The RN to BSN program at Grand Canyon University meets the requirements for clinical competencies as defined by the Commission on Collegiate Nursing Education (CCNE) and the American Association of Colleges of Nursing (AACN), using nontraditional experiences for practicing nurses. These experiences come in the form of direct and indirect care experiences in which licensed nursing students engage in learning within the context of their hospital organization, specific care discipline, and local communities.

This assignment consists of both an interview and a PowerPoint (PPT) presentation.

Assessment/Interview

Select a community of interest in your region. Perform a physical assessment of the community.

  1. Perform a direct assessment of a community of interest using the “Functional Health Patterns Community Assessment Guide.”
  2. Interview a community health and public health provider regarding that person’s role and experiences within the community.

Interview Guidelines

Interviews can take place in-person, by phone, or by Skype.

Develop interview questions to gather information about the role of the provider in the community and the health issues faced by the chosen community.

Complete the “Provider Interview Acknowledgement Form” prior to conducting the interview. Submit this document separately in its respective drop box.

Compile key findings from the interview, including the interview questions used, and submit these with the presentation.

PowerPoint Presentation

Create a PowerPoint presentation of 15-20 slides (slide count does not include title and references slide) describing the chosen community interest.

Include the following in your presentation:

  1. Description of community and community boundaries: the people and the geographic, geopolitical, financial, educational level; ethnic and phenomenological features of the community, as well as types of social interactions; common goals and interests; and barriers, and challenges, including any identified social determinates of health.
  2. Summary of community assessment: (a) funding sources and (b) partnerships.
  3. Summary of interview with community health/public health provider.
  4. Identification of an issue that is lacking or an opportunity for health promotion.
  5. A conclusion summarizing your key findings and a discussion of your impressions of the general health of the community.

While APA style is not required for the body of this assignment, solid academic writing is expected, and documentation of sources should be presented using APA format ting guidelines, which can be found in the APA Style Guide, located in the Student Success Center.

As a diabetic patient, control of glycemic index is very important with proper nutrition and adequate calorie intake to promote wound healing. Also, since she lives alone,has  been in bed for 3 days and complained of not having any help with meal preparation. Nutritional intake could be critically imbalanced in her case and strategies for weigth reduction.

With thick yellowish drainage and elevated WBC, wound is probably infected, would need antibiotic regimen to combat infection, NSAIDS to reduce fever and decrease swelling and pain.

Physician to assess extent of tissue damage and make recommendations for wound care. Wound care team/ PT to be involved in wound treatment and care. Would possibly need wound debridement and wound vacuum assisted closure.

Likely muscle groups affected include: the gastrocnemius(back calf muscle), extensor and flexor digitorum longus, anterior tibialis muscle, extensor hallucis longus of the ankle and fibularis longus. (ARC Media). 

3.What is the significance of the subjective and objective data provided with regard to follow-up diagnostic/laboratory testing, education, and future preventative care? Provide rationale for your answer.

Objective data gathered indicate a non-healing wound that is infected going by the lab results. also, yellowish wound drainage, elevated temperature and swelling to leg. As a diabetic patient this is an indication of lack of good glycemic control. Rationale is, elevated blood sugar levels prevent nutrients and oxygen from energizing the cells that fights infection and promote wound healing, prevents the immune system from functioning efficiently and increases inflammation in the body which slows down wound healing. (Dening, 2017). So going forward, blood glucose level monitoring and control is very crucial and weight reduction.

subjective data which is what the patient tell us indicate inability for Ms.G. to care for self and deficient calorie intake, as evidenced by complaint of lying in bed for 3 days, living alone and no one to help with meal preparation. Adequate nutrition is very essential to wound healing. Going forward, provision of balanced nutrition will be needed especially, high protein intake for collagen formation to promote wound closure. (Russell, 2001). It is noted that exudate loss results in protein deficit and need to be replaced. (Russell, 2001). She will need the services of OT and social workers to help with nutritional management and provision. Also, home health assistance with meal preparation and ADLS or meals on wheels services. Need to be properly educated on diet, exercise and blood glucose monitoring / therapeutic treatment of elevated blood sugar and care of lower extremities to prevent injuries.

the patient has an acute systemic infection and a draining wound with a Staphylococcus aureus. According to the CDC, Staphylococcus aureus is among the bacteria that cause necrotizing fasciitis which is inflammation of the fascia [tissue under the skin that surrounds muscle, nerves, and fat] (CDC, 2018).  As I mentioned in my post, patient education about managing blood glucose level and weight is vital to healing and preventing recurrent infections.  Many patients I take care of are admitted with cellulitis and many also have diabetes. I noticed that the lack of access to primary care and sufficient funds is one of the reasons patients are not able to manage their diabetes.

