1- I enjoyed reading your post. I would also like to add that assessing the teens situation will help in providing the right resources options. Finding resources are a great way for a nurse to suggest help for a teen. It should be geared towards adolescents and providing them with more options to be occupied and mentors.  Teenagers feel more comfortable in peer counseling, than talking to an adult for example having a faith-based outreach could give a foundation that they are missing. Other options, could be youth groups, volunteering, and working with shelters, animals or homeless. This can give the teen a purpose, making them feel needed, busy and important. 

  

2-Communicating with our children is very important, especially during the adolescence stage. Some adolescents struggle to balancing school work, sports, and trying to fit in society. I also believe that educating adolescents about depression/suicide is important. As parents we should make the teenage kids to be very comfortable sharing their concerns with us. We should avoid being over strict. Listen to them when they talk. Give them a chance to make choices and voice their view and even if we disagree, we need to look at things from their perspective. Help them build self-esteem.  Spent time with them but at the same time show respect when they need space.

  

3-Many depressed teens feel like their family, especially parents don’t understand what it is like to be a teen or can relate to current issues teens are faced with today. It could also be that the teen thinks the parents will scold them for engaging in a particular manner or participation in inappropriate behaviors. It depends on what cause the stressors and the relationship this teen has established with the parents. Some teens may be depressed because of bullying, not fitting in, or shamed for participating in the inappropriate activity. It was one participating incident occurred in my community where a young girl hung herself after being caught with a young boy in an improper act at school. It was less than 24 hours, and the young girl was deceased. I don’t know the circumstances, but I can imagine having to face others at school after it was discussed on face book. Think about how you could have encountered your parents after an incident like this. They believe the teens won’t speak with doctors because of confidentiality. They know the doctors are usually in good standings with the parents and already have developed a relationship with them. So they are not very forthcoming with information or seeking help.

Billing Workflow

  • 1. Providers of all types verify patient insurance eligibility with the health plan, either prior to or during the admission or visit. Medical offices collect and post copays at the visit.
  • 2. The patient is treated and discharged or checked out.
  • 3. As you learned in Chapter 9, the provider usually needs to bill a third party, the insurance plan, in order to receive payment. The insurance bill is called a claim. The first step in preparing the claim is to assign procedure codes for the services rendered and the supplies used and diagnosis codes representing the disease or medical condition.
  • 4. Using these codes and the patient registration information, a computer program generates a paper or electronic claim to be sent to the insurance plan.Before the claim is sent to the insurance plan, an insurance or claim specialist reviews the claim to make sure there are no errors. Because of the volume of claims, a computer program is used to examine the claim data and identify problems. Once the claim is correct, it is sent to the insurance plan (usually electronically).
  • 5. When the claim is received by the insurance plan, it is adjudicated. If the claim is correct, a payment is sent to the provider; this is called the remittance. A paper or electronic document is generated that explains the amounts that were paid. This is called the remittance advice or explanation of benefits(EOB).
  • 6. When the remittance is received by the provider, the payment amount is recorded in the patient accounts system. Frequently, the amount billed does not equal the amount paid. This may be the result of a contractual agreement that stipulates that the provider will accept a discounted payment and/or that a portion of the charges is the patient’s obligation. An accounting entry called a write-down adjustment is posted to adjust the charge.
  • 7. If the patient has a secondary insurance plan, a claim is next sent to the second plan. In certain cases the first plan will automatically forward the claim to the second plan. This is called a “piggyback” claim or coordination of benefit (COB) claim. For example, when a Medicare patient has a supplemental insurance policy with the fiscal intermediary who processes the Medicare claims, the company will sometimes process the secondary claim automatically. This eliminates the need for the provider to file a second claim. These are also known as crossover claims.
  • 8. Most health plans require the patient to pay a portion of the medical bill. These payments are referred to as the copay, coinsurance, and deductible amounts. The copay amount is usually stated on the patient’s insurance card and collected during the patient visit. The coinsurance amount is usually a percentage of the allowed amount and is not known until the claim has been adjudicated. The EOB tells the provider what amount is the patient’s responsibility.When all the patient’s insurance plans have responded with remittance advice, a bill or statement is sent to the patient for any amounts due that are the patient’s responsibility. The patient statement should clearly show the amounts paid or denied by the insurance plans, any adjustments to the charges required by the plan contract, and the amount due from the patient.
  • 9. When patient payments are received by the medical office or hospital, they are posted to the patient’s account. When the account balance is zero, no further statements will be sent.

Multiple Sclerosis 

Multiple sclerosis (MS) is thought to affect more than 2.3 million people worldwide. While the disease is not contagious or directly inherited, epidemiologists have identified factors in the distribution of MS around the world that may eventually help determine what causes the disease (nationalmssociety.org, 2018). It is a chronic, progressive, degenerative neurological disease, associated with immune system deregulation, culminating in demyelination and axonal damage within the central nervous system and is one of the most common neurological diseases in young adults and the leading cause of non‐traumatic disability in young and middle‐aged adults (Gold & Wolinsky, 2010).

