This is a Collaborative Learning Community (CLC) assignment. 

The purpose of this assignment is to assess leadership styles, traits, and practices as a nursing professional, establish the importance of effective interprofessional communication as a leader in nursing, and to explore the role of servant leadership in nursing practice.

Read the study materials on leadership and complete the topic quiz activities to better understand your leadership qualities.

Upon completion, summarize and share with your group what you learned about your specific leadership qualities, so you can become familiar with how you are similar and different from your peers when it comes to being a leader.

As a group, review the study materials related to servant leadership. Using what you have learned about the tenets of servant leadership and traits and practices of successful leaders, create a 10-12 slide PowerPoint presentation with speaker notes. Add an additional slide for references at the end of your presentation.  

Include the following in your presentation:

1. Each group member: Create a slide that summarizes your leadership style, traits, and practices.
2. Compare the personal leadership styles of your group members, including commonalities between group members’ strengths and weaknesses.
3. Explain why it is important for nursing professionals to be aware of their personal leadership style, traits, and practices.
4. Discuss what leadership traits and styles are necessary to be an effective communicator. Explain the importance of leaders adapting communication approaches when working interprofessionally (across ancillary departments, vendors, community members).
5. Discuss how nursing professionals can benefit from integrating the tenets of servant leadership to empower and influence others as they lead.
6. Discuss how leaders who practice servant leadership and have a strong understanding of their personal leadership traits can successfully lead others and navigate the unique challenges that are part of nursing and health care. Provide two examples that illustrate your main ideas.

You are required to cite to a minimum of three sources to complete this assignment. Sources must be published within the last 5 years and appropriate for the assignment criteria and relevant to nursing practice.

While APA style is not required for the body of this assignment, solid academic writing is expected, and documentation of sources should be presented using APA formatting guidelines, which can be found in the APA Style Guide, located in the Student Success Center. 

Blueprints are used by others besides architects. New home buyers may use the blueprint of their future home to better understand the architect’s vision and to consult with the architect on changes. Municipalities will use the blueprint to issue building permits. Builders will use it to plan construction and estimate costs.

Likewise, you don’t have to be a database designer to apply visual tools such as database diagrams. For example, creating or studying a database diagram can help you, as a leader in a healthcare organization, understand the data needs of the organization or of your team.

This is in turn can create efficiencies and drive improvements at varied levels of patient care. As a nurse, database diagrams can help you analyze the data structure and how to make best use of the available data and functionality in the provision of care. At every level of an organization, an understanding of database diagrams can help in providing feedback to database managers that in turn can help drive improvements.

In your Module 2 Assignment, you create a database diagram for the database that you proposed in the Week 3 Discussion and built upon in the Week 4 Discussion. You also create a PICOT question to be used to search your proposed database. Finally, you develop an analysis of your database design, explaining the proposed structure and design elements while justifying your approach.

To prepare:

• Review the Learning Resources.
• Review your posts from the Weeks 3 and 4 Discussions and the scenario on which they are based.
• Consider a clinical research question you might ask that could be informed by the data in your database.
• Restate your clinical question as a PICOT question.

Assignment (2- to 3-page paper and a diagram of a database as an appendix):

Using the scenario from Week 3, create a diagram of your proposed database using Microsoft Word.

• Include the additions made in Week 4.
• Complete the diagram first, but place it as the final page or pages of your submission. The diagram is separate from the required page count.

In the narrative portion of the assignment:

• Explain how your diagram articulates your planned design.
• Explain the principles behind selecting key fields and defining relationships. Be specific and support your response with evidence.
• Write a sample PICOT question (i.e., a query) you might ask based on the information in the database created during Weeks 3 and 4 to demonstrate your understanding of the connection between data and research.
• List the tables in the database that you would need to include when answering your question.

 This assignment will incorporate a common practical tool in helping clinicians begin to ethically analyze a case. Organizing the data in this way will help you apply the four principles and four boxes approach.

Based on the “Case Study: Healing and Autonomy” and other required topic study materials, you will complete the “Applying the Four Principles: Case Study” document that includes the following:

Part 1: Chart

This chart will formalize the four principles and four boxes approach and the four-boxes approach by organizing the data from the case study according to the relevant principles of biomedical ethics: autonomy, beneficence, non maleficence, and justice.

Part 2: Evaluation

This part includes questions, to be answered in a total of 500 words, that describe how principalism would be applied according to the Christian worldview.

APA style is not required, but solid academic writing is expected. 

At the completion of your intervention, you will produce a 5–8 minute video discussion script of your care coordination intervention for a peer-reviewed media platform. 
As an advanced practice health care provider, it’s likely that you will be asked to give a presentation at some point in your career. Now that you have completed this project, it’s time to disseminate the outcomes in the form of a recorded professional presentation. A professional presentation style requires the confidence and ability to convey the right image that you want your audience to see. Unfortunately, it’s easy to miss some of the opportunities available to help you present yourself in the best possible light and, as a result, enjoy the success you deserve. Remember to be forthright about what happened during the project and about how data were collected and analyzed, and present the findings so stakeholders and external parties clearly understand the steps in the process. By completing this final activity, you can help others recreate this type of project and enhance outcomes in other patient care settings.

You have completed the intervention and achieved enhanced outcomes. Now you need to disseminate your findings and provide recommendations to sustain the results of your intervention. You have been asked by nursing leadership to produce a scholarly video for a peer-reviewed media platform. The video will include 5–8 minutes of scholarly discussion covering the topics listed below.
Grading Criteria
The numbered assignment instructions outlined below correspond to the grading criteria in the Disseminating the Evidence Scholarly Video Media Submission Scoring Guide, so be sure to address each point. You may also want to review the performance level descriptions for each criterion to see how your work will be assessed.

1. Analyze care coordination efforts related to clinical priorities based on a PICOT question. 
   • Provide the PICOT question and a brief introduction of the issues.
2. Describe the change in practice related to services and resources that will be available for the interprofessional care coordination team. 
   • Describe the key care coordination efforts.
3. Explain efforts to build stakeholder engagement within the interprofessional team. 
   • How did you lead the change in practice?
   • Summarize how you encouraged and built stakeholder engagement.
4. Recommend next steps that that support thoughtful resource utilization and a safe environment in care coordination. 
   • Propose how you will sustain the current outcomes with the coordinated care interprofessional team.
   • Make recommendations on moving forward that support thoughtful resource utilization and a safe environment.
5. Produce a video that provides insight, understanding, and reflective thought about care coordination.
6. Communicate audibly and professionally, using proper grammar and including a reference list formatted in current APA style.
7. Important Note: but do address each scoring guide criterion in the video, including a discussion of authors in the literature who support the ideas presented. Please submit a separate APA-formatted reference list for the resources discussed in your reflection.
