1) According to the Youth Physical Activity: The Role of Schools, what schools can do to promote physical activity describes various activities that schools can practice to encourage students to be physically active for example: policies that support physical activity, educate parents on the importance of physical activity, motivate school staff to be physically active to be role models for students, and collaborate and invite families and local groups to participate in school-base physical activity and events that promote physical activity (Youth Physical Activity:The Role of Schools, 2009). As I reflect on my experience in elementary school we didn’t have a physical activity policy or physical activity programs. Our teacher would take all the class out to play a sport once a week and that was considered our P.E. time. Teachers didn’t get involved, they usually just watched students. I don’t remember ever having an event that promoted physical activity. I also remember that students couldn’t join a sport team until they reached 5th or 6thgrade and lower grades couldn’t participate until they reached 5th grade. I remember complaining to my parents about this, I wanted to participate and wasn’t allowed until I was in 5th grade. I felt my school did a horrible job in promoting physical activity.  We did have recess but didn’t have a quality physical education class nor physical activity breaks.

  

2) The CDC recommends that children, aged 6-17, get at least 60 minutes of moderate intensity activity daily which includes aerobic, muscle strengthening, and bone strengthening at least 3 times per week. (CDC, 2018). This recommendation is actually doable, but I feel when not influenced by parents/caregivers and school officials, this is easy to be dismissed in this day of increased technology use and indoor activities.
Although it has been quite some time since I was in school, I do have recollections of being in 4th and 5th grade and having ample opportunities daily to run and play before school, after lunch and during daily physical education classes. However, as we advanced in grade, the requirements and opportunities to exercise during the school day became less and replaced by more academic requirements. When looking back on my high school years, I recall only limited requirements for physical activities, unless we were encouraged, or interested in participating in a sport. As a part of one of my PE classes, I joined the swim team where I learned techniques and competed for one semester. In recalling my children’s time in school, I find that they also suffered from minimal times and encouragement for physical activity. As they got older, in high school for example, they were only required to have a total of 2 years of physical education or other equivalent training/participation.
But, because we were already a pretty active family, my kids not only had a variety of activities throughout their school times/days, but also during the weekends, vacations, etc.
The decline of the school-based promotion and requirement for daily physical activity in children has the potential to not only harm the child, physically, but mentally, and academically. According to a study of the effects of classroom-based physical activity done by Donnelly and Lambourne, there is “increasing evidence for the association between physical activity, cardiovascular fitness, fatness , and cognitive function “. (Donnelly & Lambourne, 2011). One example of this was noted by the Action for Healthy Kids organization which strives to help communities, schools, and parents tools and tips to help increase physical activity in children. This organization helps in fundraising and providing grants to schools who are unable to financially support increased or basic activities in their schools. In a report from 2013, the Glen Acres Elementary School in Lafayette, Indiana, was given a $5000 grant to support a program that was initiated by a PE teacher seeking to help their kids an additional 15 minutes of activity per day. She started a project that provided for kids in all grades to begin their school day with a 15 minute light workout/stretching and sending them off to class with a positive affirmation and feeling better. The other teachers all noted improved academic performances during this trial period and she found that even the children were asking for the program and were disappointed on those days that the “Stretch & Go” program was not available. This as found to be a very beneficial program for them and I feel all schools could incorporate something similar in their daily curriculum.
Although many children today are involved in extra-curricular activities and are shuffled around by well-meaning parents, I feel this type of activity, although beneficial in the long run, may not create the type of life-long habits of being active that our children today need in an increasing sedentary world.

When reviewing the Healthy People 2020 objectives for Access to Health Services, all of their objectives are legitimate and important.  All of the topics are problems in our healthcare system, however, I feel that increasing the persons with a usual care provider and increase the number of practicing care providers is the most important (Healthy People, 2018).  I do not know if this is just a problem in California, or if it is nationwide, but patients in my area have an extremely difficult time finding a primary care provider, whether it is a Physician, Physicians Assistant or Nurse Practitioner.  There is such a shortage in my area, that patients have to wait months to get established with a provider.  Then when they are ill or have an acute problem their provider is unable to see them within a few days because there is no appointments for weeks.  Most end up going to a prompt care type setting or to the emergency department.  I believe that if we can increase the number of practicing care providers patients will then be able to have a usual care provider that can see them in a timely manner for more acute problems and to adequately manage the patients chronic illness, leading to fewer emergency department visits and hospitalizations.  