Pathophysiology of Multiple Sclerosis

MS is a chronic, heterogeneous and complex disease. Autoreactive T‐cells in the periphery are activated by unidentified mechanism, possibly involving molecular mimicry or bystander activation. Activation of these T cells gives them the potential to migrate across the blood–brain barrier (BBB) via an interaction of integrins on the T‐cell surface with adhesion molecules on the endothelium of the BBB, followed by the degradation of the BBB via the secretion of matrix metalloproteases. Once in the central nervous system (CNS), T cells are reactivated on encountering local CNS antigens via an interaction with antigen‐presenting cells such as macrophages or microglia, or B cells. T cells secrete proinflammatory cytokines, and plasma cells secrete antibodies against myelin, leading to the destruction of the myelin sheath. Ongoing inflammation in the CNS promotes the recruitment of additional inflammatory cells. Activated microglia release free radicals, nitric oxide and proteases, which further contribute to tissue damage and axonal loss (Gold & Wolinsky, 2010).

Prescribed Medications to Modify Progression

Beta interferons: these medications are among the most commonly prescribed medications to treat MS. They are injected under the skin or into muscle and can reduce the frequency and severity of relapses.

Ocrelizumab (Ocrevus): humanized immunoglobulin antibody medication which is the only DMT approved by the FDA to treat both the relapse-remitting and primary progressive forms of MS. Clinical trials showed it reduced relapse rate in relapsing disease and slowed worsening of disability in both forms of the disease.

Glatiramer acetate (Copaxone): helps block the immune system’s attack on myelin and must be injected beneath the skin. 

Dimethyl fumarate (Tecfidera): reduces relapses. Side effects may include flushing, diarrhea, nausea and lowered white blood cell count.

Fingolimod (Gilenya):  reduces relapses. 

Teriflunomide (Aubagio):reduces relapses.

Natalizumab (Tysabri): designed to block the movement of potentially damaging immune cells from your bloodstream to your brain and spinal cord. It may be considered a first line treatment for some people with severe MS or as a second line treatment in others. 

Alemtuzumab (Lemtrada):reduce relapses of MS by targeting a protein on the surface of immune cells and depleting white blood cells. This effect can limit potential nerve damage caused by the white blood cells, but it also increases the risk of infections and autoimmune disorders.

Mitoxantrone: immunosuppressant drug can be harmful to the heart and is associated with development of blood cancers. As a result, its use in treating MS is extremely limited, therefore, is usually used only to treat severe, advanced MS.

Treatments for MS Attacks

Corticosteroids (oral prednisone and intravenous methylprednisolone: reduce nerve inflammation. Side effects may include insomnia, increased blood pressure, mood swings and fluid retention.

Plasma exchange (plasmapheresis). The liquid portion of part of your blood (plasma) is removed and separated from your blood cells. The blood cells are then mixed with a protein solution (albumin) and put back into your body. Plasma exchange may be used if your symptoms are new, severe and haven’t responded to steroids.

Controlling Negative Side Effects of Corticosteroids

Insomnia: evaluate if total dose can be taken in the morning. Establish a regular hour for getting into bed and small rituals that help you prepare for sleep. Make sure the bedroom is cool and dark and free of noise. 

Mood Changes: with doses over 30 milligrams per day, steroids can affect your moods. Some people can feel depressed, some extremely “up” and others go up and down for no apparent reason.  Medications can be taken to help with these mood changes.

Fluid Retention and Elevated Blood Pressure: because cortisone is involved in regulating the body’s balance of water, sodium, and other electrolytes, using these drugs can promote fluid retention and sometimes cause or worsen high blood pressure. It is important to maintain a low sodium diet to reduce fluid accumulation in turn controlling blood pressure. If hypertension persists, diuretics may be prescribed (Fields, 2017). 

MS and Genetics

Genetic factors are thought to play a significant role in determining who develops MS.For first-degree relatives of a person with MS, such as children, siblings or non-identical twins, the risk rises to approximately 2.5-5% — with the risk being potentially higher in families that have several family members with the disease.The identical twin of someone with MS (who shares all the same genes) has a 25% chance of developing the disease. If genes were solely responsible for determining who gets MS, an identical twin of someone with MS would have a 100% chance of developing the disease; the fact that the risk is only one in four demonstrates that other factors, including geography, ethnicity and the elusive infectious trigger, are likely involved as well.Research has demonstrated that MS occurs in most ethnic groups, including African-Americans, Asians and Hispanics/Latinos, but is most common amongst Caucasians of northern European ancestry. Susceptibility rates vary among these groups, with recent findings suggesting that African-American women have a higher than previously reported risk of developing MS (nationalmssociety.org, 2018).
