Personal Health Records (PHRs) are designed to give consumers access and control of their health information.  PHRs are tools that give consumers a view of their health care data, either from information they have entered, information linked to their health care provider, or information generated from insurance claim data.

As Personal Health Records (PHRs) proliferate, the consumer is faced with many choices.  PHRs offered through insurance plans and employers limit the consumer choices but they do provide links to clinical data.  Stand-alone web-based PHRs offer more features but with little interface to the patient’s current clinical information.

While there are numerous Personal Health Record products available, the industry is still at an introduction to growth stage.  There are no established standards such as those available with electronic medical records and no agency that certifies the offerings.  The consumer must rely on their own criteria in choosing a product.

Some of the criteria that have proven to be critical for electronic medical records and for health web site services can be combined to apply them to PHRs.  They include: privacy, security, usability, portability, completeness, vendor reliability, and access/availability.

  1. Availability/Access:  This refers to the consumer’s ability to access and manage their personal record via the Web.  Access also includes minimal accessibility standards defined by the W3C and by federal laws (508 compliance).
  2. Completeness: This refers to the information fields available in the records.  The basic PHR should capture, demographics, health behaviors, social history, family health history, allergies, medications, lab results, past medical care, current existing conditions, physician information, insurance information, etc.
  3. Privacy: Web site has published privacy policy that protects individually identifiable information (no disclosure without consent).  Privacy refers to policies that protect the unauthorized secondary usage of information.  The PHR should indicate if it meets HIPAA security requirements.
  4. Security: This refers to the technical protections available on the system including passwords, authentication mechanisms, secure connections, etc.  In an ideal situation, the site should have a consumer-accessible audit log to see who has viewed the information.  The site should also provide details of if/when the consumer is notified about breaches of security.  Are you aware of the site security through policies and symbols?
  5. Usability: This refers to the features available on the site. The PHR should provide the ability for a non-technical or not IT-related individual to easily join, navigate, and enter information. The level of jargon on the site as well as navigation links can influence the perceived usage.  A novice user should be able to successfully complete registration, information entry, printing, saving, and information retrieval. Is there help or support available? Is the information presented using “common language” or is it full of jargon (Basically, can you grandmother understand it? Can someone with an 8th grade reading level understand it?)
  6. Portability: This refers to the consumer’s ability to move records out of the current provider to others.  This feature can be both via paper or electronic. Is the information portable – do you have to print it?  Can you electronically download any of the information?  Can you email the information?
  7. Vendor reliability: This refers to the reliability and history of the vendor.  A new service may not have the ability to continue business operations and has a high chance of closure.  Vendors linked to or partnering with sponsoring national organization provide more stability.

Nephrolithiasis is the growth of a kidney stone in the kidney or the lower urinary tract. There are four different stones that can form during this disease. The first is calcium salts (75%-85%). These types of stones are predominantly in patients with a high salt  diet, animal fat, animal protein, and oxalate from green leafy vegetables. A low calcium diet is a risk factor also which leads to oxaluria. The second stone is struvite (10%-20%) and predominantly found in women. They occur when the urine is alkaline (pH greater than 7.0) and a urea-splitting organism such as Proteusor Klebsiellais present (Dunphy, Wineland-Brown, Porter, & Thomas, 2015). Uric acid (7%) are formed when there is increase in uric acid such as the disease gout. It may also result from acidic urinary pH, significant bicarbonate loss, regional enteritis, hereditary factors, or ulcerative colitis. Lastly, cystine (1%-2%) which are created from autosomal recessive disorder called cystinuria. These may form in the kidney, bladder, and ureters. It rarely causes kidney failure. Diet considerations from certain cultures eating lifestyle may increase or decrease the risk for Nephrolithiasis. Vegetarianism has been shown to protect against calcium stone formation in some studies; However, excessive oxalate intake from green, leafy vegetables can cause stones in patients with hyperoxaluria (Wells, 2000). Excessive protein probably contributes to stone formation in 2 ways: by excess purines possibly leading to uric acid stones and by the observed hypercalciuric effect that develops from protein loads; a global reduction of protein, particularly animal protein, is typically recommended, with the daily protein intake suggested at 60 g/day (Wells, 2000). Other considerations such as lack of fluid and obesity can also be factors.

  The Clean Air Act is a federal law that was created over 40 years ago in the United States. It helps to control air pollution nationally and protect citizens from air pollution that may be hazardous to their health. Globally it is one of the most comprehensive laws focusing on air quality. Currently congress is working to block the EPA’s efforts from protecting the health of the public and delaying their work in blocking life-threatening air pollution. I was able to submit a letter to senators via the APHA website in opposition of congress hindering the EPA’s abilities to enforce the Clean Air Act.The stakeholders in this issue are literally anyone in the United States who is breathing. While some individuals, such as those in higher pollution areas, are going to be more affected than those in rural areas, everyone is affected by the issue. By creating this page for citizens to speak up, a platform is being opened as well for those who may want to take further action than submitting a letter. Coalitions could be form in opposition of the actions congress is taking that are risking the health and lives of millions of individuals.

