Please include a narrative that defines the baseline data (this is described in section A below). You may also add additional information as noted in the sample paper if appropriate to your EBP proposal. The narrative must be 2-4 pages in length. EBP Evaluation Plan

1. Define baseline data sources and collection strategies – describe data that has been collected which identified need for proposal. It has to be specific baseline data that the results (outcomes) of your proposed implementation will be compared to post implementation.
2. Define outcome indicators for your project (see table).
3. Define outcome measures (see table).
4. Define how data will be collected – time of collection and person responsible (see table)

Moving stage—The change agent clarifies the need to change, explores alternatives, defines goals and objectives, plans the change, and implements the change plan. The change agent develops a plan of action after identifying possible objections and reasoning and includes all parties that will be affected by the plan. The change agent serves as a resource and point support during the implementation of the plan, then evaluates the efficacy of it, and modifies if applicable.

3. Refreezing stage—The change agent integrates the change into the organization so that it becomes recognized as the status quo. If the refreezing stage is not completed, people may drift into old behaviors. Rather than the plan being identified as new, the change agent ensures that it is recognized as the standard of care. Involvement and education are key to successful change. To be effective clear, two-way communication and a concerted effort to garner information and feedback from all affected parties are needed. Education and training also are important components of effective change as it can reduce fear of the unknown and allow the staff to feel prepared and comfortable with taking on new or different responsibilities (Cherry & Jacob, 2016).

Imagine you are a curator at your local city’s Museum of Art. You have been asked to organize a small exhibition of objects of art from the cultures of China, Japan, Africa, and the Americas; you will also showcase Islamic art. Your exhibition should include representative objects that highlight significant and essential ideas of each culture. You may plan to include actual objects in your exhibition, or photographs or models of larger structures or sculptures.

Survey your course textbook and reliable internet websites to locate two art objects from each of the following five cultures: China, Japan, Africa, the Americas, and Islam, that you would like to include in your exhibition. Your objects must date to the time period we are studying in this course (Prehistory -1300 CE). You will select a total of ten objects for display in your “Exhibition of World Art.”

Then, write a “museum label “for each object within a Microsoft Word document. The following should be on the “label” for each object:

1. A photograph of the object
2. Identifying information:
   1. Name of the artist (if known—otherwise, attribute the culture)
   2. Title of the work
   3. Date of the work
   4. Medium/materials used to create the work
   5. Current location
3. A paragraph of 4-5 sentences describing why the object is representative of its culture and is culturally significant. Briefly explain why you have selected the object for display.

Finally, in a paragraph of 8-10 sentences, write a summarizing overview of your “Exhibition of World Art,” highlighting key similarities and distinctions between the objects you have curated.
