 Discussion – Dialysis Safety

Hemodialysis (also known as “dialysis”) is a common treatment for patients with kidney disease. In the United States, more than 468,000 patients are on dialysis (NIH, 2018). Since 1993, hospitalization rates related to the process of dialysis has risen between 47-87% (CDC, 2018).

Many patients seek dialysis care at outpatient facilities that are not inspected as frequently as other healthcare facilities, like nursing homes and hospitals. For example, in California, kidney dialysis clinics are only inspected on average once every six years.

In your post, address the following questions:

• Should kidney dialysis centers be held to the same safety standards as other healthcare facilities?
• Are current regulations for dialysis centers strong enough?
• Should more frequent inspections be required?
• Are there other ways to reduce the risk of infection for dialysis patients?

Discussion 2         

Betty, the chief nursing officer, had to make a decision about buying 120 new hospital beds for patient rooms. After she interviewed nurse mangers at the units where the beds were going to be placed, Betty compiled her findings and decided to contact a well-known equipment company to obtain prices and contracts. The equipment company’s executive salesperson, Jim, discussed options at length with her and invited her and her significant other to an upcoming all-expenses-paid lavish retreat at a five-star hotel in Hawaii to see demonstrations of the beds and to hear a comprehensive sales pitch. Betty thought to herself, “We badly need some relaxation and stress relief. Hawaii would be so much fun. Would it be wrong for us to go?”

• If you were Betty, what would you do? Give your rationale. Justify your answer with an ethical framework—a theory, approach, or principle.
• Do you consider this situation a conflict of interest? Why or why not? Give your rationale.
• What policies, if any, should be in place regarding a scenario such as this one?  Do you have any such policies in place at work for similar situations? Do such policies impact day-to-day activities in any way?  Explain.

Patient Scenario

35-year-old man with a history of anxiety, chronic marijuana use, and chronic back pain. No current medications or other history, but he has been consistently complaining of back pain since joining the Thursday night league volleyball team. Single, lives alone, and works at a fast food restaurant. Heterosexual and consistent use of protection (barrier).

· Offer three differential diagnoses for what might be troubling your patient.

· Provide rationale with citations for each of your differential diagnoses.

· What would be your first course of action for your patient?

· What safety issues, if any, did you recognize for your patient?

· Summarize the scenario and what you chose as your final diagnosis for your patient and why.

· Make sure that you address psychosocial issues/concerns/contributors, along with content related to substance abuse, should this be appropriate.

For example, if one paragraph accounting for only 10% of a paper is cut and pasted, the paper could be returned for revision, despite the low SI score.
