COMMENT 1

primary intervention in disaster planning is the planning prior to the disaster (Davis, et al., 2010). In the case of the Haiti earthquake the disaster was present prior to the actual earthquake. The country was already stricken by poverty. This national poverty lead to infrastructure problems that were difficult for disaster response to overcome. 

Managing the problem of impoverished nations infrastructure prior to a disaster would slow the progression of disease after disaster strikes. The healthcare needs of Haiti were not being met prior to the disaster. Hospitals were underfunded and when disaster struck the hospitals and medical facilities that were not built to be earthquake resistant were severely damaged. Funding of proper facilities and education of disaster planning of healthcare workers and the community would have gone a long way in easing the extent of the Haiti disaster. 

Tertiary interventions directed at managing this disaster would be funding of programs to improve the economy and lessen the impact of poverty for Haiti. Once immediate healthcare needs are met, the process of rebuilding with a focus on overall improvement of economy so that it is sustainable for the future is needed. 

There are many organization and charitable foundations involved in improving the economies and infrastructures of countries to lessen the overall impact of poverty. International Action is a charitable foundation that currently has a focus on bringing clean water to Haiti. They are building sustainable supply of clean water. The organization is working the local population to provide affordable clean water. I would choose to work with this organization because they are making an improvement in an infrastructure that improves health and safety of a basic necessity, clean water. 

COMMENT 2

Primary prevention with patients who experienced the 2010 earthquake in Haiti, would focus on preventing disease from occurring. For Instance, the community health nurse could attempt to prevent patients from breathing in dust, and air contaminants by having them, and staff wear masks. Medical staffing was somewhat scarce, however staff available are to make sure to use universal precautions to ensure that patients are not exposed to illness, and disease, from other patients, by staff who did not take their time to wash their hands properly, and Don gown, and gloves when necessary.

An example of a Secondary Prevention, would be to require staff keep all patient wounds covered and clean. Patients that may have suffered injury, or loss of a limb would need proper wound care.  Although, water supplies were limited, making sure that patient wounds are cleaned with peroxide, or betadine, and keeping the wound covered with clean bandages so that patient’s wounds wont’s become infected. Also, if patients skin has been exposed to foreign objects (such a wood or nails) if the patient is not up to date on their shots they may need a tetanus shot or booster. Tertiary intervention would focus on reducing complications of disease. Patients that were suffering from Sepsis would need to be monitored closely, and treated with antibiotics or Patients with fever would be treated with antipyretics.

All three of the proposed interventions listed above fall under the Post Impact Phase. “The post impact phase has to parts, the emergency phase, and the recovery phase” (Mauer, & Smith, 2009 All the above listed interventions happened during the Post Impact Phase. The disaster had already occurred, and these are the steps to be taken after. In a disaster situation, I would work with organizations such as FEMA. FEMA’s (or the Federal Emergency Management Agency) “mission is to help people before, during, and after disasters” (FEMA, 2018)

Clinical Check-Offs 

1. Medication Administration Check-off

STUDENTS CAN ONLY ADMINISTER MEDICATIONS WITH THE CLINICAL INSTRUCTOR. This includes the administration of any kind of medication by any route. Medications cannot be administered with a staff nurse. Failure to follow this rule will result in failure of the check-off, immediate dismissal from the clinical site and inability to pass NM 322. 

The clinical instructor will give medications with two students per day. Students must pass the medication check-off by the end of the semester.

2. Clinical Performance Evaluation

The Clinical Performance Evaluation will be reviewed with the student as needed, at mid-semester and at the end of the semester.  Failure to satisfactorily meet core competencies on the Clinical Performance Evaluation can result in being sent home from the clinical site, an academic alert, a corrective action plan and/or dismissal from the nursing program.

Please review the Student Laboratory and Clinical Handbook, especially the sections entitled “Standards for Student Behavior in the Laboratory and Clinical Setting” and “Clinical Attendance Policy.” 

Other Clinical Expectations

1. Timeliness

Students are expected to be in the assigned room ready to go for pre-clinical at 2:00 pm. Time will be determined via the instructor’s cell phone. 

