Urinary tract infections (UTIs) occur when bacteria invade the normally sterile urinary system, causing inflammation to the epithelium. If the infection is found in the urethra and bladder, it is considered a lower urinary tract infection. When the infection goes above the level of the bladder and into the ureters and kidney, it is considered an upper urinary tract infection or pyelonephritis (Huether & McCance, 2017). Risk factors for UTIs include female gender, diabetes, sexual intercourse, urinary incontinence and/or retention, and immobility (Arcangelo, Peterson, Wilbur, & Reinhold, 2017).

            Lower urinary tract infections are most commonly caused by intestinal microbes like E. Coli. These pathogens bind to the urinary epithelium and create an inflammatory response. This response created edema and stretching of the bladder wall, which signals the stretch receptors to relay the feeling of fullness. This created the urgency, frequency, dysuria, and low back pain that are commonly felt. If left untreated, the epithelial wall will become more and more damaged, eventually leading to bleeding, ulcerations, and even gangrene (Huether & McCance, 2017).

            Upper urinary tract infections, or pyelonephritis, can be caused by the pathogen climbing and spreading into the upper part of the urinary system. Unlike lower urinary infections, upper urinary infections can also be caused by contaminated blood flowing through the kidneys. If left untreated, abscesses can form in the adrenal medulla and cortex. Back or flank pain and costovertebral tenderness are symptoms that are more unique to the upper urinary infection and can help differentiate along with diagnostic testing. Fibrosis formation in the tubules can form after the initial infection (Huether & McCance, 2017).

            Females are far more likely to suffer from urinary tract infections because of the shorter urethra and closer proximity to the rectum (Huether & McCance, 2017). Age is also a factor, as immobility, dehydration, and urinary incontinence increase the risk of developing UTIs. A UTI in an older adult can have different symptoms and manifestations, such as delirium, hallucinations, sudden decrease in mobility, and subsequent falls. These symptoms can be dangerous and may cause significant injury (Armstrong, 2015).

            Along with antibiotics, urinary analgesics can help with the discomfort associated with urinary tract infections. Urised is a common urinary analgesic that is prescribed which often has anticholinergic side effects. Patients should be educated on the chance for urinary and/or bowel retention and when to seek treatment (Arcangelo, Peterson, Wilbur, & Reinhold, 2017). Encouraging fluid and fiber intake may help offset these side effects.

For both patients and health professionals to ensure quality health services, they need to consider cultural barriers. Evidence shows that there is a connection between poor cultural competence and low-quality health outcomes in health care practice. In this case, the client is a teenage girl who is admitted to the hospital with fevers (Gopalkrishnan, 2018).When dealing with teenagers, one should consider the legal requirements to maintain confidentiality, obtain consent, and language assistance services especially if the client is not a proficient English speaker. When interviewing her, the nurse should also consider her level of openness, special distance, taboo subjects, and eye contact.

Dealing with a female teenager of the Hindu faith requires one to have adequate knowledge of their beliefs and faith. According to the Hindu religion, all individuals are free to practice their religion the way they see fit. Besides, Hindus have a strong belief in karma, which is the law of cause and effect. They believe that each action, thought, and word makes up karma and affects their future and current lives. The nurse should consider this fact when interviewing the young girl as it may affect the decision-making process (Zavos, 2015). Also, Hindus believe that serious illnesses and other occurrences in their lives balance their lives and help them become better people. Hindus embrace the use of western medicine among many other options for treatments.

The United States provides health resources that help in effective communication among health care providers and Indian patients, which include the ward communication tool, language identification card, among others. Also, some Hindus may prefer fasting while in hospital, which may affect their wellness (Zavos, 2015). The nurse should ensure that the teenager is aware of the dangers of fasting when sick, while still considering the requirements of the Hindu religion.

Scenario: Admitting a 19-year-old female college student for fevers. Patient has immigrated from Russia to complete her education.

