Extrinsic motivation is performing or engaging in an activity to earn rewards or avoid unishment. Intrinsic motivation is engaging in a behavior because it’s personally rewarding. Examples would be participating in a sport because it’s enjoyable, solving a puzzle because it’s fun or playing a game because it’s exciting. Essentially, extrinsic motivation arises from outside an individual, while intrinsic motivation arises from within. An example would be a pizza party if employees complete a survey with group participation being greater than 90%.

Characteristics of a performance driven team are:

Intrinsic motivation is defined as performing an activity for personal satisfaction rather for some consequence, a person who is intrinsically motivated acts For the Fun of a challenge rather than external pressures or rewards (Ryan, 2000). Extrinsic motivation is when an activity is performed to gain a separable outcome, an extrinsically motivated person acts in hopes of gaining an outside reward (Ryan, 2000). Motivation for the extrinsic person would be such as pay raises, bonuses, and benefits. Intrinsically motivated people perform an activity because they simply like the activity or the challenge, an extrinsically motivated person performs an activity for its instrumental value whether it be receiving a reward or avoiding punishment.

Characteristics of a performance-driven team is a team who works together and because of that a greater outcome is achieved. Eggenberger, Sherman, Keller (2014) stated that in any setting, “high-performance work teams rarely occur naturally. They must be created and managed. To instill effective teamwork into health care, leaders need to recognize and emphasize its importance”. Having a strong team with strong communication skills is a key characteristic to have.  Cultural competency is a process that is learned and requires self-awareness, reflective practices, and knowledge of cultural issues (Ball et al., 2019).  Healthcare providers need to learn to be sensitive to a patient’s heritage, sexual preferences, socioeconomic situation, and ethnicity (Ball et al., 2019).  The purpose of this discussion is to explain the specific socioeconomic, spiritual, lifestyle, and cultural factors of a 32-year-old lesbian pregnant female, and to describe the issues that I would need to be sensitive too when interacting with this patient.

Lifestyle and Cultural Factors

 The specific socioeconomic factors for the 32-year-old lesbian pregnant female is unknown; however, there is a chance that the patient is at least in the middle class standing as she was able to provide a sperm donor for her pregnancy.  On the other hand, female couples are also usually on their own financially and medically regarding labs, ultrasounds, sperm, and professional expertise (Singer, 2012).  The spiritual factor is also hard to determine with this patient, but asking questions such as “Is there religion or faith which you partake in?” can be helpful in determining spiritual affiliation (Ball et al., 2019).  One lifestyle factor that is associated with this patient is that she is a pregnant lesbian.  I think that creating a comfortable environment and also learning new patterns of behavior and effectively applying them in the correct settings can help patients with trust (CDC, 2015).  According to Singer (2012), for lesbian women, trust is received when a healthcare professional has the philosophy to maintain inclusion and cultural sensitivity.

Sensitive Issues

 The two main sensitive issues to keep in mind with this 32-year-old lesbian pregnant women is her sexual orientation and pregnancy.  Both categories are considered to be sensitive because oftentimes, health care providers are usually considered heterosexisms, which means that they tend to see the world in the view of heterosexual terms and ignore homosexuality (Singer, 2012).  Therefore, as a medical provider, I need to remember to do the opposite of heterosexism and ask questions such as “Tell me the story of how you became pregnant, this question takes the providers assumptions out of the equation and allows for a deeper understanding” (Singer, 2012, p. 38).  The provider also needs to avoid hetero-assumptive dialogue and utilize open-ended questions, which allows the patient to open up about their sexual identity and family construct (Singer 2012).

           There are several ways that a person can choose to focus during incorporation of a person who has a different culture in your team. Some of this ways may base on good capture to the essence of that person which is one of integrity that many communities focus as there is a communication barrier. In the same manner, one may feature on core values of that person which are on its beliefs and baseline of behavior and which explicitly define clear how the person behaves after being incorporated into the team. Also focus on customer interaction feedback and response as this issues might embody some of the decline changes which may undermine the manageability of credibility (Niebuhr, 2016). Ideally, if culture and values may not align then this issue can create business performance decline and suffer. It would be of importance to consider the beliefs that our new member in the team follows to make him/her align with the performance of the team.

Your comment on the difficulties of communication among a diverse group is a very real issue. The differences in languages, accents and dialects can cause communication difficulties. Often a person from another culture may have communication apprehension. Communication apprehension, is defined as “an individual’s level of fear or anxiety associated with either real or anticipated communication with another person or persons” (McCroskey, 1977, p. 78). The leader’s responsibility in this situation would be to identify ways to help the person feel more comfortable and assisting assimilation into the group. According to Asherman & Bing (2010) ways to assist is to allow individuals who do not speak English as a first language more time to respond to the discussion. Due to communication apprehension, the leader can take written ideas after the meeting to avoid the need for the individual to talk to a group. Also teaching the team to speak distinctly and avoid the use of jargon is helpful. Paraphrasing and summarizing more in meetings will help to identify and clarify any miscommunications. Identify an experience where you had to assess the needs, interests, and goals of a patient with an ethnic background, different from yours, that resulted in a positive learning experience. How did you turn the situation into a positive learning experience? Did you personally have to deal with any stereotypical beliefs?

