Public and Private Behavioral Health Services Private psychiatric hospitals have been in operation way before their public counterparts. Public services developed out of the need to provide services to those individuals who either did not have access to behavioral services or lacked financial resources. “There is a difference in the mission and philosophy of care between a public and a private facility providing behavioral health services.” Do you agree or disagree with the above statement? How? Justify your answers with appropriate research and reasoning. Describe some of the similarities and differences between public and private facilities providing behavioral health services. Support your answer with relevant examples. Review a Psychiatric Hospital In today’s healthcare environment, behavioral health services are provided at both specialized psychiatric facilities as well as within a larger and more comprehensive hospital setting. Psychiatric hospitals are dedicated to the treatment of behavioral health disorders, whereas a general hospital provides an integrated model of mental and physical health. Search the Internet using the key words “division of mental health, developmental disabilities, and addictive diseases.” View the list of regional hospitals provided under the heading Regional Hospitals Listing. Choose any one of the psychiatric hospitals listed and review its organizational structure. Next, search the Internet to find a Web site on a local general services hospital and review its organizational structure. According to your understanding, are the organizational structures of both types of hospitals similar or different? How? Do you think a similar organizational structure should be followed for all hospitals? Why? Justify your answers with appropriate research and reasoning.

Discussion post 2025

Hello Class, When my elderly mother’s health began to fail a few years ago, my sister and I were often at her side at the doctor’s office, at the ED, and eventually, in personal care and then a nursing home. She saw many physicians and had numerous therapies in a distant community in which her health records were neither electronic nor shared. Everyone needed her information and she was not a good historian, so I organized what information we needed for a doctor/ER visits into a huge 3 ring notebook, which I grabbed for every scheduled visit or emergency trip. Many of you may have organized the same kind of records for your family members. How much easier this situation would have been for everyone if we, and her providers, had access to an electronic, personal health record for her. A review of the literature on the consumer’s perspective of the PHR found that consumers are interested in adopting PHRs, which are powerful tools for management of health. Features that consumers want in a PHR include the ease of navigation, explanation of medical terminology, being able to view their medical documents, understand their test results and medications and communicate with their providers (Pushpangadan, and Seckman, 2015). In addition, barriers to the use of the PHR included the digital divide, medical terminology as well as privacy and security concerns. How would a client’s use of a PHR impact your practice? Would you encourage your client’s to initiate a PHR? Response to nursing discussion board-peer repsonses 2025

Response to peer discussion board-150 words-1 reference within 5 years-NURSING JOURNALS ONLY. There is a large push for evidence-based practice to become the main foundation of all clinical practices and clinical decision making. In order to provide the best up to date quality care evidence-based practice must be used. “By the year 2020, 90% of clinical decisions will be supported by accurate, timely, and up-to-date clinical information and will reflect the best available evidence” (Boswell& Cannon, 2017). Though it is ideal to have evidence-base practice driving health care practices there are many obstacles in the way of changing the culture of health care practices. There are many obstacles to change such as resistance to change from staff, available resources to support changes, lack of support and training from mentors, and “research findings incompatible with the realities of their practice” (Johnston, Coole, Feakes, Whitworth, Tyrell, & Hardy, 2016, p. 392). Working at Shands Hospital in Gainesville Florida there is most definitely a strong push for the best quality of care through evidence-based practice. Each unit throughout the hospital has a clinical leader. The unit’s clinical leader’s main job description is to assure the unit is clinically using practices that are evidence-based. The main obstacle that I have found on my unit has been “research findings incompatible with the realities of their practice” (Johnston et al. 2016, p. 392 ). Working on such a high acuity unit in the cardiovascular intensive care unit a lot of evidence-based hospital driven policies and practices have had to be adjusted specially to fit our unit. Hospital wide practices have had to be adapted because certain practices do not apply to the realities and acuity of our unit. One main example is the MEWS assessment. “The modified early warning score (MEWS) is an example of a physiological ‘track and trigger’ system designed to identify patients at risk of clinical deterioration in order to provide a timely response to request appropriately-trained nursing or medical staff to assess the patient and provide any interventions required” (Harris, 2013, p.432). MEWS is an extremely useful tool on floor units, but on my particular unit a majority of our patient population have such a high baseline MEW Score. If following our hospital protocol for our patients’ MEWS we would be constantly calling physicians and bringing them to the bedside. Our patients have extremely high acuity which is one of the reasons they are in the intensive care unit. Our physicians are aware of the acuity of our patients and therefore the hospital wide MEW score does not really apply to our unit. We had to adjust the triggers of the MEW Scores of when to notify a physician. On the floor a MEWS of 4 a physician is notified and needs to come assess the patient, but in the CICU the score for when a physician has to come to bedside was adjusted to a 6. MEWS is just one of the many obstacles present in implementing evidence-based practices on our unit. There are many obstacles to implementing evidence-based practices in our clinical practices, but it is imperative that we adjust or policies and practices to reflect the research found in order to provide the best quality care.

response to nursing DB 2025

Respeonse to nursing DB (Peer). 150 words. 1 reference within 5 years. NURSING JOURNALS ONLY. Change tends to bother many people, both inside and outside the medical profession it can be an issue. At the CVICU I work in at Memorial Regional there is no exception, from changing the equipment like the IV pumps to updating insulin protocols to remain current with new therapies there always seems to be resistance and trouble unifying the team at first. Guo, Hao, Liu, and Zhou (2016) conducted a study on Nurses in several different specialties with varying levels of education and noticed a trend of increased compliance with evidence based practice in those nurses with higher levels of education, experience with research, and job satisfaction. I think this supports the trend of our nursing boards to increase the levels of education required to practice, examples including their promotion of BSN only programs and as you discussed in class this week the transition of the MSN programs to the Doctorate level. The more educated one is and the more experience they have in the viewing the process behind what puts these new treatment modalities in effect then the more respect they have for the research itself and the information it provides. In my unit, specifically there had been an issue one of our respiratory therapist had seen in administering breathing treatments, such as albuterol, to patients without a significant respiratory history. This protocol, was formed by the surgeons and intensivists through their experience and research. The problem was addressed by involving these decision makers to educate the staff on their reasons in a non-confrontational and educational manner which allowed the respiratory therapists a chance to understand and buy into the program This article and experience illustrates that this method of providing education instead of simply implementing new protocols may promote a positive outlook on evidence based decision making. Education and research should be presented to employees and the process explained so it can aid its acceptance and compliance.
