Extraneous Variables are undesirable variables that influence the relationship between the variables that an experimenter is examining. These variables are undesirable because they add error to an experiment. A major goal in research design is to decrease or control the influence of extraneous Variables as much as possible. (Psychology world, 2017). The extraneous variables can be hold constant by creating a standardized environment and procedure so that all variables are the same in each condition and therefore cannot be confounding. These variables exist in all studies and can interfere with obtaining a clear understanding of the relationship among the study variables. For example, if a study focused on the effect of relaxation therapy on the perception of incisional pain, the researchers would have to control the extraneous variables, such as type of surgical incision and time, amount, and type of pain medication administered after surgery, to prevent their influence on the patient’s perception of pain. Selecting only patients with abdominal incisions who are hospitalized and receiving only one type of pain medication intravenously after surgery would control some of these extraneous variables. Controlling extraneous variables enables researchers to determine the effects of an intervention or treatment on study outcomes more accurately. (Grove, Gray & Burns, 2015).

Quantitative research is a type of formal, objective type of research and generates numerical information about concepts and relationships between variables and includes descriptive, correlational, quasi-experimental and experimental (Burns & Grove, 2011). 

The type of quantitative research used is based on the amount information the researcher has on the subject before the experiment. If a researcher knows little about the topic they may begin with descriptive to gather better information then move on to correlational, quasi-experimental or experimental studies (Burns & Grove, 2011). Descriptive research is typically without manipulation and leads to further quantitative studies (Burns & Grove, 2011). 

Correlation studies describe relationships between variables in a positive, negative relationship or no relationship such as smoking cigarettes negatively affecting the smoker’s life span however does not study to which a degree or the exact cause and often will lead to a more extensive study such as quasi-experimental or experimental research (Burns & Grove, 2011). 

Quasi-experimental research examines relationships to better determine a cause-and-effect relationship between variables and involve implementing a treatment to a concern and evaluating the effects (Burns & Grove, 2011). Quasi-experimental research involves less control of variables than strict experimental research and often utilize humans or patients as a variable and is most often used in nursing research (Burns & Grove, 2011). An example of quasi-experimental research can include a certain exercise program for cardiac rehab patients post CABG and the effects on improving the health or assisting in the rehabilitation.

The definition of family has always been determined by societal acceptance. By historical reference, the definition of family was linked to blood lines or marriage. Through the years, societal attitudes regarding adoption, foster parenting, divorce, remarriage, and LBGTQ rights have changed the traditional definition (Green, 2018). Today, family is defined by more than blood ties or marriage. It is defined by function, level of commitment and attachment. In fact, some family members may not even live in the same household. According to Wright and Leahey (2012), “The family is who they say they are” (p.55).

Nontraditional family structures need to be acknowleged when working with patients. Perceptions of wellness and the health disparities present are influenced by the dynamics within a family system. For instance, a grandparent raising a grandchild may have different attitudes toward health promotion for the child, health issues of their own, or be experiencing economic disparities. In addition, caregiver roles may differ from a traditional family structure in single parent family. Understanding who is “family”, and how the family functions is key to creating stimulus for change in health promotion. The Calgary Modle of Family Assessment (CMFA) is one tool nurses can use to gain a better understanding of the family dynamics (Green, 2018). 

Bowen’s family systems theory can also be used to gain a better understanding of the interactions within a family. The family is made up of individuals that impact the lives of the other individuals within the family. For instance, a parent who has a substance abuse problem creates financial, emotional or even physical consequences for every other individual within the family. Any stressor or change within the unit, can create a ripple effect of negative or positive individual consequences that influence the entire family. Using the CMFA model the nurse can begin assessing the family in the initial assessment. This simple assessment can be utilized to understand the connections and interactions within her patient’s family. Then appropriate nursing interventions can be created that the family as a whole can adopt. Since the family is encouraged to work together toward a common goal, compliance and the incidence of desired outcomes increases (Green, 2018).

