An organizational structure identifies the roles and responsibilities of those in leadership positions that will impact the goal of the organization and (Renani, Ghaderi & Mahmoudi, 2017). It also indicates how information is shared through the various levels of leadership (Marquis & Huston, 2017). It identifies the hierarchical leadership structure of an organization for accountability ( Renani et al., 2017).

SLMC is a healthcare organization that has cared for a number of its neighboring communities in the Chamberlain County of South Carolina for the last 50 years. The organization consists of 500 physicians and 450 nurses and as such needs an organized way to function where roles are defined so the goals of the organization can be met. The purpose of the organization is to improve quality care and safety and to make improvements in healthcare and to provide care of the surrounding communities.

As the nurse administrator, the organizational structure that would be best for St. Louis Medical Center (SLMC) would be the shared governance. Shared Governance in healthcare is an essential component of providing quality and safe care to patients (Marquis & Huston, 2017). Based on the structure, the decision-making process is in collaboration with board members, physicians, the leadership team, and nurses (Marquis & Huston, 2017). The structure is made of several committees tasked to lead and make decisions which will affect the care of the community. Shared governance also supports empowerment through nurse autonomy and decision making (Marquis & Huston, 2017).

The organizational theory best for this type of structure would be the contingency theory. The contingency theory allows the organization to respond to its priorities based on the needs of the organizations as well as its community  (Birken, Bunger, Powell, Turner, Clary, Klaman, Weiner, 2017). The reason this type of theory would be ideal is that the structure would facilitate the development of a committee to respond to its community’s challenges. For example, addressing issues related to the ten percent increase in heart failure admissions noted. This organizational theory will positively impact the institutional goals based on the changes that can be made that will address issues inside and outside the organization and its mission and vision.

  Mr. Jones’ current non-compliance with treatment appears to be due to a disturbed body image and lack of education regarding the aging process. Although development of arthritis is not a normal change of aging, it is something that may happen due to the natural alterations in the musculoskeletal system (Tabloski, 2014). Bones become stiff, weaker and more brittle as people age and vertebral disc thinning is likely (Tabloski, 2014) . Osteoarthritis affects over 50% of individuals over age 65, restricting self-care ability – even more so when the recommended treatment regimen is not followed (Tabloski, 2014) . The nurse caring for Mr. Jones should continue to educate him on the risks of non-compliance such as falls and hip fracture. Talking to Mr. Jones about alternatives to pain medication (such as lifestyle changes like exercise, weight loss and healthy diet) may prove to be helpful in this situation. Mr. Jones believes that if he uses a cane to get around, he will be seen as undesirable to women and that is affecting his willingness to comply with his treatment. Reassigning the patient to (or collaborating with) a younger female nurse who is empathetic to how he feels may be helpful and he may decide to listen to her advice. Anxiety disorder, OCD, or something else? 

8-year-old black male

                                                                    BACKGROUND

Tyrel is an 8-year-old black male who is brought in by his mother for a variety of psychiatric complaints. Shaquana, Tyrel’s mother, reports that Tyrel has been exhibiting a lot of worry and “nervousness” over the past 2 months. She states that she notices that he has been quite “keyed up” and spends a great deal of time worrying about “germs.” She states that he is constantly washing his hands because he feels as though he is going to get sick like he did a few weeks ago, which kept him both out of school and off the playground. He was also not able to see his father for two weekends because of being sick. Shaquana explains that although she and her ex-husband Desmond divorced about 2 years ago, their divorce was amicable and they both endeavor to see that Tyrel is well cared for. 

Shaquana reports that Tyrel is irritable at times and has also had some sleep disturbances (which she reports as “trouble staying asleep”). She reports that he has been more and more difficult to get to school as he has become nervous around his classmates. He has missed about 8 days over the course of the last 3 weeks. He has also stopped playing with his best friend from across the street. 

His mother reports that she feels “responsible” for his current symptoms. She explains that after he was sick with strep throat a few weeks ago, she encouraged him to be more careful about washing his hands after playing with other children, handling things that did not belong to him, and especially before eating. She continues by saying “maybe if I didn’t make such a big deal about it, he would not be obsessed with germs.” 

Per Shaquana, her pregnancy with Tyrel was uncomplicated, and Tyrel has met all developmental milestones on time. He has had an uneventful medical history and is current on all immunizations. 

