There are many strategies that I can use to create change in my current workplace. One of them is to develop alliances and social support systems that can legitimize, nurture, and stimulate related change-actions. Such alliances and social support will focus on healthcare institutions that legitimize and foster healthy life-style in work setting. In this perspective, I will use the power of nursing organizations and establish a broad coalitions to maximize workplace protections for nursing staff, patients, and other stakeholders. Nursing organizations represent an influencing force that can generate demand and pressure for health-supportive environment and policies that benefit healthcare staff and patients.

            The second strategy I will use is empowerment. That is, I will use the arm of empowerment to equip co-workers and others with the knowledge, values and skills that inspire real action for grow.  

The third strategy has to do with the change for transforming the culture. This approach calls for personal involvement. Consider yourself as the principal driven force that leads to change.  That is, imagine how you can play a dynamic, an active role in altering your culture. By embracing this mind-set, ‘when you put skin in the game, you become the change that transforms the culture’

Zolierek (2012), defined advocacy as using “one’s position to support, protect, or speak out for the rights and interests of another”.  As nurses, it is in our pledge and scope of practice to advocate and protect the safety and rights of all of our patients. A change in the workplace culture would increase patient safety and patient satisfaction. Healthcare is changing and we must be able to adapt and change along with facing the challenges that come with change. To make a change, we need to create an environment that will infuse energy and commitment to the team and the whole organization. The first step in advocating for the transforming the culture is actually being the change. We must each understand the power, influence, and the ability to see and understand the culture in which we each work and how we can play a role in inspiring and transforming the culture. The second step would be to embrace the opportunity. This would be stepping out of our “comfort zone” and not letting fear or embarrassment confine us but we step out with excitement. We should provide positive effects to each of the individuals we are influencing. Be a role model and mentor. A third step is being a team player. Allow opportunities for feed-back or one-on-one conversations. We should create an environment for our team to have to room to learn, grow, and be encouraged by new ideas.

Advocacy is an important part of the health car field. Advocacy is the act of supporting a cuase. There are many skills and strategies that be utilized to be a successful advocate at home and in the workplace. A specific skill that needs to be used is communication. By communicating, you will be able to bring individuals together to address any issues or concerns that anybody may have. Communication does not just have to be verbal, it can also involve electronic communication as well. With communicatin, you will want to make sure that all informatin is factual. Another skill that is important is collaboration. By collaborating, you will be forming a relationship with co-workers and peers. By collaborating, it enables you to focus on one particular problem and to achieve a common goal. Another effective way to create change in the workplace and be a successful advocate is to continue education. By continuing education, it allows you to be an advocate for your patients. It also allows you to familiarize yourself with new treatments and enables you to properly treat and educate your patients.

Knowing scientific management theory as defined as supervising by way of focusing on task accomplishments rather than interpersonal relationships (Huber 2014). Some routines in health care that seem to be inefficient in regards to scientific management are the overwhelming focus on tasks, the evaluations based on tasks accomplished, and the pressure to join committees to add more tasks to nurses’ duties. A lot of times nurses have trouble tending to the needs of their patients because they are so focused on getting the tasks done so as to not get in trouble. Many times tasks are added to the nurses’ workload making it hard to complete everything on time. This causes incidental overtime which is also greatly frowned upon. Finally, the committees are supposed to be a good thing, allowing nurses to participate in shared governance. The issue arises when the requirements for the committees capitalize on the time the nurse should be working or decompressing from the stress of daily work. Some examples of participative decision-making at the hospital that I work at are unit forums and shared governance council. The unit forums allow for nurses and aides in the unit to meet and discuss issues to resolve amongst themselves. Shared governance councils allow for nurses to meet with administration to discuss issues at the bedside that can only be fixed by administration.