The traditional definition of a family is two parents, man and a women living together with there biological offspring with the same ancestry. (Green, S.Z.). Times have changed tremendously and the definition of family can take on several different meanings. Family of choice is a more accurate term to call families. It is who people decide are their family and it doesn’t depend on bloodline, gender, racer sexuality.

The importance of acknowledging nontraditional family structures is because who they decide is family affects their outlook on their health patterns. For example, A NICU mother did not visit her child to do skin to skin and breastfeed as planned. It is unusual for a mother to skip. She called to say had to rush her dog to the emergency room which turned into an emergency surgery because he almost died. This is a family by choice. At this moment she was distraught and made a decision switch baby to formula and no longer breastfeed, which can be done but for the health outcomes of the child breastmilk is best. Understanding the family dynamics was important on how to aide in education on health promotion. Encouraged to take efforts to get the dog well and educated on different options such as expressed breast milk/donor milk for her growing baby in the meantime. Giving her time and understanding this is her family dynamic made all the difference to keep the baby on the health plan.

The family systems theory describes how a family interacts with one another and society. It’s divides into different systems. A whole system is the entire family. A subsystem is an individual within the family 

And a supersystem is the community. Each of the systems affect one another. (Green, Z.E) For example, a husband is going through a program to quit smoking. His wife smokes as well. They both decide to go through the program together to elevate the triggers that would cause one to relapse. Everyone in the family system affects one another.

This study is a cross-sectional descriptive study that talks about chronic wounds in a nursing degree program. This article is about a study that offers a point of reference to consider where we are in this field and what should be done to improve the preparation of healthcare professionals, thereby improving the prevention and treatment of chorionic wounds.  114 colleges were analyzed in this study.

Conclusion:

This article concludes that they observed differences in the presence of the four top educational priorities in wound management and tissue repair. Pain management in patients with the chronic wound is not addressed in any course, and more courses consider the treatment of pressure ulcers than their preventions

Mental  health disorders, even though suffered by many, remains stigmatized and  receives less attention than other health care issues (Swanton, 2016).   Many patients are not willing to disclose their mental health diagnosis  because of the fear of being looked down on or judged. There are also  limited mental health resources as well as some communities are not able  to gain some of these benefits due to insurance issues or the inability  to pay (Choi et al, n.d.).

             Two of the issues, that this writer believes need change are that of   the granting of FMLA leave to patients with diagnosed psychiatric  disorders and making treating options available to everyone. This writer  has worked with several clients who were having difficulties being  granted  FMLA or being denied. The FMLA is the Family and Medical Leave  Act. It works on behalf of those with medical and mental health  disorders who are unable to perform their job duties due to the  disability from the diagnoses (Wray et al., 2015).  Clients with  physical ailments may find it less difficult to be granted this paid  leave as their disorder is visible by the employer and those granting  the leave. When it comes on to mental health disorders, client tend to  run into several road blocks as their disorder is not readily visible. 

Many  clients explain that their request was denied or met several obstacles  as their diagnoses was not considered chronic or disabling. Many of the  clients argue that anxiety and depression make it difficult for them to  carry out their jobs that require them to come in contact with people.  The increased stress from interacting with others and trying to fit in  to the workplace can be debilitating but is rarely considered so by  employers. This is an area that needs attention and change. This writer  believes that Nurse Practitioners and other health care providers can be  an advocate for the client by providing thorough information on the  client’s illness that support the need for a leave.  The Nurse  Practitioner should be readily available to answer questions and educate  the employers on mental health disorders. In addition, the Nurse  Practitioner could advocate for change on a federal or state level by  writing congress or their elected leaders.

