The term “knowledge worker” was first coined by management consultant and author Peter Drucker in his book, The Landmarks of Tomorrow (1959). The aim of health promotion is to improve the health of not only individuals but also populations through health education. One among other models that ensures health promotion is the Ponder’s health promotion model. This model was developed when Nola Pender noticed that health care professionals only paid attention to treating diseases but didn’t recognize the need for promotion of healthy lifestyles. In 1982 therefore, this model came into existence to provide a framework that helps to understand health promotion behaviors by recognizing the family as the unit of assessment and intervention. Through identification of available resources and fostering resilience among individual family members this theory encouraged behavioral changes that led to promotion of health among family members. The other components of this model that influences behavioral change include; recognizing experiences and characteristics of individuals, understanding behavior-specific cognition and affect and implementing behavioral outcomes (Kwong & Kwan, 2007)

As important as health teaching seems, nurses should also be aware of other variables that are likely to affects patient’s ability to learn. Such variables include; race, ethnicity, disability, sex, sex orientation, environmental threats, poverty levels, access to health care and lack of education. Other factors such as cultural, socio-economic and socio-political influences affect the patient’s experiences while other components like stereotype, biases and other forms of discrimination can limit the healthcare providers’ ability to share relevant health information. Being able to self-analyze and analyze their target populations, health providers can effectively eliminate these barriers to safeguard health promotion strategies (Whitney, 2018)

Through the transtheoretical model, health providers are able to assess the patient’s readiness to change. The different stages involved include; precontemplation, contemplation, planning, action, maintenance and termination. As the providers work with patients through this model, they are able to note any positiveness or unwillingness depending on their participation. If patients turn out to be positive, they are more actively involved in the decision making of their specific tailored care plans and this ultimately leads to effective learning outcomes as they continue in healthy living behaviors (Whitney, 2018)

Bandura’s self-efficacy theory of behavior change is a model developed by Albert Bandura that is used to initiate behavioral changes. Whitney (2018) states “He believed that feelings of self-efficacy can lead to competency; in other words, he believed that individuals have the ability to bring about their own outcomes” (Whitney, 2018, para. 30). This model emphasizes the importance of the patient feeling that he/she is capable of making changes that will lead to the desired outcomes. The patient must feel confident in their own abilities to bring about postive outcomes.

Bandura’s model helps in teaching behavioral changes by giving the patients the knowledge needed so that they can feel confident in themselves. Whitney (2018) states “This conceptual theory explains how learning is influenced by repetition, reinforcement, and symbolic modeling.learning” (Whitney, 2018, para. 30). When patients are in doubt of their own capabilities it makes it hard for them to believe that they are capable of attaining the outcome that is desired for them. However, when patients are taught through repetition and reinforcement, they gain confidence in their own abilities and that is what is needed to bring about the desired outcome.

There are many barriers that may affect a patient’s ability to learn. According to Whitney (2018) “Patients’ physical condition, including limited vision and auditory function, mobility, alertness, mental capacity, or high levels of physical pain, may impede learning capability” (Whitney, 2018, para. 18). All of these physical conditions can negatively affect the patient’s ability to learn. There are other barriers, such as language barriers, that can make it a difficult for a patient to learn, so it is imperative that there is a translator available when needed to be certain that the patient understands what is being taught. 

It is important for the nurse to assess the patient to identify their readiness to learn. If a patient is not willing and ready to learn or willing to make the needed changes then the learning outcomes will be negatively affected. The learning outcomes will be positively affected by the patient that is ready and willing to make changes. The patient must be ready to learn and change for the teaching to be successful.

Hay fever causes seasonal allergies along with a clear runny nose, post-nasal drip and irritated nares. Food allergies can cause eczema and allergic symptoms affecting the ear note and throat (Mayo Clinic, 2017). A viral infection such as the common cold can also exacerbate a runny nose, sneezing, post-nasal drainage and inflamed lymph nodes (Mayo Clinic, 2019a).

2. What additional history questions would be useful in your evaluation of Janet?

You can ask valid questions such like when did the symptoms begin? Have you been around anyone else who is sick? What makes the symptoms worse? Have you had a recent in change in environment or food? Janet is suffering from Hay fever during the changes in seasons during Spring and Fall which is when pollen counts are elevated. According to Mayo Clinic (2019) Hay fever, also called allergic rhinitis, causes cold-like signs and symptoms, such as a runny nose, itchy eyes, congestion, sneezing and sinus pressure. But unlike a cold, hay fever isn’t caused by a virus. Hay fever is caused by an allergic response to outdoor or indoor allergens, such as pollen, dust mites, or tiny flecks of skin and saliva shed by cats, dogs, and other animals with fur or feathers (pet dander).

