     Low income and culture are two of the many risk factors for diseases. Because of the lack of proper education, there is lack of knowledge that is essential to promote and prevent diseases and restore health. Culture plays a role because of traditions, customs and beliefs, the family may not think, act or feel the same way about their overall health.

    The communicable disease chain model is a means of addressing the different transmissions that happens when the agent leaves it host (reservoir – human or animal or the environment such as mosquitos in stagnant water, poor air quality) through a portal exit and enters via transmission through a portal entry to infect a susceptible host. “Th emode of transmission is th emeans of the agent’s travel from the portal of exit to the host” (Green, 2018). This sequence is usually referred to as a chain of infection. If there is a break in the chain of infection, then it is aborted.  For a community that is already at risk because of social determinants of health it is important that diseases are addressed promptly to avoid the widespread infection. For example, in a nursing home where there are less workers and many patients, a staphylococcus bacterium, such as methicillin-resistant Staphylococcus aureus (MRSA) can be passed easily via a source to a susceptible host. In order to avoid this transmission, proper handwashing is necessary before and after patient care to abort or prevent this chain of infection.

Nursing for Women’s Health is the official clinical practice journal of Association of Women’s Health, Obstetric and Neonatal Nurses (AWHONN). The mission of this journal is to “improve outcomes for women and newborns through the dissemination of clinical scholarship” (Association for Women’s Health, Obstetric, and Neonatal Nurses [AWHONN], 2019). This journal focuses on evidence based practice (EBP) research for obstetrics, women and neonates. One type of manuscript that Nursing for Women’s Healthpublishes is original research with a clinical focus (AWHONN, 2019). This category would align with my project. Nursing for Women’s Health is well read among obstetric (OB) nurses, so placing it within this journal would give access to other OB nurses around the country who could glean from my OB triage project.  A health care conference where I might disseminate my project is the “Round Table for OB Nurse Managers” in Minnesota.This is a yearly event where Minnesota OB nurse managers gather and share ideas and processes that are helpful in their respective OB units. This is a great avenue for dissemination as many of the OB managers that come are from rural Minnesota areas like mine and also a good number of these hospitals also use the Excellian electronic medical record platform, so the OB triage order set developed would be easily transferrable.As the project gets implemented and reassessed and evaluated for adjustments, I believe it would be beneficial to other facilities in their OB triage process.  Reference  Association for Women’s Health, Obstetric, and Neonatal Nurses. After reviewing your benchmark evaluation, senior leaders in the organization have asked you to draft a policy change proposal and practice guidelines addressing the benchmark metric for which you advocated action.

In their request, senior leaders have asked for a proposal of not more than 2–4 pages that includes a concise policy description (about one paragraph), practice guidelines, and 3–5 credible references to relevant research, case studies, or best practices that support your analysis and recommendations. You are also expected to be precise, professional, and persuasive in justifying the merit of your proposed actions.

When creating your policy and guidelines it may be helpful to utilize the template that your current care setting or organization uses. Your setting’s risk management or quality department could be a good resource for finding an appropriate template or format. If you are not currently in practice, or your care setting does not have these resources, there are numerous appropriate templates freely available on the Web.

You may also find it useful to have worked through the Problem Identification and Policy Development | Transcript media piece. This activity could be useful in refining your understanding about aligning policy development to address specific problems.

If existing staff is unable to clearly explain the new way of caring for hemorrhage patients, they may instead teach the “old” way. If staff has worked in other labor and delivery units, they may be used to how their old facility cared for postpartum hemorrhage patients, and resort to how they have always done things.  Postpartum hemorrhage only happens in about 2% of births. Though nurses may have a strong desire to continue to provide evidence-based care, they may forget some of the information they learned during the implementation phase.  One way to overcome both concerns is to hold simulation trainings several times per year, and to re-educate as necessary. Additionally, including the training during new hire orientation will help new staff learn about the importance of evidence-based care for postpartum hemorrhage.  Clark and Marks-Maran (2014) discuss the importance of leadership engagement in sustainability.
