“Culture has a powerful effect on an organization and is recognized by many as the “glue” that holds an organization together and allows it to adapt to changing environments” (Glickman, Bagget, Krubert, Peterson, & Schulman, 2007). An environment that embraces diversity and equality will help assure the standards of nursing to include cultural competency. It is important for leadership and teams to understand the practices and behaviors of other cultures. Lack of cultural competence can be a barrier to effective communication and this can flow down to patients and the families we take care of. Patients are more diverse with the increase of global travel. All levels of the nursing workforce should reflect the diversity of the population the institution serves (Coe, 2013).  “Effective nurse leaders realize that diverse groups are more likely to consider a variety of different viewpoints; therefore, they are more likely to develop better, more creative solutions to problems”

    My institution is located near one of the largest military installations in the United States. We see many patients of different cultures. I also live in Texas and we see a majority of patients that represent the Mexican culture. I would most definitely incorporate someone from the Mexican culture into my team. As a nurse leader I will need to understand the cultural differences among the staff. Having someone from the Mexican culture will help the team understand the language, health practices, beliefs and customs and this will allow for more effective communication with staff, patients and families. There are many dialects spoken in Mexico. They include Spanish, English and indigenous Indian language. Verbal communication is important and understanding that someone of Mexican cultural may stand close and use a loud voice is also important. Older adults do not maintain eye contact. Knowing that this culture may show up late to appointments, it is important to stress the reasons for being on time for health care appointments. Extended family tend to be a good resource for this population (Purnell, 2014).  This person will be able to help provide insight and education to staff. Having cultural diverse team and members will help the group learn and grow and provide safe, effective, quality care to our patients 

 

Rationing Available Capital 

For this assignment, imagine that you are now the Chief Operating Officer of a large hospital in Riyadh. As part of your duties, you are to approve and allocate the necessary funds for the upcoming year’s major capital expenditures. There are two major requests and unfortunately, the hospital simply cannot afford both. The first request comes from facilities management. They have submitted the proposals and costing information for a major five-story parking structure. Complaints about limited parking arise from staff and patients on a daily basis. You ponder the hospital’s desire to increase services and realize that this is certainly worth considering. However, on an equal level of need, the medical department has submitted a request for an upgraded and expanded operating room including the addition of robotic surgical assisting devices (DaVinci).

In this proposal to the Board of Directors, analyze the steps you would use in determining which one will be funded this year and which one will be funded next year. Will you use surveys, if so will they be available to the public, employees only, or both? Will you favor the parking facility as it will generate income or will you favor the increase of technology as it lends itself well to marketing? Who will you bring into the decision-making process? As you endeavor in this assignment, the thought process is more critical than choice as there is no right or wrong answer.

Your proposal should include the following components:

1. A one-page discussion identifying the process of capital investment decision making, using citation and supporting reference articles.
2. A two-page proposal including steps in determining the funding:

• Assessment (survey, target audience, etc.).
• Compare and contrast both options.
• Final proposed decision with supporting spreadsheet.

Your paper should meet the following structural requirements:

1. Three pages (not including title or reference page)(MS file)
2. Excel spreadsheet which supports your position .(Excel sheet file )
3. Formatted according to APA  writing standards.
4. Provide support for your statements with in-text citations from a minimum of four scholarly articles. Two of these sources may be from the class readings, textbook, or lectures, but two must be external.

6. The references should not be more than 5 years back

For our discussion board this week, we will discuss a neurological condition known as Cluster Headache.

Case Study:

Richard is a 40-year-old man with a history of 4 weeks of cluster headache once each year. These began when he was 35 years old. His cluster periods occur in the fall. The cluster period begins slowly, increasing over 1 week, reaching a peak where Richard has two or three severe cluster attacks each day. They occur from 10 p.m. to 3 a.m. Each cluster headache lasts from 40 to 90 minutes, and the headaches are severe. The pain is always on the right side, with eye tearing and nasal congestion. 

Richard comes into our office 1 week into this fall’s cluster series. The headaches are increasing in intensity, and he is miserable with the pain.

Please complete the following questions: 

• 1. Describe Cluster Headache and its epidemiology.
• 2. What would be your goals for therapy for Richard? Give rationale with evidence from articles.