Students arriving after 2:00 pm will be given the opportunity to prepare for patient care. Failure to complete the Pre-Clinical Patient Care Form completely by the end of pre-clinical will result in the student being sent home from the clinical shift and losing all points associated with the clinical day. 

Students who are able to complete the Pre-Clinical Patient Care Form will be allowed to stay for clinical, but will lose all points associated with the clinical day. All assignments must still be completed for the clinical day. 

Failure to arrive on time will be reflected on your Clinical Performance Evaluation. Failure to meet core competencies on the Clinical Performance Evaluation can result in being sent home from the clinical site, an academic alert, a corrective action plan and/or dismissal from the nursing program.

Any student who is not at pre-clinical at 2:00 pm needs to have notified their clinical instructor. See the first page for how to notify instructor.

2. What You Can Do as a Student Nurse

Provide the best possible care for your patient that day. This includes assessing the patient, documenting your assessment and providing personal cares. Learn as much as you possibly can from this experience. Attend therapy or tests/procedures with your patient. Please notify the patient’s nurse, so they know where you and the patient are at all times. 

Students should answer call lights when they are not providing care for their patient. The student should then check with the patient’s nurse regarding the patient’s request before carrying it out. For example, a patient may ask for water and be NPO. The student would not know this without checking with the nurse. A patient may say they can walk to the bathroom, but really need assistance with transferring. 

3.  What You Can’t Do as a Student Nurse (Per BSN Clinical Instructor handbook)

1. Administer Blood

2. Program a PCA pump

3. Manage Epidural Infusion, Tubing or Pump Settings

4. Do EKG or Fetal Monitoring Interpretation (it is crucial that students not document in a fashion that would suggest they have done any monitoring interpretation)

5. Take Verbal or Telephone Orders

6. Verify Informed Consent

7. Advance the plan of care (must be licensed professional)

8. Blood Glucose Monitoring 

9. Blood draws from any line

10. Discontinue a central line

11. Administer IV push medications, including IV flushes

12. Start IVs

13. Administer chemotherapy

14. Insert a urinary catheter (Regions new policy)

4. What You Can Do as a Student Nurse, But Only with the Instructor Present

a. Administer medications by any route

b. Anything IV or tube feeding related (hanging a new bag, programming the pump, etc.)

c. For anything other than a patient assessment or personal cares, the student must connect with the clinical instructor. The clinical instructor will then determine if the student can perform the skill with the patient’s nurse or the instructor. Students can always observe a nurse doing anything for a patient, if it is okay with the patient.

Failure to follow the rules regarding what a student can/can’t do will be reflected on your Clinical Performance Evaluation. Failure to meet core competencies on the Clinical Performance Evaluation can result in being sent home from the clinical site, an academic alert, a corrective action plan and/or dismissal from the nursing program.

As nurses, we have a duty to protect their information, rights, dignity, and act as a voice for them when they aren’t able to speak up or advocate for themselves. Having the courage to say something when certain aspects of care are missed or the best interest of the patient is overlooked is another role nurse’s play in patient advocacy. Patients may need you to speak up for them regarding family members who may not have the patient’s best interest in mind as well. “Advocates become facilitators when patients and family members need to discuss uncomfortable information or explore its implications. Nurses may sometimes need to advocate for patients against their families” (Gerber, 2018).

Patient safety is a huge goal for advocating for patients. For example, if a doctor makes a mistake and prescribes a certain drug dose that doesn’t seem right for that patient, it is important to question that and verify it with the provider, even though you may feel uncomfortable doing it. The patient’s safety and health are at risk and it is up to us to be the final check for those circumstances.

A situation I can recall occurred during one of my clinical rotations. A patient I was assigned to with my nurse was homeless and spent a few days on the med/surg unit recovering from pneumonia. The patient was sheduled to be discharged and both myself and the nurse didn’t feel right about the situation. The patient had no where to go and was still recovering. We wanted to make sure the patient could transfer herself from her wheelchair to the bathroom so she didn’t soil herself and contacted a social worker to help find a place for her to go. Once she had a place set up, we bathed her before leaving and made sure she was able to transfer herself before discharging her. If she didn’t have an advocate, she could have not recovered and healed appropriately in addition to being at risk for falling and harming herself. This would have resulted in a worse condition than she came in to begin with. It was a wonderful learning experience to witness.