                                                                OBJECTIVE

During your assessment of Tyrel, he seems cautious being around you. He warms a bit as you discuss school, his friends at school, and what he likes to do. He admits that he has been feeling “nervous” lately, but when you question him as to why, he simply shrugs his shoulders. 

When you discuss his handwashing with him, he tells you that “handwashing is the best way to keep from getting sick.” When you question him how many times a day he washes his hands, he again shrugs his shoulders. You can see that his bilateral hands are dry. Throughout your assessment, Tyrel reveals that he has been thinking of how dirty his hands are; and no matter how hard he tries to stop thinking about his “dirty” hands, he is unable to do so. He reports that he gets “really nervous” and “scared” that he will get sick, and that the only way to make himself feel better is to wash his hands. He reports that it does work for a while and that he feels “better” after he washes his hands, but then a little while later, he will begin thinking “did I wash my hands well enough? What if I missed an area?” He reports that he can feel himself getting more and more “scared” until he washes his hands again.  

        

                                                         MENTAL STATUS EXAM

Tyrel is alert and oriented to all spheres. Eye contact varies throughout the clinical interview. He reports his mood as “good,” admits to anxiety. Affect consistent to self-reported mood. He denies visual/auditory hallucinations. No overt delusional or paranoid thought processes were apparent. He denies suicidal ideation. 

Lab studies obtained from Tyrel’s pediatric nurse practitioner were all within normal parameters. An antistreptolysin O antibody titer was obtained for reasons you are unclear of, and this titer was shown to be above normal parameters.

There may also be an element of social phobia developing, but at this point, the PMHNP has not assessed the nature of the school avoidance—that is, why is the child avoiding school. Notice that nothing in the scenario tells us that the PMHNP has assessed this. Zoloft is FDA-approved to treat OCD in children. However, between ages 6 and 12, it should be started at 25 mg orally daily. If starting doses are too high, the child may experience side effects that he associates with the medication and as such, may refuse to take the medication. Starting at too high a dose can result in unfavorable side effects (gastrointestinal side effects are notable in this drug), and we can see that Tyrel is experiencing nausea and decreased appetite. In this case, it is recommended to wait to see if the side effects dissipate. Decreasing the dose to 12.5 mg orally daily for about 3 or 4 days, then going back to 25 mg orally daily may help to overcome the unfavorable side effects. If side effects persist, the PMHNP may need to consider switching to a different medication.Fluvoxamine controlled release is not FDA-approved for use in children with OCD (see “Special Populations: Children and Adolescents” in the Fluvoxamine monograph of Stahl’s Prescriber’s Guide for further details). Fluvoxamine 100 mg orally daily may not be tolerated in the morning secondary to the drug’s sigma-1 antagonist properties, which can cause sedation. Dosing of Fluvoxamine should be such that the larger dose is given in the evening to minimize daytime sedation. It is also worth noting that nothing in the scenario tells us that the Zoloft will not be effective.

Guidance to Student

In terms of an actual diagnosis, the child’s main symptoms are most consistent with obsessive-compulsive disorder. There may also be an element of social phobia developing, but at this point, the PMHNP has not assessed the nature of the school avoidance—that is, why is the child avoiding school. Notice that nothing in the scenario tells us that the PMHNP has assessed this.

Fluvoxamine immediate release is FDA-approved for the treatment of OCD in children aged 8 years and older. Fluvoxamine’s sigma-1 antagonist properties may cause sedation and as such, it should be dosed in the evening/bedtime.

At this point, it would be appropriate to consider increasing the bedtime dose, especially since the child is responding to the medication and there are no negative side effects.

Atypical antipsychotics are typically not used in the treatment of OCD. There is also nothing to tell us that an atypical antipsychotic would be necessary (e.g., no psychotic symptoms). Additionally, the child seems to be responding to the medication, so there is no rationale as to why an atypical antipsychotic would be added to the current regimen.

Cognitive behavioral therapy is the psychotherapy of choice for treating OCD. The PMHNP should augment medication therapy with CBT. If further assessment determines that Tyrel has social anxiety disorder, CBT is effective in treating this condition as well.