The scientific management theory states that humans are motivated by money, and there should be a separation between managers and workers. The expected result is a dramatic increase in productivity of the workers and profits for the organization. Some of the routines in the health care setting that are inefficient is nurse-to-patient ratio and staff retention. I feel the nurse-to-patient ratios are unsafe as it overloads the nurse and nursing assistants in carrying for high acuity patients with a higher prevalence of medical errors and oversight. When staffing is is done according to census instead of census it creates an unsafe environment for the patients and the staff. Without adequate staffing, patient care is inadequate and the patients suffer. Which in turns causes patient surveys to report negative feedback giving the hospital low numbers.Staff retention is important to ensure there is adequate staff to meet the demands of an aging population with numerous chronic illnesses. When facilities ignore the concerns of staff or do not provide attempt to implement change that benefits the staff and the patients, morale falls and staff leave. When nurses feel their safety and license are in jeopardy, they will seek out a better work environment they feel safe in. The facility I work for currently does annual retention surveys that allow its employees to voice any concerns and share what they feel the facility is doing well or could improve on. The nurses are asked various questions in regards to the leaders and managers they work directly under and whether their needs are being addressed. The facility offers committees that focus on allowing staff to have voice. Some include unit base council for each unit of the facility, committee for staffing concerns and safety committees that focus on sentinel events.

The clinician must be knowledgeable about how these medications work so that safe and effective treatment can be given to the client. According to Stahl (2013), thirty percent of psychotropic drugs impact the G-protein linked cascade, and twenty percent of psychotropic drugs target the ligand-gated ion channel. Understanding these systems give the practitioner clarity on how these drugs can be useful.

Agonist to Antagonist Spectrum

            Receptors are specialized proteins found in the cells or on the surface of neurons. (Nutt et al., 2007). Post-synaptic receptors generally regulate the actions of the released neurotransmitter (Nutt et al., 2007). Drugs may be referred to as agonists, partial agonists, antagonists, or inverse agonists, which means that they affect the level of neurotransmission at the post-synaptic neuron (Stahl, 2013). An agonist imitates the action of endogenous neurotransmitters.  Agonists may directly bind to the neurotransmitter site (which may cause a change in the shape of the receptor) or may indirectly cause an increase in the level of neurotransmitters released. A partial agonist also causes an increase in neurotransmission effects but does not have as high and effect as a full agonist would. An antagonist, on the other hand, may either block normal neurotransmission or cause a reversal in the shape of the receptor so that the effects are opposite to the agonist (Stahl, 2013). For example, in schizophrenia, it is believed that overactive or high levels of dopamine contribute to the illness; hence a D2 antagonist (such as Haldol) may be given to inhibit dopamine release (Guzman, 2019). An agonist will increase neurotransmitter signals or levels by a significant amount; partial agonist with also increase levels but not to the extent of the full agonist, while an antagonist will inhibit signals or reverse what an agonist has done.

G couple proteins and Ion Gated channels

            As stated earlier, G couple proteins and Ion gated channels are molecular targets for psychotropic drugs. Both G couple proteins and Ion- gated channels are activated by neurotransmitters which then activate genes in the cell nucleus (Stahl, 2013). G couple protein works have a cascade involving cAMP (cyclic adenosine monophosphate) and protein kinase A. On the other hand, the ion gated channel works through calcium, and it activates different kinase called calcium-dependent protein kinase (Stahl, 2013). Both systems affect gene expression; however they do so at via separate systems.

Epigenetics

            Neurotransmission seeks to modify the cell function of gene expression, meaning, turning some genes on and others off. When the cell nucleus changes, it is because the structure of chromatin is modified; hence, gene expression is modified. Epigenetics speaks to changes in the genes that lead to long term changes in how the gene is expressed. This is important in pharmacology as some illnesses such as cancer may have several mutations and, as such, may be challenging to treat (Stefanksa and MacEwan, 2015). However, if the changes in the genes that lead to cancer are studied, then drugs that target those genetic changes can be developed, hence, solving the root of the problem rather than trying to eradicate cancer. Simply put, change gene expression so that it does not lead to cancer (Stefanksa and Mac Ewan, 2015).

How It Impacts the Way Medications are Prescribed

As previously mentioned, prescribers/ clinicians need to know this information for safe, effective, and efficient treatment. Understanding the neuroscience can reduce prescriber error. Stahl (2013) discusses the fact that twenty percent of Asians are poor metabolizers of the Cytochrome CYP 2C19. Hence, the practitioner needs to know that psychotropic drugs metabolized by this enzyme need to be closely monitored as the drug can quickly build up in the patient’s bloodstream. Watching dosing strengths, as well as other medications that may further inhibit this enzyme, could cause an increase in drug levels in the body.