Discussion Two,

I selected the American Public Health Association. The APHA is an organization that is dedicated to improve the health and wellbeing of the public. They publish a journal and a newspaper to share the newest information in the public health world. Oftentimes this news includes current campaigns and legislative actions to urge readers to write and ask policymakers to vote and act of certain policies. Current polices that the APHA is hoping members voice their opinions are the opposition of Trump’s administration proposal on the current public health crisis, the Protect the Clean Air Act, support of paid sick leave, and many more. When a member clicks on one of the policies to voice their opinion, they are able to read more information on the topic, read a standard message to congresspersons, and then compose their own message to congress.

One method that the APHA utilizes to communicate the best possible message to congress members is by allowing members to send a tailored message (with the option to personalize the message) that states specifically what the APHA is backing on the policies. For instance the message for the Protect the Clean Air Act has a clear message that states that the APHA wants congress members to reject any measures that would not allow the EPA to protect the public from air pollution, then after the unchangeable message members are able to either personalize or send a standard message that supports or gives more research/opinion behind why congress should not hinder the EPA protecting the public from air pollution.

When you decide to purchase a new car, you first decide what is important to you. If mileage and dependability are the important factors, you will search for data focused more on these factors and less on color options and sound systems.

The same holds true when searching for research evidence to guide your clinical inquiry and professional decisions. Developing a formula for an answerable, researchable question that addresses your need will make the search process much more effective. One such formula is the PICO(T) format.

In this Discussion, you will transform a clinical inquiry into a searchable question in PICO(T) format, so you can search the electronic databases more effectively and efficiently. You will share this PICO(T) question and examine strategies you might use to increase the rigor and effectiveness of a database search on your PICO(T) question.

To Prepare:

  • Review the materials offering guidance on using databases, performing keyword searches, and developing PICO(T) questions provided in the Resources.
  • Review the Resources for guidance and develop a PICO(T) question of interest to you for further study.
By Day 3 of Week 4 USE THE TOPIC MEDICATION ERRORS

Post your PICO(T) question, the search terms used, and the names of at least two databases used for your PICO(T) question. Then, describe your search results in terms of the number of articles returned on original research and how this changed as you added search terms using your Boolean operators. Finally, explain strategies you might make to increase the rigor and effectiveness of a database search on your PICO(T) question. Be specific and provide examples.

A Clinical Question

            The best way to form clinical question is in the form of PICO(T)  (Ask: Write a focused clinical question, 2020). I have done such a thing and have found that the question formulation and changes to it were not as challenging as finding research related to the question.

The Question:

            (P) In the hospital or clinical acute care setting, (I) is a 2 person double-check of insulin, (C) compared to a single person double-check of 5 rights, (O) more effective in the improvement of (01) insulin medication errors, (02) nurse satisfaction during a (T) 30-day term of observation?

Databases Used

            The Walden University database was used to search databases for nursing. Of those available Science Direct, CINAHL, and Cochrane, and PubMed were used. Background questions were first asked to get a sense of what has been studied in this area. It was important to understand when and why this practice first started to determine the relevance of data acquired and to determine how recently we needed to search. Then, foreground questions began to form until finally the PICO(T) question.

            During the background question period, the terms ‘double-check’ became the preferred terminology for the practice I wished to study. So using this ‘double-check’ in the PubMed database, 244 articles were found but not all pertained to my topic. Changing the text to be included only in the title or abstract, the same result achieved. Adding ‘medication errors’ reduced the yield to 21 results. At this level, the evidence is available on the basic topic of using the double-check to prevent all medication errors and relates often to high-risk medications. However, I want to know about the labor-intensive practice of using a 2 nurse double-check for subcutaneous injection of insulin. Adding a level of inquiry which includes insulin, this yielded 2 studies. One from 1983, and one from 2009. Neither of which addresses the current question other than provides background for why the practice may exist in the first place. Changing terms to highlight ‘insulin double-check” yielded two results, both relevant, and one is the most current and referred to peer-reviewed study on this topic.

Affecting Rigor and Effectiveness

            To improve the effectiveness and rigor of a database search one much identify proper and strong keywords in the question or relating to the question. Use filters to find peer-reviewed data first. Use filters for the type of study if possible, including systematic studies, and work your way down the pyramid for strength of research available. If the question is somewhat original, there may not be much evidence. This is the case here. Other filters and Boolean operator’s include dates of publication, with the last 5 years preferred as ‘current’, using ‘and’ and ‘or’ and ‘not’ to include or exclude words in the title, abstract, text, or any part of the article or work.
