MEDICAL MYSTERY FOR CHAPTER 3: MARY

Mary is a married woman with three grown children. She has just quit her job as a receptionist because of poor health. For the last few years, she has found it very difficult to walk up the stairs at work. She also noticed that her right knee was quite painful and swollen at times. The daily trip to work on the subway, negotiating the bustling crowds, was too much for her knee. At home, she noticed that her knee felt okay when she did not walk much. However, at night she continued to feel some pain in her right knee and often required pain medications in order to sleep. She tried several home remedies, including applying garlic paste, warm compresses, and even Chinese herbs on the knee. As time went on, she became more disabled. Every time she walked, she felt that her knee might give out. She tried to use a cane, but the pain was still unbearable at times. She wanted desperately to lose some weight, yet she couldn’t really exercise. Her sister told her that she should try some calcium and vitamin supplements to strengthen her bones.

THE EVALUATION

Mary’s husband finally convinced her to come to my office. She was always afraid of doctors and had been very reluctant to come. She was also fearful that she might have rheumatoid arthritis, which had crippled her best friend Sadie. But she decided that the pain was just too much and couldn’t put off a visit to the doctor any longer. I examined Mary and found that her right knee was swollen and painful to the touch. It also seemed very unstable. I ordered some x-rays and prescribed pain pills. I gave Mary a referral to an orthopedic surgeon and suggested that she use a walker in the meantime to prevent a fall.

THE DIAGNOSIS

After reviewing the x-rays of her knee, the orthopedic surgeon told Mary that she has developed osteoarthritis of the knee. He explained to her that this was basically a degenerative process and the fluid in between the joints had completely disappeared. The friction between the two joints was causing the pain and destroying her joint. She was very relieved when he told her that her other knee was fine.

THE TREATMENT

The surgeon confirmed that Mary had developed severe degenerative arthritis of her right knee and recommended total knee arthroplasty as the best option for her. One month later, she had the surgery, and she remained in the hospital for 3 days before being discharged to a rehabilitation center for 3 weeks of physical therapy. She also worked with a dietitian while in rehab. Mary’s postoperative course was unremarkable, and she was told that she should continue to watch her diet. It was also highly recommended that she maintain a regular regimen of physical activity.

CASE CLOSED

Mary came back into my office 6 months after her surgery and was walking unaided. She said that the recovery from the knee replacement had been a slow and sometimes painful process, but that she was now able to get around well enough to resume working part time. She looked much happier and healthier than when we first saw her, and she thanked our staff for making her feel at ease during her first appointment.

Assessment: Physical examination is unremarkable. Her BP is 128/68 and her pulse is 74. Her urine human chorionic gonadotropin (HCG) is positive. Her potassium is 4.2, blood  urea nitrogen (BUN) is 14, and creatinine is 0.6. Alanine aminotransferase (ALT) is 29. White blood cells (WBCs) are 6.5, hemoglobin (Hgb) is 12.8, hematocrit (Hct) is 39, and platelets are 330,000.

Dr. Johnstone didn’t like what he saw.  The x-rays of Gladys Gwynn’s hip showed a fracture of the femoral neck and severe osteoporosis of the hip.  Mrs. Gwynn had been admitted to the orthopedic ward of Hamilton Hospital after a fall that morning at Sunny Meadows, an assisted-living facility.  The accident had occurred when Sheri Smith, a new aide, lost her grip while helping Mrs. Gwynn in the shower.

A frail but alert and cheerful woman of 85, Mrs. Gwynn has osteoarthritis and osteoporosis that have forced her to rely on a walker.  Although her finances were limited, she has been living at Sunny Meadows since her husband’s death four years ago.  Dr. Johnstone knew that she didn’t have any close relatives, and he did not think that she had signed a Health Care Power of Attorney designating someone to help with medical decisions like this.

A total hip replacement would be the logical treatment for a younger patient because it could restore some of her lost mobility.  However, for a frail patient like Mrs. Gwynn, internal fixation of the fracture might be the treatment of choice.  This would repair the break but not improve her mobility.

Dr. Johnstone needs to make a decision soon, but he knows that Mrs. Gwynn is groggy from pain medication.  With one more look at the x-ray, Dr. Johnstone sighs and walks toward Mrs. Gwynn’s room.

Because of the pain medication, Gladys Gwynn may not be able to speak for herself.  Since she has no relatives to help, is it appropriate for Dr. Johnstone to make the decision about surgery for her?  Given these circumstances, is it possible that when Gladys moved into Sunny Meadows they had her sign a Health Care Power of Attorney to someone in the facility?

In labor & delivery we have recently adopted a research-based or evidence-based practice, which is the use of the Peanut Ball during labor. It is a tool used for promoting the progress of labor, by facilitating normal fetal descent (bringing the baby down) for an easier delivery, and to prevent the patient from having to have a primary C-section. Failure to progress during labor is the most common indication for a first time C-section (Tussey, et al., 2015). Before last year, my hospital had let the mother labor down on their own without any assistance, other than letting the patient reposition themselves in bed as needed. Needless to say the hospital primary C-section rate was high and needed to be lowered. According to, Reducing Length of Labor and Cesarean Surgery Rate Using a Peanut Ball for Women Laboring With an Epidural 2015, “The ball promotes spinal flexion, increasing the uterospinal angle, and increasing the pelvic diameters to facilitate occiput posterior rotation, which results in a widened pelvic outlet.” Now, once the patient comes in and it has been established that she is in labor, we utilize the peanut ball on all patients by placing it between their legs to keep the pelvis open and rotating the patient from side to side to help shorten the length of labor and to help decease the primary C-section rate.

Evidence-based practice is utilized to promote quality, cost-effective outcomes for patients as well as the healthcare professionals and healthcare system (Burns & Grove, 2011). Evidence-based practice is the action of implementing best evidence-based care, supported by research, and integrating the care with patient’s needs and values as well as clinical expertise (Burns & Grove, 2011). Nurses utilizing evidence-based care began as the profession began. Florence Nightingale realized that cleanliness to air, water and diet promoted health to her wounded soldiers and aided in their recovery (Burns & Grove, 2011).  Examples of evidence-based practice include better management of patient’s blood pressure on a primary care level to better avoid heart attack, stroke and kidney disease, as well as understanding risks factors for diabetes leading to earlier screening for patients who possess the risk factors, or the appropriate guidelines for IM medication administration to promote effectiveness of medication delivery and patient’s comfort (Burns & Grove, 2011). In Singapore, 64% nurses expressed positive attitude towards evidence based practice and bettering their patient’s outcomes, however also expressed concern on their already heavy workload while attempting to keep up with data and new evidence (Majid & et. al, 2011). The main barriers in implementing evidence-based care to this facility in Singapore was found to be lack of time, lack of ability to understand the statistical data as well as jargon used in the research articles (Majid & et. al, 2011).
