I just need to 2 responses to the Discussions, and about 100 words for each responses.

  

1. I chose the population of children under the age of 6 and I chose the Safety by Room checklist. I reviewed my family room, kitchen, and bedrooms; I was able to cross off each area on the check list. The modifications I would need to make would be in my bathrooms, backyard, and in my garage. In my bathrooms I don’ t have any anti-scalding devices on the faucets, locks on the toilet or cabinets, or non-skid rugs. In my garage I would need to install a carbon monoxide detector and in my backyard I would need to install a lock on my hot tub. Currently I do not have any children of my own so this isn’t a large concern of mine as there aren’t young children that live with me. I do have younger brothers that are under 10 years old so there are a few changes I would want to modify as they do come visit occasionally.

 

2. What is the role of Cal-Osha?  The role of Cal-OSHA is to set health and safety standards, enforce those standards, and provide education and outreach, certifications and licensing for all workers in the state of California.   As part of enforcing the standards, they are responsible to respond to complaints of unsafe practices and they cite and fine organizations based on the set standards.  They maintain and provide some general safety guidelines that apply to all places of employment in California along with some guidelines for specific types of industry such as the the agriculture or film industries.  Additionally, they also regulate the safety of those riding in public conveyances; such as elevators, trams, or amusement park rides.

    I selected a citation issued to Butler Amusements Inc. that operated rides for the Big Fresno Fair in Fresno, California in OCT 2013.  They were cited on 3 violations relating to the same thing, which was basically unsafe operation of 2 rids and “failure to maintain equipment according to manufacturers” guidelines.  These violations were considered”Serious Willful” violations because the owners knowingly removed or left off some parts that maintained stability of the rides. Their overall fine was $101,250.00 and it seems they fixed the issues. I am uncertain if that was a significant fine because the citation did not mention if there were any injuries or fatalities as a result of the violation. 

Throughout the year, and with California weather being so nice, one can encounter a number of these fairs popping up in most cities in California and all having some form of ride or another.  Many people flock to these fairs, which seem to be reminiscent of decades-past travelling circus, and reviewing this citation brings to mind something that I previously did not truly consider.  Although most people are aware that these events/fairs need to register and have permits for for holding their events, I for one don’t think that one would readily consider if the rides are being inspected or held to any major safety standard.  We all know that we want them to be fun and safe, and if something does go wrong with a ride, we would likely sue or complain to management. But to think that it would be a requirement to answer to the State’s Occupational Safety and Health Administration would not be something I would consider, until now.   Considering how many people these fairs attract, the money that is spent there (especially if you have children), and the potential for injury or harm, perhaps it would be good to ensure the public is aware and able to confirm/verify that at least the minimum safety standards are being followed and met, before we place our lives, or lives of our family members/friends in the hands of the fair operators.

Presenting Symptoms of Influenza

          According to the Centers for Disease Control and Prevention (CDC), Influenza is a contagious respiratory illness caused by influenza viruses. It can cause mild to severe illness and even cause death (Influenza, 2018).

  • Fever or feeling feverish/chills
  • Cough
  • Sore throat
  • Runny or stuffy nose
  • Muscle or body aches
  • Headaches
  • Fatigue
  • Some people may have vomiting and diarrhea (More common in children than adults)

Outbreak characteristics

          According to the CDC, people who have the flu are most contagious in the first 3 to 4 days after infection. People who are healthy can start infecting others, 1 day before symptoms develop and up to 5 to 7 days after getting sick (Influenza, 2018). In addition, some people can have the virus, have no symptoms and spread it to others.

