Background

The public health system in the United States has long been underfunded.  Analyses by the Institute of Medicine (IOM), the New York Academy of Medicine (NYAM), and the Centers for Disease Control and Prevention (CDC) have found that federal, state, and local health departments often are hampered by limited funds and consequently unable to carry out core functions.1

Budget shortfalls pose difficult decisions for local health departments about which programs will be discontinued.  These decisions are often “tragic choices” in which programs that are valuable for the community’s health must be sacrificed in order for other programs and services to survive.

In response to chronic underfunding and pressing health needs, public health agencies increasingly are looking to the private sector as a funding source, and  in some instances public health organizations have developed partnerships with the private sector as a way to achieve important health goals.  These public-private partnerships (PPP) have been promoted by the World Health Organization (WHO) and have played an instrumental role in addressing global health issues, such as access to drugs and vaccines in poor countries.2,3 

Case Description

You are the director of a local public health department facing a significant decrease in state funds for the coming financial year.  The budget cuts threaten a major health promotion initiative developed in response to a recent study showing that rates of obesity are particularly high in your area.  The planned initiative targets childhood obesity, and has received significant input and support from the local community.  After budget cuts are implemented the cost of the initiative will exceed the department’s available funds for health promotion activities. 

A national company that makes products for the diet industry, including diet shakes and other meal supplements, has offered money to your department for health promotion activities in your community.  Many of this company’s products promote extreme diets and dieting techniques.  The funds offered will enable the department to implement its planned initiative targeting childhood obesity.

In exchange for the funds the company wants their logo to be used on all educational materials distributed to the community.

Step 2: In a 500+ word paper discuss the following:

1. Analyze the impending budget cuts and the impact to the community.

DQ#2 Discuss the concepts of meditation and how they may assist in helping your patient deal with stress/anxiety.

Meditation is part of the Ayurvedic Medicine in CAM. Meditation consists in the focused concentration of the mind in a specific space where thoughts have no place. Meditation is practiced with the end of relieving mental pressure and stress. This Ayurvedic therapy can be practiced in any place at any time, preferably using a quiet environment to focus the five senses of our body in the current activity and to avoid distraction or stimulation of our senses. Fontaine, K. (2019).

I believe Meditation can help our patients in a positive way. When a person suffers a state of illness the first reaction of our mind, in most of the cases, is to worry and think about any complication that illness can bring. During Hospitalization the patient goes through a process that first of all, requires a different environment, the fact that physicians and nurses go in and out the room with different explanations, treatment and interventions can be also very stressful and sometimes the process is harder because an excess of stress and tension is experienced during this time.Jansen, E. (2017).

 Therefore, meditation can be very helpful in a state of illness, through the practice of this therapy the person can be able to relief stress and be able to cope easily with the diagnosis, prognosis and treatment. I believe that nurses could make a big impact on patients by teaching them how to mediate in such a stressful time, this practice could bring many benefits to the patient’s psychological state.Jansen, E.   With an aging population and healthcare reform, palliative care is an essential component of the work of nurse practitioners. Inpatient and community-based palliative care programs are helping health systems, ACOs, physician practices, and skilled nursing facilities provide care that enhances patient outcomes, decreases hospitalizations, and improves patient satisfaction (Johnson & Gray, 2013).

           Palliative and end-of-life care programs help improve care quality throughout the course of a patient’s illness. Patients that utilize palliative and end-of-life care tend to be more satisfied with their overall care and communication with providers, and they are less likely to end up in intensive care units and emergency departments (Johnson & Gray, 2013).

           Collaboration is taking action to reach a common goal (Fawcett, 2010). It commits two or more persons or organizations to achieve a common goal through engagement, partnerships, and advocacy. For example, at the University of Miami Health, the nurses in my unit collaborated with the Florida Department of Health and other local agencies to plan a program for children experiencing developmental problems. Processes such as problem setting, direction setting, and structuring were addressed as some of the communication strategies used to accomplish the project’s goals.