             Another issue that this writer believes need special attention is that  of making special procedures available to everyone. When treating mental  health disorders there are many treatments that would be beneficial for  a client, but the client will end up not receiving the treatment due  their inability to pay or insurance coverage (Wang & Xie, 2019).  This writer has worked with several client suffering with depression who  have exhausted the use of antidepressants but were not qualified for  ECT due to monetary reasons. The client is simply placed back on the  antidepressants with limited hope to treat their disorder. This issue is  also another social change issue that we as Nurse Practitioners could  propose to our elected leaders. The mental health community on a whole  is stigmatized and separated and this only separates them more by  causing a separation amongst themselves.

The Institute of Medicine (IOM) nursing reports from 2010 to 2016 are the summarized findings of the IOM committee that examined changes in the field of nursing since the publishing of “The Future of Nursing: Leading Change, Advancing Health” (Shalala et al., 2011). The summarized findings found in our class reading assignments outline recommendations and provide an essential path for the individuals involved to implement the changes outlined in these works. Some of the direct “take away” from the IOM nursing reports align with the nursing scope of practice. They identify that nurses should practice to the full extent of their education and training, achieve higher levels of education through an improved educational system, gain rights as full partners with physicians and other health care professionals and require effective workforce planning and policy making through better data collection and an improved information infrastructure.

Throughout my career, I will directly be addressing these main points as I work to achieve and work with full practice autonomy. I recently moved from Maryland, which is one of 28 states that allow nurse practitioners full practice authority to treat and prescribe without formal oversight (Weiland, 2015). This report calls all advanced practice nurses to practice to their full extent, meaning we should implement all necessary and appropriate interventions within our scope of practice as defined by the American Nurses Association. In Maryland, this is an expected outcome in my daily practice where I use my full power to treat patients for the optimal health outcomes. Another area I will strongly indulge in is the necessity for continuing education. As a health care professional, we take on the role of a lifetime learner as we continue to learn best practices and evidence-based practice updates that form as time goes on. Health care is an everchanging field and staying up to date on your education and health understandings can be the difference between patient outcomes. We have a professional duty to our patients to learn everything we possibly can to make the best decisions when developing a plan of care for our patients who expect nothing but competent care. Describe how you will utilize and implement the IOM Nursing Reports from 2010 and 2016 in your role as an Advanced Practice Nurse.

In 2008, The Institute of Medicine (IOM) along with The Robert Wood Johnson Foundation (RWJF) comprised a report that would made recommendations for an action-oriented blueprint for the future of nursing. This was in response to a need to assess and transform the nursing profession. Nurses have an important role in transforming our current health care system (IOM, 2010). The IOM nursing reports have several recommendations that will propel us into the future of nursing.  

There are eight total recommendations and it is important for all advanced practice nurses to focus on the recommendations to provide better practices. There is a lot of focus on education for nurses. IOM states that nurses should have lifelong continuing education, the number of nurses with a DNP should double by 2020, and the number of nurses with a baccalaureate degree should be increased to 80% by 2020 (IOM, 2010). I will have my master’s degree as an advanced practice nurse and depending on the setting of my practice, I will encourage other registered nurses to complete continuing education courses as well as pursue a higher degree in nursing. I, myself, plan to obtain a doctorate of nursing in 2020 if not before that time. Continuing education will help prepare nurses to lead change in advancing health and providing positive patient outcomes, another recommendation of IOM. It is my plan to participate in any continuing education course that is pertinent to my patients and specialty.

            According to the IOM (2010), implementing nurse residency programs will be extremely helpful to nursing. This not only applies to new graduate nurses but nurses going from one position to another. As a nurse practitioner, I will make myself available to participate in these residency programs. If there is a nurse advancing from a staff nurse position to nurse practitioner, I will volunteer to be a mentor or preceptor to them just as I have with nursing students. I currently work at a teaching facility and recently a nursing residency program was implemented. Many advanced practice nurses function in helping this program succeed by teaching and mentoring. Nurse residency programs improve nurses’ work experiences, increase retention of new nurses, and show financial return for organizations investing in nurses as resources. Residency evaluations show increases in leadership and communication skills and decreases in stress and turnover (Kosman, 2011).