This is one of the reasons I would like to pursue a Master’s Degree and become a Family Nurse Practitioner after obtaining my BSN.  I see so many patients in the Emergency Department, that are there every week for problems that would be easily treated by a physician or mid-level.  Most of these patients are elderly without access to resources and are failing outpatient treatment.  When you have a practitioner that can see the patient, set up in home resources and follow up, the patients outcome is likely to much more successful.  

It appears that the shortage of primary care providers presents a challenge for the entire US healthcare system and is not only in my area.  Historically, physicians did not present with a positive attitude when the topic of including PAs and NPs, as key players in augmenting primary care.  Current medical practice has become a complex, multilayered problem, that is requiring a team approach.  Physicians alone will be unlikely to meet future demands for primary care and and also specialty care. Therefore, greater attention will likely be focused on the PA and NP profession and its ability to continue to adapt to meet society’s healthcare needs (Coplan, Smith & Cawley, 2017).

            Caring for patients in healthcare today, providers will encounter many different patient populations that come from a diverse set of cultures, lifestyles, and backgrounds that have beliefs and practices regarding health and illness which may be different from the provider. Providers must first examine their own beliefs, values, and culture for self-awareness and to examine for potential bias. Approaching individuals from a variety of cultures and backgrounds also requires flexibility and the ability to adapt the health history and physical exam to the patients’ needs or culture. Having an understanding of the patient’s beliefs, lifestyle, culture, and background and how they impact the patient is necessary for providers to provide culturally competent care.

 The patient selected for this discussion is MR, a 23-year-old male patient, complaining of anxiety.  

Identifying Information: MR is a 23-year-old Native American male seen in the office on September 5, 2018. The history is obtained from both a written questionnaire filled out before the visit and from the patient, and he is considered a reliable historian.

Chief Complaint“I’ve been anxious lately and used both pot and alcohol to help me feel better” and is concerned about “not getting into heaven.”

History of Present Illness: a 23-year-old male patient that uses both marijuana and alcohol for anxiety symptoms. MR describes the anxiety as starting when he lost his job one month ago. MR describes the anxiety occurring on 4-5 days during the week, and MR describes it as a “feeling of fear about providing for my family,” and on 4-6 days/per week he uses alcohol and smoking “pot” to “feel better.”

Current Medications-None

Family History- Diabetes-Father, Hypertension-Mother, and Alcoholism-Father.

Social History-smokes marijuana –per patient written questionnaire: typical amount described as 1 “joint”; with use of 6-7 times per month. Drinks alcohol, described as beer; amount varies from 36-64 ounces 3-6 times per week. Occasional use of liquor; the typical amount is 3-8 ounces per week usually with beer. Smokes cigarettes with a six pack year history.

Native American patients may have a lower socioeconomic status with the median annual household income is $ 37,353 for Native American households compared to $ 56,565 for non-Hispanic whites (US Department of Health and Human Services [HHS], Office of Minority Health [OMH], 2018).  Patients with lower socioeconomic status may not seek medical care due to lack of insurance, ability to pay insurance premiums, have transportation issues, not fill prescription medications, or take prescribed medications sparingly with the belief to “make the medication last longer.”

Native American patients’ spirituality is also a cultural factor that requires cultural competence for the practitioner to understand how the patients’ spiritual practices influence his care and how the provider can assist the patient. The practitioner must have an understanding of the relationship between spiritual beliefs and health practices and how beliefs relate to illness, health, family, symbols, and taboos (Sullivan, 2012). The patient’s spiritual belief may include that mental health issues are viewed as spiritual punishment that is unable to be healed.

Native American patients have twice the rate of diabetes than non-Hispanic white patients (HHS, OMH, 2018). The influence of both smoking cigarettes, marijuana and a family history of diabetes and hypertension concerns for the development of diabetes and hypertension in the future.

The Native American culture has undergone a significant shift in the past 30 years. Nearly 60% of Native Americans now live in major metropolitan areas, and 22% still live on reservations (HHS, OMH, 2018). This shift from reservations to metropolitan areas has changed the Native American lifestyle and culture which strives to find harmony and live with nature (Ball, Dains, Flynn, Solomon & Stewart, 2015). Native Americans have higher rates of alcoholism, and chronic liver disease either from alcoholism, obesity, and exposure to hepatitis B and C virus is the leading cause of death (HHS, OMH, 2018).
