Assignment Prompt

Applying the principles from the Nursing Leadership and Health Policy learning modules students will explore the role of the Registered Nurse Leader in policy advocacy.  Groups of two to three students will create a policy challenge presentation with a focus on collaboration. All members must participate to earn points. Each group member will receive an individual grade for the assignment and will participate in a rated peer evaluation of each group member. The final grade for each student will be based upon the presentation and peer evaluations and determined by the course professor.  The course professor will randomly assign students to a group and post the assigned groups and student names in a course announcement in week one. Contact peers in your assigned group by university email in week one to begin planning and collaborating on the project between weeks one and seven.

Directions:

1. Interview a nurse leader from a professional nursing organization to collect ideas for the project. 
2. Utilize your readings, research and resources to build the content. This should be a collaborative team process. Divide the workload for the project in an equitable manner among team members. 
3. Each group will design a voice- narrated Powerpoint or Prezi  to create a presentation and provide an overview of a grassroots policy challenge (present or past) that the identified organization supports. The policy challenge can  be a local, state or federal policy issue. Create a title slide or page with the full names of each group member. Include a final slide or page with key references cited in APA 6th ed. format.
4. Each presentation should be between 5-10 minutes in length. Rehearse and time the presentation in advance to ensure that the 10-min. time limit is not exceeded. 
5. One person from each assigned group will post the group signature assignment for his/her group as an attachment in the week seven discussion forum on or before Day 3, Week 7. 
6. Each team member will submit his or her group powerpoint presentation individually in the D2L assignment upload box for grading. 

Each presentation will need to include (but is not limited to):

• Brief description of the policy challenge the team has chosen and the nursing association that was identified.
• Describe the “Agenda” for the identified policy challenge. Include how the policy issue was identified, (Did the organization complete a review of the literature, an environmental scan and/or a SWOT analysis, etc.?)
• Brief description of key stakeholders (those who will be affected and those who can effect change), ie, interest groups, opposition groups/individuals or coalitions.
• List the different strategies that the organization plans to or has already used to inform/lobby legislators, ie, e-mail campaign, in-person visit, testimony, protest, etc.
• List and describe any non-legislative strategies that were used or plan to be used to support the initiative.
• Describe where this policy challenge is in the legislative process and its implications for the future.

An Advanced Directive is a document in which a client who is competent is able to express wishes regarding future acceptable health care. The document may include decisions including the desire for extraordinary lifesaving measures, such as resuscitation, intubation, and artificial hydration, and nutrition. The advanced directive may also be used to designate another person to make decisions when the client becomes physically or mentally unable to do so. The Patient-Determination Act requires all clients admitted to a health care facility be asked if they have advanced directives.

Planning guides for seriously ill clients includes:

1. Living will: Legal document that instructs health care providers and family members about what, if any, life-sustaining treatment an individual wants if at some time the individual is unable to make decisions.
2. Durable power of attorney for health care: Legal document that designates another person to make health care decisions for the client when the client becomes unable to make decisions independently.

Nursing intervention in these types of situations is to ensure to communicate to the interdisciplinary health care team that the client has provided and advanced directive and that it has been placed in the client’s medical record. Clients who do not have advanced directives must be given written information outlining rights related to health care decisions.

Initial Discussion scenario:

Mr. Gomez is a 73-year-old man who is estranged from his wife. Since he prefers that she doesn’t make any medical decisions for him should the need arise, he has filled out an advance directive naming his daughter Elena as his health care proxy. He has suffered from diabetes for many years and is adamant that he does not want dialysis, even if it would sustain his life. He does, however, wish to have tube feeding for however long it will sustain him.

One evening while at home with his daughter, Mr. Suarez has a seizure. He is taken to the hospital immediately, but never regains consciousness and doctors do not expect him to do so. Elena contacts her mother Mr. Gomez’s estranged wife and she quickly joins her daughter at the hospital. They want to put him on dialysis (as his diabetes is worsening) and a feeding tube (as there is no other way for him to receive nutrients). His daughter informs the doctors of her father’s end-of-life wishes. The doctors warn her that without dialysis her father will pass away more quickly, even with the feeding tube. Mr. Gomez’s estranged wife informs Elena that she needs to follow the advice received from the doctors in order to save her father’s life. Elena says that although she is aware of the consequences, she wants them to carry out her father’s wishes.

The clients estranged wife immediately has her attorney file an order to stop Elena from following her father’s wishes that are in his advanced directive. She feels that even though they are estranged, she still should have the right to make decisions regarding Mr. Gomez’s care. This action delays Elena’s ability to honor her father’s wishes and the doctors feel that it is necessary to inform the hospital’s ethics committee about the ongoing issue.

Assignment: Discuss the following questions

1. What is the role of the doctors in deciding whether or not to respect Mr. Gomez’s advanced directive?
2. Does Mr. Gomez’s estranged wife have the authority to step in and ignore Mr. Gomez’s advanced directive. Explain your reasoning while providing evidence-based solutions in your conclusion.
3. Gomez’s estranged wife has filed temporary order to ignore the advanced directive. If you were Elena, how would you handle this dilemma? What rights does Elena have that allow her to honor her father’s wishes?
4. What is the role of the ethics committee and how can they best support the client and his family?

Diabetes and Drug Treatments

Each year, 1.5 million Americans are diagnosed with diabetes (American Diabetes Association, 2019). If left untreated, diabetic patients are at risk for several alterations, including heart disease, stroke, kidney failure, neuropathy, and blindness. There are various methods for treating diabetes, many of which include some form of drug therapy. The type of diabetes as well as the patient’s behavior factors will impact treatment recommendations.

For this Discussion, you compare types of diabetes, including drug treatments for type 1, type 2, gestational, and juvenile diabetes.

Assignment Details

Collaboration in Pediatric Primary Care

This formal paper should include APA format, literature review, and logical sequence. This assignment should demonstrate your understanding of the topic. Citations and reference list is required. Please refer to the Grading Rubric for point distribution. This paper should be 3–5 pages excluding the title and reference pages.

This assignment is specific to a particular pediatric primary care practice and is mostly based upon student observation and experience. In your paper, include the following elements:

• From your observations and experiences in your pediatric clinical rotation, provide an example of multidisciplinary collaboration.
• From your observations and experiences in your pediatric clinical rotation, provide an example of how multidisciplinary and family collaboration affect patient outcomes.
• Discuss how you have been able to promote communication and collaboration among healthcare professionals, patients, and family/caregivers.
• Discuss a particular case where collaboration among healthcare professionals and family members affected patient/family satisfaction.