4. Differentiate the types of hypersensitivity mechanisms.

Type I hypersensitivity is also known as immediate or anaphylactic hypersensitivity. The reaction may involve skin (urticaria and eczema), eyes (conjunctivitis), nasopharynx (rhinorrhoea, rhinitis), bronchopulmonary tissues (asthma) and gastrointestinal tract (gastroenteritis). The reaction may cause a range of symptoms from minor inconvenience to death. The reaction usually takes 15 – 30 minutes from the time of exposure to the antigen, although sometimes it may have a delayed onset of 10-12 hours (Ghaffar, 2016).

Type II hypersensitivities are also known as cytotoxic hypersensitivities and may affect a variety of organs and tissues. The antigens are normally endogenous, although exogenous chemicals (haptens) which can attach to cell membranes and lead to type II hypersensitivity. Drug-induced hemolyticanemia, granulocytopenia and thrombocytopenia are examples. The reaction time is minutes to hours. Type II hypersensitivity are primarily mediated by antibodies of the IgM or IgG classes and complement. Phagocytes and K cells may also play a role (Ghaffar, 2016).

Type III hypersensitivity is also known as immune complex hypersensitivity. The reaction may be general such as serum sickness or may involve individual organs including skin like systemic lupus erythematosus, an Arthur reaction, kidneys, lupus-nephritis, lungs-aspergillosis, blood vessels-polyarteritis, joints-rheumatoid arthritis, or other organs. This reaction may be the pathogenic mechanism of diseases caused by many microorganisms.

Type IV hypersensitivity is involved in the pathogenesis of many autoimmune and infectious diseases (tuberculosis, leprosy, blastomycosis, histoplasmosis, toxoplasmosis, leishmaniasis, etc.) and granulomas due to infections and foreign antigens. Another form of delayed hypersensitivity is contact dermatitis such as with poison ivy, and heavy metals (Ghaffar, 2016).

5. As per your analysis, what type of hypersensitivity reaction is Janet experiencing?

Allergic disorders (type I hypersensitivity) associated with asthma, hay fever, and drug reactions, as well as parasitic infections (particularly with metazoan parasites) are often cited as causes. Allergic reactions can present as the patient having and increased number in eosinophilia is an absolute increase (more than 450/μL) in the total numbers of circulating eosinophils (McCance&Huether, 2014).

An evidence-based practice paper allows you to explore best practice and help improve client outcomes on a psychiatric unit. Your paper should describe how you, as a nurse, will include – or have included – cultural awareness in a client diagnosed with a mental illness. The person you describe could be someone in your clinical setting, someone you have worked with in the past, or a theoretical client.

Remember that culture can also include gender equality, sexual orientation, and other cultures besides race and religion – including cultures unique to one particular family. Your paper should be at least 3 pages (double-spaced, not including the title or reference pages) in APA Format and include:

  • Assessment: Discuss what you would assess in regard to a client’s culture. For example, are there specific dietary requirements? Are schedule changes necessary to avoid conflicts with religious practices? Who is the spokesperson for the family? What would you assess?
  • Diagnosis: List any mental health nursing diagnoses this person has or may be at risk for. Include at least one cultural diagnosis.
  • Planning: What planning needs to be done to ensure the cultural and emotional safety of the client?
  • Implementation: What are interventions that would ensure the safety of your client in regard to culture? Include at least two interventions. For example, if your client has religious beliefs that affect the ability to take medications, what interventions would you create to ensure the client’s safety? What can you do to make sure the client’s cultural needs are met?
  • Evaluation: How will you evaluate whether your implementation was effective? Make sure the parameters are objective and measurable.
  • In your summary, discuss whether any completed interventions were successful. What could be done differently in the future? If the interventions have not yet been carried out, you might discuss some institutional changes that could be made to ensure cultural safety for all clients in that setting.
  • Your paper should utilize proper APA guidelines and include at least three scholarly sources to support your paper. A scholarly source is a source that has been peer reviewed and has appropriate authors that are credentialed.