            Collaborating with an interdisciplinary team is also a recommendation of the IOM. As a nurse practitioner it will be my duty to collaborate with others so that my patient receives adequate care. The patient should be treated holistically so if there is a service that I can not provide, it would be wise of me to collaborate with other people so that the patient gets the best care possible. Collaboration could include working with other physicians, physical therapists, occupational therapists, case managers, or social workers. 

            As a nurse practitioner, with my knowledge and experience, I should be able to practice to the full extent of my education. Working in Georgia, I will be aware of what my scope of practice allows me to do in this state. If there is something outside of my scope of practice, I will be sure to refer my patient to someone more appropriate. Along with my contributions, congress and legislative members should be diligent in allocating funds to research and appropriate technology as well as making sure that nurse practitioners get proper insurance reimbursements.

With recommendations from the IOM, nurses should be able to provide effective and efficient care. Patients will receive the best care because the nurses will be up to date on current evidence based practices.

1-This is a tough topic.  Here you are, a working family, you are going to school to better yourself even further and you millions of others are still faced with these decisions about health care.  When you say you pray I know you mean that literally because our daughter is in the same boat.  My husband keeps talking about wanting to retire early but he just does not understand the cost of having good health care.  I will probably be working until I am 90 because I will be afraid of Medicare and not being able to afford good care.  Wish we had better answers but we are not getting anywhere fast…that’s for sure. 

  

2-The most important elements of the Affordable Care Act were to get more people insured, to keep costs down and to improve overall care and outcomes.  New laws were to make affordable health insurance available to more people.  The law would provide consumers with premium tax credits that will lower costs for households that qualify.  To qualify households must have incomes between 100% and 400% of the federal poverty level. Another component was to expand the Medicaid program.  This would now cover all adults with income below 138% of the federal poverty level.  In addition, the goal was to support medical care delivery methods designed to lower costs of health care overall.

As nurses, we need to continue to be innovators and continue our work in research.  We need to focus on continuing to improve on patient safety and improving quality of care.  Nurses need to continue to be lifelong learners and meet demands of a changing society.  We must also continue to raise the bar for education with advanced degrees.  We must continue to push for more diversity in our profession to keep up with our diverse society to make patients more comfortable.  We also need to continue to bring our ideas to the table and be leaders within our organizations.  As we foster interprofessional collaboration the patients will benefit from teams working more closely together to improve patient care. 

  

3-The key elements of the Affordable Care Act are the following: providing national access to healthcare coverage for the entire population from birth through retirement, this will provide coverage for about 94 percent of the American people, to improve quality, affordability and fairness of health insurance coverage, to enhance healthcare efficiency, quality, diversity and values, to reduce wasteful spending, and to improve public health by expanding clinical preventive care and community investments.

The Act focuses on improving public health by investing in primary health care, especially in medically underserved communities. It also increased insurance coverage for clinical preventive services and procedures. For instance, physical examination, annual wellness visits, well child visits, immunizations, screening exams, like colonoscopy, mammograms and cervical cancer, are all covered by insurance with no cost sharing. In addition, the development of a national preventive plan and the establishment of a Prevention and Public Health Trust Fund, to fund investments in communities that will improve public health. Further investments are being made to in school-based health programs, preventive oral health care, tobacco cessation and adding prevention planning to Medicare.

Nurses are play a vital role and a an extremely important driving force in the implementation of this law. By developing innovative care models like transition of care, home care, care management, and nurse managed clinics, nurses will improve quality of care and lower health care cost.  Also, participating in research to aid in scientific evidence-based practices which improves quality, safety and patient outcome. Advanced practices nurses help increase the availability of primary care providers and provide cost effective care. Lastly, nursing leadership will be needed to implement better workflows to reduce medication errors, improve quality and care coordination, and increasing access to medical care. For these reasons and many others, nurses are of upmost importance in the implementation process of this Act.