Technology will be utilized to successfully implement the postpartum hemorrhage bundle. The main way we will use technology is during simulation training. Simulation trainings using the Noelle mannequin offer life-like scenarios in the department where patient care occurs. Using simulation allows staff to work as they would in real life but does not put patient safety at risk. It also allows for debriefing afterward and identifies gaps in the workflow (Lutgendorf et al., 2017). We can also use technology to help us track postpartum hemorrhage data. We are developing a spreadsheet that tracks the number of patients who experienced hemorrhage and need for transfusion. This allows us to validate the effectiveness of our postpartum hemorrhage bundle.

This human tendency to avoid the unpleasant makes end-of-life care and hospice decisions difficult for many patients to discuss with their families. Kummer is a former social worker who used to counsel patients with terminal illnesses, yet she avoided discussing her own end-of-life wishes with her children. While many Americans, like Kummer, agree that these discussions need to take place, few have actually had these conversations with their families (Lazar, 2012). Although initiating conversations about end-of-life care and hospice might be difficult for patients, as an advanced practice nurse, facilitating these conversations is an integral part of your geriatric nursing practice. For this Discussion, consider how you would facilitate care conversations with the patients and families in the following case studies:

Case Study 1

Mrs. Sloan, a 69-year-old widow, is about to enter the hospital for an elective cholecystectomy; she is being medically cleared by her primary care provider. During the discussion, she requests to be placed on a no code status during her hospitalization. Mrs. Sloan claims that besides her gallbladder problem, her general health status is good. She wishes to have the surgery to avoid any further attacks, which have been very painful. She states, however, that if during surgery or her postoperative period, she undergoes a cardiac arrest, she would prefer not to be resuscitated. She has read about the chances of successful resuscitation, and has determined that the risk of brain damage is too high. For this reason, she is requesting a no code status.

Case Study 2

Ms. Stearns is an 83-year-old nursing home resident with hypertension, coronary artery disease, arthritis, renal insufficiency, hearing impairment, and a previous history of stroke. She also has a foot deformity from childhood polio. She is disoriented at times. She has lived in the nursing home for 10 years and rarely leaves the chair beside her bed. She has recently developed urinary incontinence, but has refused a bladder catheterization to determine postvoid residual urine or possible bladder infection. She does not have a diagnosis of dementia; however, current testing reveals that she performs poorly on a standardized mental status examination. She can, however, identify all the staff in the nursing home, and she can describe each patient who has been in the bed next to hers over the past 10 years. When asked to explain why she does not want bladder catheterization, she gives several reasons. She states that the incontinence does not bother her, and that she has always been a very private person. She particularly dislikes and objects to any examination of her pelvic organs; in fact, she has never had a pelvic examination nor has she ever had sexual intercourse. She realizes that she has a number of medical problems and that any one of them could worsen at any time. She states she is not willing to undergo any treatment for any of her current problems should they become worse.

Case Study 3

Mr. Marley, age 91, is admitted to the intensive care unit following a stroke. The stroke progressed from mild hemiparesis and difficulty speaking to complete unresponsiveness and an inability to swallow. His daughter feels certain, based on prior explicit conversations with her father, that he would not want to have any treatment that would prolong his life and leave him in a severely disabled state. Mr. Marley’s oldest son disagrees with his sister’s assessment of their father. The son claims that their father still has a strong desire to live, and that he has been very active in his church until this stroke. Because Mr. Marley cannot swallow, he cannot be fed. The family is asked about insertion of a feeding tube. It is explained to the family that without food and fluids, their father will die fairly quickly. There are no existing advance directives or a designated health care decision maker noted for Mr. Marley.

To prepare:

  • Review Chapter 14 of the Holroyd-Leduc and Reddy text.
  • Reflect on the role of the advanced practice nurse in facilitating the discussion of end-of-life care with patients and their families.
  • Think about how you, as an advanced practice nurse, would approach a family who wants “everything” done for a patient with only a limited time to live.
  • Consider when it is appropriate to involve hospice and how to approach patients and/or families who refuse hospice services.
  • Select one of the three provided case studies related to the end-of-life care of the frail elderly. Reflect on potential patient outcomes and how you would facilitate the discussion of care with this patient’s family.
By Day 3

Post an explanation of the role of the advanced practice nurse in facilitating the discussion of end-of-life care with patients and their families. Explain how you would approach a family who wants “everything” done for a patient with only a limited time to live.