          The World Health Organization’s Global Influenza Surveillance and Response System was established in 1952 (Influenza, 2018). The purpose is to reduce the spread of infection by vaccines.  Based on the surveillance system, Influenza activity in the United States remains low throughout the summer months and early October. Outpatient illness surveillance: The proportion of outpatient visits for influenza-like illness remained low and was 1.4% which is below the national baseline of 2.2%

To prepare:

  • With your quality improvement issue in mind, as well as the other work you have completed on your Course Project thus far, think about how creating a process map could help you to better understand your quality improvement issue and redesign an associated process.
  • Create a process map using Microsoft Word or PowerPoint.
  • Refine the strategies for promoting systems-level change to minimize or close the performance gap(s) that you began to think about in Week 7. As part of this, identify a way to redesign at least one process based on your analysis of the process map that you have created.

To complete:

  • Write a 2- to 3-page paper in which you describe quality improvement strategies that you selected related to your quality improvement issue.

The major difference between a Doctor of Nursing Practice (DNP) and a Doctor of Philosophy degree (PhD) is whether nurses translate research into practice to improve patient outcomes with a DNP or whether nurses want to advance the science of nursing, conducting research using a PhD (AACN, 2018). Most nurses who want to complete their DNP are oriented in the direction of improving outcomes, patient centered care and population health. Most nurses who want to complete their PhD are oriented in the direction of developing new nursing knowledge and scientific inquiry (AACN, 2018). Nurses who are committed to their practice will usually choose to purse a DNP when deciding on a doctoral program and nurses who want to commit their lives to research will most likely choose a PhD program of study. According to the American Association of Colleges of Nursing (2018), nurses who complete the DNP program or PhD program of study will be able to contribute to health care improvements through the development of practice and new knowledge from scholarly expertise in teaching and research.

Personally, if I had to choose between a DNP or a PhD in nursing I would go with a Doctor of Nursing Practice-Psychiatric Mental Health Nurse Practitioner (DNP-PMHNP), since that is where my passion and clinical expertise remains. After completing my BSN, I plan on becoming ANCC Psychiatric-Mental Health Nursing certified. There is so much more to discover when it comes to assessing, diagnosing, and treating patients who are seeking help with mental health disorders. I wish to acquire the advanced skills and abilities to integrate my knowledge, while making a positive contribution to the community and improving patient outcomes. I see myself as a leader in health care, taking initiatives to promote health, educating peers, patients and other health care providers.  I believe that a DNP would be in my best interest when it comes to my professional and personal goals as a professional nurse.

The Doctor of Nursing Practice (DNP) prepares BSN or MSN degreed nurses for the highest level of nursing leadership and practice within organizations with the ultimate goal of improving health care. The DNP will prepare nurses to become nurse leaders in interdisciplinary health care teams by providing the skills necessary to translate evidence gained through nursing research into their practice. The DNP program focuses on evidence-based practice and quality improvement and system leadership. The PhD in Nursing prepares nurse “scientist” to develop new knowledge for the science and practice of nursing.  They are prepared for an intellectual inquiry through scholarship and research. They pursue research interests; utilize external funding and creating well-designed methods to discover new knowledge related to nursing. PhD nurses will lead interdisciplinary research teams, design and conduct research studies and disseminate knowledge for nursing and related disciplines, particularly addressing trajectories of chronic illness and care systems. During my research of the DNP and PhD, I learned things about both degrees that I was not aware of. If I decided to continue my education to the doctoral level, I think I would go for the DNP. The DNP is more evidence based and hands on. I am more a hands on person and prefer to have the evidence there in front of me.

Difference between DNP AND PhD
 Based on Curriculum Content wise
The DNP is a practice doctorate that places greater emphasis on direct clinical practice. The curriculum focuses on organizational management, systems leadership, clinical-practice administration, quality improvement and program evaluation, in addition to other specialty areas, and less on statistics and research methodology.
 Ph.D. is more research-intensive and takes a scholarly approach to nursing. The curriculum for these degrees focuses on scientific inquiry and provides the evidence for nursing practice, as well as improving general nursing knowledge.
Based on Research Competencies
DNP-prepared nurses develop the skills necessary to implement the scientific findings conducted by nurse researchers with a Ph.D. or DNS (Doctor of Nursing Science).
