Part I: Interview

Select a patient to interview, be sure to focus on the interviewee’s experience as a patient.

Create a spiritual assessment for evaluating the spiritual needs of the patient. Create your own tool for assessing the spiritual needs of patient.

Your spiritual needs assessment survey must include a minimum of FIVE questions that can be answered during the interview. During the interview, document the interviewee’s responses.

The transcript should include the questions asked and the answers provided. Be sure to record the responses during the interview by taking detailed notes. Omit specific names and other personal information through which the interviewee can be determined.

Part II: Analysis

Write a 750 word analysis of your interview experience. Be sure to exclude specific names and other personal information from the interview. Instead, provide demographics such as sex, age, ethnicity, and religion. Include the following in your response:

1. What went well?
2. Were there any barriers or challenges that inhibited your ability to complete the assessment tool?      How would you address these in the future or change your assessment to better address these challenges?
3. How can this tool assist you in providing appropriate interventions to meet the needs of your patient?
4. Did you discover that illness and stress amplified the spiritual concern and needs of your interviewee? Explain your answer with examples.

Submit both the transcript of the interview and the analysis of your results. This should be submitted as one document. The interview transcript does not figure into the word count.

At a local hospital, a decision was made recently to downsize the nursing staff. The local television station sent a reporter and camera crew to interview the administrator regarding the impact of this action on patient care. After spending 20 minutes filming the interview, the reporter left. That evening a 15-second segment of the interview was shown that left an unfavorable impression regarding the impact on quality. The administrator wondered what went wrong. Explain how more control could have been used to send out the message about the downsizing.

Polypharmacy is a common issue that NPs and any prescriber should keep in mind. That is because a common risk factor for polypharmacy is elderly patients. After living for many years, it is common to have multiple comorbidities. Multiple comorbidities represent different treatment which most of the time includes 6 to 7 pills taken daily. Another risk factor for this includes obesity. Obesity rates continue to rise in the US and with that many other diseases. Obese patients usually suffer from cardiovascular, respiratory, gastrointestinal, and neurological problems at the same time (Ersoy & Engin, 2018). All of these combined also lead to them needing multiple drugs to be taken as treatment daily.

            As an NP, it is our duty to make sure we look out for the safety of the public specially when it comes to polypharmacy. One intervention we can apply in our plan of care for patients at risk of polypharmacy is providing education in regard to the side effects and information regarding the drug. Another intervention we can apply is finding medications that can be taken once a day and recommending using a pill dispenser (Dagli & Sharma, 2014). Another intervention that could be applied is eliminating one of the multiple drugs if possible or if the medication is unnecessary. Polypharmacy is an issue for many of our patients that’s why it is our job to manage it and prevent adverse effects from it.