Considerations

When interviewing this patient, it is okay to shake her hand and make direct eye contact. As healthcare is not very accessible in Russia, it is safe to assume the patient has not had much experience being in a hospital, so it is important to make her feel comfortable. Explain your role thoroughly prior to conducting the interview. When obtaining a history of medication, be sure to ask about homeopathic remedies the patient might have used. When asking about mental health history, avoid using the word “mental” as it is taboo for this culture (Stratis Health, 2020). Instead, words such as depression or anxiety can be replaced. Russians are very hesistant to admit mental health problems or familial history of mental health problems. When assessing patient, always explain what you are going to do prior to doing it and why. Russian culture is very dependent on the family unit, so when delivering diagnoses, it may be wise to ask the patient if she has any family with her. Ask her who she would like to be involved in her medical decisions. Russians believe that if bad medical news is shared with the patient, it will only make them sicker (University of Washington Medical Center, 2007). It is important to know what your patient believes and if she wants the family to make decisions for her or if she wants to make them for herself. When considering the cause of the illness, take into consideration the lack of access to healthcare in Russia. Russians are not as likely to be vaccinated as U.S. citizens, so it is important to know the patient’s vaccination status. There is also a higher prevalence of TB and HIV in Russia. At this age, it is important to assess patient’s sexual activities. When assessing the patient’s sexual history, do so privately. Also, stress the importance of honesty in her answers. Patient may feel more comfortable with a nurse of the same sex, but if a male nurse were assigned to her, he can just ask her if she is okay with him taking care of her. The author was unable to find healthcare support systems that are specific to the Russian culture that are targeted for this patient, both locally and nationally.

Background and Physical Examination

J. P.  is a 46-year-old man presents to the emergency department with a 5-day history of progressively worsening breathlessness on exertion and mild, general flulike symptoms. He also complains about night sweats and an intermittent low-grade fever, both of which started about 2 weeks ago.

Upon physical examination, the patient does not appear to be in any acute distress. His vital signs are measured as a pulse of 89 beats/min, blood pressure of 140/85 mm Hg, and a respiratory rate of 19 breaths/min. He is afebrile, with a temperature of 99.8°F (37.7°C).

The chest examination reveals nothing out of the ordinary, and his cardiovascular and respiratory examinations, including auscultation, are unremarkable. The abdominal examination reveals no fluid thrill, shifting dullness, or bruit. The liver and the spleen are not enlarged. No lymph nodes are palpable.

Multiple areas of hyperpigmentation are noted; otherwise, the skin inspection is unremarkable, with no hematomas, bruises, wounds, or scars noted. Electrocardiography (ECG) reveals a sinus rhythm with a heart rate of 84 beats/min, and the T waves are somewhat flattened in leads V1, aVL, and III, but they are otherwise unremarkable.

Laboratory Test Results and Other Findings

Significant laboratory findings include a white blood cell count of 9.1 x 103/µL (9.1 x 109/L; reference range, 3.5-8.8 x 109/L), a platelet count of 429 x 103/μL (429 x 109/L; reference range, 140-350 109/L), a C-reactive protein level of 91 mg/L (reference range, < 10 mg/L), a lactate dehydrogenase (LDH) level of 4.7 microkatals (µkat)/L (reference range, < 3.5 µkat/L), an erythrocyte sedimentation rate (ESR) of 30 mm (reference range, 1-12 mm), and a D-dimer of 2.2 mg/L (reference range, < 0.25 mg/L).

A spiral CT scan is performed, which shows no pulmonary embolism. You work as a technical support manager for a satellite television company. As part of a company-sponsored tuition reimbursement program, you are also attending college courses part time and pursuing a degree in communications. Your Human Resources representative asks you to give a presentation to a group of people at an upcoming career fair. The HR manager wants you to motivate potential applicants to apply for part-time positions that pay a relatively low hourly wage and include only limited benefits. This is exactly the type of position you accepted when you first started with the company. The job fair will be held in a part of the city with a relatively low unemployment rate. The people attending are likely to come from middle- to upper middle-class families. The people who attend the career fair from upper middle-class families are used to getting what they want without too much effort; therefore, they may not be able to relate directly to the persistence you showed to work your way from a part-time, low-paying position into a higher-paying one that includes benefits. The ones from middle-class families may relate more to your personal story. You can’t gather all the characteristics of all the career fair attendees in advance, but you do know that the way in which you explain the advantages of working for the satellite television company and the type of language you use will affect the attendees’ decision on whether or not to apply for a position with the company.