When I was working in SICU (Surgical Intensive Care Unit), I was assigned to a patient who was admitted to ICU after an episode of bradycardia and loss of consciousness on the medical surgical floor. A rapid response was initiated and she was intubated and brought to our unit. She was Haitian and came from a very different ethnic background. At the time I was not familiar with their culture or beliefs nor did I know her health history and what originally brought her to the hospital or the reason for her admission.

After a couple days of being assigned to her and taking care of her and her family, I began to understand a little more. She had been diagnosed with breast cancer the previous year and had undergone chemotherapy which unfortunately was unsuccessful. The cancer had spread throughout her body including her esophagus which was later found out to be the reason for the intubation. A tumor had attached itself to her airway and she was unable to breath on her own. 

I instantly became attached to the patient and family and enjoyed learning about their culture and belief system. Many people including doctors, nurses and other medical professionals couldn’t understand why a patient who was terminal would want everything done and continue aggressive treatment. Many gave up on her and the family which is why I believe they requested that I take care of her every shift I worked. Special arrangements were made between the family and my director to accommodate their wishes. 

Each shift I worked, I took care of her. Her daughter and her husband never left her side other than for bathing or other emergent matters that couldn’t be handled without them. Eventually the primary physicians would stop rounding on her daily and healthcare professional started discussing her prognosis, treatment and plan of care. No one understood why we were continuing to treat this individual as they called the treatment “futile”. 

As time went on, the hospital decided to call in Risk Management as they had a different belief and understanding regarding by patient’s wishes and medical management. This is when I realized I did, I understood her and her family and knew I needed to be her patient advocate and her voice. I fought for her treatment and plan of care which was aggressive not palliative. She expressed to me she wanted to live, even if it meant her last days were spent in the hospital. She would lay there and listen to her children laugh and tell stories about their day and her husband’s voice telling her how much he loved her. She was a mother of 10 and had the most beautiful family I had ever met. 

Before I met her, I may have had the same thoughts and comments as the other healthcare professionals. However, once I got to know her and her family as well as their cultural beliefs and background, I understood her. This was more than a positive learning experience it changed my life, my outlook and opened my eyes to cultural diversity and understanding.

 For the purpose of this discussion, the assessment tool that shall be  analyzed is the Geriatric Depression Scale. It is important to note that  depression in elderly patients is a common problem and this leads to  significant emotional suffering and loss of quality of life. Depression  in elderly patients also lead to increase in the risk of physical  inactivity as well as disability. As elderly persons grow older an get  to the age of 80 and above, depression becomes prevalent especially if  they live in nursing homes or if they suffer from dementia (Conradsson,  Rosendahl, Littbrand, Gustafson, Olofsson & Lovheim, 2013).  Unfortunately, regardless of the fast that depression is more common  than dementia in old age, it remains underdiagnosed and undertreated  (Allan, Valkanova & Ebmeier, 2014). It is because of this that there  is need to have a proper tool for diagnosing depression in geriatric  patients. The tool should be effective in regard to the assessment  process and help in the evaluation of treatment in order to monitor  progress. One such tool is the Geriatric Depression Scale.

   The Geriatric Depression Scale is a tool that is used to assess elderly  patients with a view of establishing whether they are depressed. The  tool was developed by Yesavage et al. in 1983. At the time it was  established, it had 30 items. However, 30 items were found to be too  many to be effective. The items made the tool time consuming for both  elderly patients as well as clinicians. This necessitated the  formulation of the GDS-15 which is said to be effective in diagnosing  depression in elderly patients (Durmaz, Soysal, Ellidokuz & Isik,  2018). The effectiveness of this tool has been researched widely and  scholars have established that it is not only effective in diagnosing  depression in elderly patients, but it also has a significant  correlation with DSM-5 criteria in patients that have depression  (Durmaz, Soysal, Ellidokuz & Isik, 2018).

   Psychometric properties mean that a tool is valid and reliable. It  means that it has to be accurate in assessing what it is meant to  assess, and it should also be consistent in providing results (Asunta,  Viholainen, Ahonen & Rintala, 2019). The psychometric properties of  the GDS-15 has been assessed in different studies. In fact, in one  study, these properties were assessed in regard to different elderly  populations including those that are cognitively intact, those that are  functionally impaired and primary care elderly patients that are  community dwelling. The study established that the tool’s internal  consistency reliability was moderate but acceptable. There was construct  validity in the tool’s ability to measure depressed mood, suicidal  ideation and life satisfaction. The ability of the tool to show the  difference between patients that were depressed and those that were not  depressed showed acceptable specificity and sensitivity. However, when  it came to suicide attempt status, the scale showed significant  weakness.  In conclusion, the scholars pointed out that in all the  geriatric populations that were included in the study, the scale showed  impressive psychometric properties (Friedman, Heisel & Delavan,  2005).

  This  scale basically has 15 questions. They are easy to understand and  answer for elderly patients. They all have yes or no answers. 10  questions have to be answered as Yes to attract a point and 5 questions  have to be answered as No to attract a point.  A score below 4 is  normal. A scale between 5-8 is mild depression. A score of 9-11 show  moderate depression and a scale between 12-15 indicates severe  depression. This is an assessment tool that should be applied in all  geriatric patients and especially those that are not cognitively  impaired. Since the scale can be adjusted depending on what the patient  feels, it means that it can be used in the assessment of the  effectiveness of a psychopharmacological therapy in patients.
