involved (i.e. charge nurse) could also be subpoenaed to court and may face legal ramifications as well if they had any involvement or awareness of a situation resulting in patient injury as well.

I see a little confusion however when you say that having the associate degree and bachelor degrees are assets and needed, but then stated you support the bachelor degree as the minimum requirement for licensure. We can’t have both. If the BSN was to be required to sit for licensure, the associate degree would go away. OR maybe the associate degree would replace the LPN requirement. That remains to be seen. Don’t forget to use your textbbook in initial responses (Black, 2020)! Please attach the pdfs of your articles so we can read them.

Objective:  To help nursing students become accustomed to searching the literature for evidence that supports nursing practice, and to polish poster creation and presentation skills. 

Task:  You are an experienced nurse who just signed a contract for a staff position at Passaway General Hospital.  On your first month at work, you notice a few questionable nursing practices that you learned to do differently at your previous place of employment.  As a BSN-prepared nurse, your unit manager has asked you to review the best evidence on the topic and create an A/V poster presentation for your colleagues on the unit.   He has asked you record your presentation so both night and day shifts can view it at their convenience.

Issues:  The following is a list of your priority areas.  You must choose one to focus on right now.

1. Choose a topic area from the list above.

2.  Review nursing, medical, and evidence-based practice literature related to your topic, following guidelines from the rubric to ensure you obtain information that addresses each section of the rubric.

3.  Critically appraise the evidence you have located using the CASP Tool (refer to textbook) to determine the strength of the studies you base your recommendations upon.

4.  Synthesize information using the grading rubric as a guide.

4.  Create a Professional Poster:  Create a professional scientific poster in MS Power point, to be saved as a computer file (you do not need to have it printed) using a scientific poster template.  Poster templates can be downloaded from many sources.  Here are a few suggestions.

Please Note:  A good presentation should address each of the areas of the rubric below.  However, your poster should not contain, word for word, everything in the rubric.  There should not be full paragraphs on your poster. Please use bullet points. The purpose of the poster is to highlight and summarize the information.

Synthesize   the findings of all three (or more) articles into a summary of research on   your topic.
