25 yo Russian female evaluated for psychiatric evaluation referred from her retiring practitioner for PTSD, ADHD, Stimulant Use Disorder, in remission. She is currently prescribed fluoxetine 20mg po daily for PTSD, atomoxetine 80mg po daily for ADHD.  

Today, client denied symptoms of depression, denied anergia, anhedonia, amotivation, no anxiety, denied frequent worry, reports feeling restlessness, no reported panic symptoms, no reported obsessive/compulsive behaviors. Client denies active SI/HI ideations, plans or intent. There is no evidence of psychosis or delusional thinking.  Client denied past episodes of hypomania, hyperactivity, erratic/excessive spending, involvement in dangerous activities, self-inflated ego, grandiosity, or promiscuity. Client reports increased irritability and easily frustrated, loses things easily, makes mistakes, hard time focusing and concentrating, affecting her job. Has low frustration tolerance, sleeping 5–6 hrs/24hrs reports nightmares of previous rape, isolates, fearful to go outside, has missed several days of work, appetite decreased. She has somatic concerns with GI upset and headaches. Client denied any current binging/purging behaviors, denied withholding food from self or engaging in anorexic behaviors. No self-mutilation behaviors.

She is a 25 yo Russian female who looks her stated age. She is cooperative with examiner. She is neatly groomed and clean, dressed appropriately. There is mild psychomotor restlessness. Her speech is clear, coherent, normal in volume and tone, has strong cultural accent. Her thought process is ruminative. There is no evidence of looseness of association or flight of ideas. Her mood is anxious, mildly irritable, and her affect appropriate to her mood. She was smiling at times in an appropriate manner. She denies any auditory or visual hallucinations. There is no evidence of any delusional thinking. She denies any current suicidal or homicidal ideation. Cognitively, She is alert and oriented to all spheres. Her recent and remote memory is intact. Her concentration is fair. Her insight is good. 

CLINICAL IMPRESSION

Client is a 25 yo Russian female who presents with history of treatment for PTSD, ADHD, Stimulant use Disorder, in remission. 

Moods are anxious and irritable. She has ongoing reported symptoms of re-experiencing, avoidance, and hyperarousal of her past trauma experiences; ongoing subsyndromal symptoms related to her past ADHD diagnosis and exacerbated by her PTSD diagnosis. She denied vegetative symptoms of depression, no evident mania/hypomania, no psychosis, denied anxiety symptoms. Denied current cravings for drugs/alcohol, exhibits no withdrawal symptoms, has somatic concerns of GI upset and headaches.  

At the time of disposition, the client adamantly denies SI/HI ideations, plans or intent and has the ability to determine right from wrong, and can anticipate the potential consequences of behaviors and actions. She is a low risk for self-harm based on her current clinical presentation and her risk and protective factors. 

DIAGNOSTIC IMPRESSION

Future Plan: monitor for decrease re-experiencing, hyperarousal, and avoidance symptoms; monitor for improved concentration, less mistakes, less forgetful

2) Education: Risks and benefits of medications are discussed including non-treatment. Potential side effects of medications discussed. Verbal informed consent obtained.

Not to drive or operate dangerous machinery if feeling sedated.

Not to stop medication abruptly without discussing with providers.

Discussed risks of mixing  medications with OTC drugs, herbal, alcohol/illegal drugs. Instructed to avoid this practice. Praised and Encouraged ongoing abstinence. Maintain support system, sponsors, and meetings.

Discussed how drugs/ETOH affects mental health, physical health, sleep architecture.

3) Patient was educated about therapy and services of the MHC including emergent care. Referral was sent via email to therapy team for PET treatment.

4) Patient has emergency numbers: Emergency Services 911, the national Crisis Line 800-273-TALK, the MHC Crisis Clinic. Patient was instructed to go to nearest ER or call 911 if they become actively suicidal and/or homicidal.

5) Time allowed for questions and answers provided. Provided supportive listening. Patient appeared to understand discussion and appears to have capacity for decision making via verbal conversation. 

6) RTC in 30 days   

7) Follow up with PCP for GI upset and headaches, reviewed PCP history and physical dated one week ago and include lab results

The definition of the traditional family is “is the parents and offspring living together as a societal or social unit with the same ancestry” (Green, 2018). However, the definition of family that encompasses the different family structures today would be “societal units of single-parent families; lesbian, bisexual, gay, transgender, queer, and questioning (LBGTQ+) families; children raised by grandparents or other relatives; multigenerational units; and people who are together out of friendship and love, not bloodlines. Pets, often part of a household, may acquire the status of “family” members. Thus, family tends to connote a complex, diverse, and dynamic unit, often consisting of parents and children related by legal, genetic, or emotional ties” (Green, 2018). For example, I know some people who are unable to have children of their own and consider their pets as their children. I am sure many people have even heard the term ‘dog-mom’ being used throughout society as well. The family systems theory states that “Family members influence each other and the family as a whole” (Green, 2018), furthermore, each “…individual does not exist in isolation, but is part of a group or family system. Each family member experiences the togetherness, attachment, and intimacy of the family system, but the family member remains an independent thinker with individual purpose-driven actions” (Green, 2018). Whether or not a patient’s family is considered traditional or nontraditional, it is important for nurses to understand the family systems theory when providing and planning care because a patient’s family truly impacts their environment.

I think the definition of family is one that is likely to change per the patient and their individual take on the definition. What a family to me could be and is probably very different than what a family is to one of my patients. Family is either by choice, so the person or people have chosen this as their family and it could range from a best friend to a significant other or a family could be blood, the “biological” part of what makes people “family” and lastly, it could be a mixture of both choice and “biologic” that makes a family. It is important to acknowledge a non-traditional family because like I said before the people in the “family” does not matter but how they interact and the dynamic in which they live is what is more important than the who of the family. “A family systems approach to individuals and their families provides a means to better understand the interactions of a societal family unit ”  and family systems theories help us to understand what we, as nurses, need to look for when assessing and evaulating a family and their health.

This course has brought a new awareness to formulating researchable problems, using a systematic and scholarly approach that has the power and capability to improve patient outcomes. Through the use of credible scientific, evidence-based research, we as nurses can change the future and develop our research skills further to create new evidence-based changes in not only nursing but in healthcare in general. The experience of developing a possible research proposal on a nursing practice issue has brought a whole new understanding and respect for those individuals who conduct this type of research on a daily basis. The process and efforts involved in the development of a research proposal can be tremendous, and the methodical detail and protocol required take a very disciplined person. However, I am glad I was exposed to the process because I may need to conduct a research project in the future, this class has shown me the process and has decreased my anxiety about the research process.

I learned that there are International Human Research guidelines, protecting human participants in 130 countries around the world. This, however, does not protect human participants in every country, allowing those who wish to conduct unethical and inhumane experiments to continue inflicting harm to their fellow man in the name of “research”. It is very important to maintain ethical standards to prevent violations of human rights and the abuses made in the past when conducting research with human beings. In 1979, the Belmont Commission Report defined few ethical principles for the protection on humans in research serving as guideline for future practice (Rebar, Macnee, Gersch, & McCabe, 2011). Currently, in the participating countries, there are over 1000 laws, regulations, and guidelines on human subject protections that are very specific as to the expectations (Health & Human Services, 2018). Education is the key to reducing unethical practice, by learning and practicing all ethical principles.
