Roussel, Thomas, and Harris (2017) report that a microsystem analysis includes five P’s of purpose, populations, processes, patterns, and providers. A comprehensive analysis using a standardized approach to high light opportunities for improvement. Our purpose is to find the cause of the increase in malpractice claims and improve patient outcomes. The population is patients with operative procedures using Davinci robotics. The process is the Davinci robotic procedure. Next, patterns to narrow the search to understand cause and the providers are the nursing staff working in the OR. Jones, Polancich, Steaban, Feistritzer, and Poe believe quality and patient safety requires efforts from all nursing leadership. The nurse executive (CNO) is paramount to the team and drives the vision of nursing to deliver safe, quality, efficient, patient-centered care (2017, p.186). The leadership skills of CNO are necessary to achieve high-quality patient care and excellent clinical outcomes (Jones, Polancich, Steaban, Feistritzer, & Poe). The CNO establishes the direction and identifies goals to provide the foundation for strategic planning.

The first action as a leadership team is to determine if all the claims revolve around the same type of issues by gathering data on the claims. I will meet with the quality team and nursing leaders to identify the issues. Once we study the data then we will determine how to proceed. Does it appear to be a process issue or a person issue? Is there a lack of knowledge? From the data, we find a trend with procedures done by Davinci robotic surgeries. The clinical nurse leader will observe a Davinci surgery to examine the workflow process to determine areas of opportunity. The process is mapped out in detail to determine areas of concern. It was found that Davinci procedures are now being performed with new equipment. The data is further examined to determine a pattern. Is the data related to a process, structural, or outcomes-based to decide what needs to be improved? We must decide if there is a flaw in the procedure or equipment. Finally, we look at the staff and their experience, education, and familiarity with the procedure and equipment. Is there a knowledge-based error? We know from our previous reports that staffing has been a concern and there are many new RNs working in the OR. We look at the roles of all staff associated with the procedure including the providers. The data shows that many newer nurses may be unfamiliar with the use of the new equipment, so we have an area of opportunity to educate and train the staff. The education plan, communication plan, and timeline will be developed and shared. This communication will explain the plans for the change and allow for additional input if necessary. Physicians and key stakeholders will also need to be informed about the change in advance of implementation. This gives time for additional training of physicians and ancillary staff. Listening to the feedback from teams is important. As the changes are implemented, staff will have feedback on what is working and what is not so daily huddles will be needed with staff. Feedback will be monitored, and changes made as needed to improve patient outcomes. Effective communication and enhancing team culture are especially important in strategic planning. As part of our commitment to our community, we want to provide safe high-quality patient-centered care so we will strive to decrease malpractice claims.

The purpose of mandated reporting is to identify suspected abused and neglected children as soon as possible to be protected from further harm. According to VDSS (2021), Virginia law for mandated reporter states that anyone can report suspected child abuse or neglect. Under Virginia law, certain professionals are required to report child abuse when acting in a professional capacity. But if you are identified in the Code of Virginia as a mandated reporter or you have received training in recognizing and reporting suspected child abuse and neglect, you are required by law to immediately notify your concerns to the local department of social services or the Child Abuse and Neglect Hotline.

The steps in reporting a mandated reported issue are; when you suspect that a child is being abused or neglected, you should immediately report your concerns to the local department of social services in your community. When making a report, it helps if provided as much information as possible; mentioning the victim’s name, age, sex, type of abuse, and address can also include your contact information, which will aid in carrying out an investigation. Reports can also be made to the Child Abuse and Neglect Hotline (1-800-552-7096), seven days a week, 24 hours a day. Section 63.2-1512 of the Code of Virginia protects a person who either makes a CPS report or participates in a court hearing that results from a CPS report from criminal and civil liability unless proven that the person acted with malicious intent.

A couple brought their seven-year-old boy with multiple injuries on the back, and visible scars on thighs come to ER. On assessment patient appeared to quiet and terrified; parents refused whole-body skin check and had doubtful answers about the occurrence of injury. The assigned Nurse identified it as child abuse and reported it to the provider and head nurse. Then management team in ER reported the abuse to the child abuse hotline. The child was taken for further assessment and parents for questioning for possible child abuse.

The NCSBN also incorporates a policy for nurses to manage professional boundaries, ethics, and compliance with state and federal law when engaging in social media platforms. The obligation to protect patient privacy and confidentiality extends to all environments, including the health facility and social media platform. For example, in Texas, when a nurse is accused of breaching and disclosing a patient’s privacy, any nurse within the department must report the matter to Texas BON. 

The breaching of trust involving identifying patients by names on social media platforms attracts the board action immediately. The 22 Tex. Admin. Code 5 217.11(1) (E), (K) on BON regulation laws in Texas provides a legal system platform on which the state board can pursue individual nurse contribution to disclosure of patients’ information without owner consent. In code 15.29 of Texas BON, the standards statements provision address professional boundaries, including social media use by nurses. The guidance from definitive statements prevents boundary professional violation, as depicted in provisions of 22 Tex. Admin Code 5 217.11(1) (E), (J). This Admin policy requires evidence tabling for communication on patients’ issues in a reproachful manner. Failure for nurses’ colleagues or individuals to share mistrust breach incidents by other nurses with Texas BON can result in disciplinary action for cover-up and firing of the incident’s perpetrators. 

Social media platforms have saturated various industries, including nursing, where nurses can connect to personal and professional lives. The significant facilitated conversations include sharing ideas with colleagues on health care advancement measures and best current practices in healthcare. Incidents of nurses’ harmful use of social media platforms have been on the rise through breaking patient confidentiality and privacy (Casella et al., 2014). The profound impact of break-in confidentiality leads to a breach of trust leading to a long-term rift between nurses and patients.

The 3Cs Model of Test Item Writing:

  • Content, concepts, and criterion. Use the content of the lesson as well as overarching concepts and nursing criterion (like NCLEX and other formal assessments) as a blueprint for your questions. In addition, consider the ideas and teachable moments that have occurred in the classroom or during clinical rotations.
  • Clarity and complexity. Be sure that your question stems are clear, straightforward, and use simple language. In addition, write your questions at the application or analysis level of Bloom’s Taxonomy. This will prompt students to move beyond basic comprehension and memorization.
  • Critique. Always go back and examine the questions that you have created. This process should be completed both before and after examinations. As with all teaching strategies, evaluation and revision is imperative to ensure the effectiveness of instruction.