The disease process I chose to discuss is heart disease and stroke. Heart disease is caused by various risk factors including, smoking, hypertension, high cholestrerol, poor diet, and inactivity. Patients can also be genetically predisposed to heart disease. Healthy people 2020 has set a goal and interventions to help reduce the amount of people affected by this disease and to improve the quality of life of those that are affected. Improve cardiovascular health and quality of life through prevention, detection, and treatment of risk factors for heart attack and stroke; early identification and treatment of heart attacks and strokes; prevention of repeat cardiovascular events; and reduction in deaths from cardiovascular disease (Healthy People 2020, 2018). This goal can be met by patients becoming aware of their risk for heart disease. This takes place by having screenings such as labs to check for high cholesterol, smoking cessation counseling, and management of high blood pressure. The CDC supports Healthy People 2020 in reaching their goal. With $165 million for heart disease and stroke programs (including WISEWOMAN) in Fiscal Year 2018, CDC supports all 50 states and the District of Columbia to conduct heart disease and stroke prevention efforts (CDC, 2018). The goal can be met with interventions being implemented not only by patients but healthcare providers. Healthcare providers must use screenings to determine the risk of heart disease. Also important would be educating the patient of the effects of the disease and management of the disease including compliance with medication such as a daily regimen of aspirin if not contraindicated. Heart disease and stroke are big topics in the United states that need awareness and initiatives for meeting goals. In addition to being the first and fifth leading causes of death, heart disease and stroke result in serious illness and disability, decreased quality of life, and hundreds of billions of dollars in economic loss every year (Healthy People 2020, 2018).

The Joint Commission on Accreditation of Healthcare Organizations (JCAHO) reports that 126,000 nursing positions are unfilled in hospitals, accounting for an overall vacancy rate of 13% for nursing positions” (Robinson, Jagim, & Ray, 2004). The nursing shortage is a personal and proffesional hazard not only to the nurses, but the patients as well.”If staffing is inadequate, nurses contend it threatens patient health and safety, results in greater complexity of care, and impacts their health and safety by increasing fatigue and rate of injury” (Gooch, 2015). I work in a small ER in California. Now, most people will say, “California has mandated staffing ratios”, but I am here to tell you by experience that that statement is not 100% accurate. There are MANY nights where we are understaffed and over ratioed, with only 3 nurses to triage, asses, start IV’s, medicate, chart, discharge and sometimes  handle multiple codes and traumas in one night. And no, most nights we do not have any ED techs or CNA’s to help with EKG’s, placing people on monitors or any of those types of task. Being short staffed is physically and mentally draining and extreamly unsafe for patient care and overworked nurses. So the question is why is there such a nursing shrotage? Is nursing school/state boards to strict? Is it because the  aging workforce is moving into retirment and into less stressful roles? Is it because of overall job dissatisfaction with the heavy workloads, underpay, and inadequate staffing? All of these could be a reason as to why there is a large nursing shortage and should be further evaluated to help promote nurse and patient safety.

5-One of the most commonly seen errors in the nursing fields
that has become a growing and significant problem, can be seen in patients
being incorrectly administered their medications. All too often, patients are
given the incorrect medications; whether this is due to the nurses who are
administering the medication, the doctors who write out the initial
prescription, or the pharmaceutical workers who fill the prescriptions in the
first place (Keers, Williams, Cooke & Ashcroft, 2013). The errors seen are
not entirely on the medical side of this, as it can be seen that patients can
also incorrectly dose themselves as well (Wright, 2013). Aside from being given
the incorrect dosage of medication, patients can often be given the incorrect
medication in general, or given their medications in either too short, or too long
of a time span (Wright, 2013).

            Medical malpractice is an incredible cause of
concern, not only for the issues that can arise from a legal standpoint by
making errors in medication distribution, but from the severe risk of harm
coming to the patient, or even the death of the patient due to receiving a high
dosage of a medication that was incorrectly given (Makary & Daniel, 2016).
Medical malpractice is the third highest cause of death in the United States,
and in order to remedy this dire state of the medical field and for the public
to regain their trust in medical professionals, hospitals must take steps
towards correctly administering their medications (Makary & Daniel, 2016).
This is a role that must be worked at diligently, by not only the nurses at the
facility, but the doctors and pharmaceutical workers as well (Keers et al,
2013).

         6- One area that I observed that could be studied more is the long shifts that nurses are required to work. This leads to fatigue and risk of medical mistakes. Industrial research over much of the 20th century indicates worker productivity declines significantly after 10 to 12 hours of work (ANA journal, 2017). A tired nurse is more likely to make errors, putting themselves and patients at risk. Its normal for nurse to work 12 hours shift, however, many times after a challenging 12 hour,s nurses are expected to put in another 4 or so hours when the need arises. Furthermore, many nurses have a long commute home, so they are up for another 6 -8 hours, as they need to take care of their families. After which, possible a few hours’ sleep and back to another long challenging shift. With all this, many nurses are studying and must find time for classes and assignment, which mean less time for rest.  According to U.S. Army studies, staying awake for 17 hours is functionally equivalent to having a blood alcohol concentration (BAC) of 0.05%; staying awake for 24 hours equates to a BAC of 0.10%. (In most states, it’s illegal to drive with a BAC of 0.08% or higher.) It further states that loss of even one night’s sleep can lead to short-term memory deficits and impaired cognitive functioning and the risk of bloodborne-pathogen exposure for workers increases during the last 2 hours of a 12-hour shift (ANA journal, 2017).

               Injury risk and the risk for patient error as risen significantly. These risks are not worth the convenience of 12-hour shifts, therefore more studied should be done to highlight the negative effect overwork has on the body and the risk associated with nursing fatigue. The aim is to improve patient safety, nurses’ safety and job satisfaction and not vice versa.
